Effects of gradual discontinuation of selective serotonin reuptake inhibitors in panic disorder with agoraphobia.

The aim of this investigation was to explore the prevalence and features of discontinuation syndromes ensuing with gradual tapering of selective serotonin reuptake inhibitors (SSRIs), in optimal clinical conditions in patients with panic disorder and agoraphobia. Twenty-six consecutive outpatients met the DSM-IV criteria for panic disorder and agoraphobia while taking SSRIs. Twenty remitted upon behavioural treatment. Antidepressant drugs were then tapered at the slowest possible pace and with appropriate patient education. Patients were assessed with the Discontinuation-Emergent Signs and Sym

Recommendations on Switching or Deprescribing Hypnotic Medications for Insomnia.

BACKGROUND: 10-30% of the world's population suffers from insomnia. One-quarter of those who take hypnotic medication become dependent on it. Given the wide variety of available drugs, switching them is complex. The existing recommendations do not cover all drug classes. There is a need for practical, evidence-based recommendations that take account of specific aspects of care both in Europe as a whole and regionally. METHODS: This narrative review is based on publications from May 1953 to February 2026 that were retrieved by a search in six English- and German-language databases, including g

Treatment of Acute Painful Thyroiditis with Low Dose Prednisolone: A Study on Patients from Western Nepal.

INTRODUCTION: Thyroiditis is a disorder that involves inflammation of the thyroid gland. Subacute thyroiditis is the most common cause of acute painful thyroiditis. It is thought to be a viral inflammatory disorder. Subacute thyroiditis has been treated with either nonsteroidal anti-inflammatory drugs (NSAIDs) or high dose corticosteroids. The response to steroids is more dramatic and quicker than the NSAIDs. Prednisolone is the most commonly used corticosteroid to treat subacute thyroiditis. The usual dose is one mg/kg/day tapered over six weeks although the basis for this dose has not been e

Effectiveness of preventive cognitive therapy while tapering antidepressants versus maintenance antidepressant treatment versus their combination in prevention of depressive relapse or recurrence (DRD study): a three-group, multicentre, randomised controlled trial.

BACKGROUND: Keeping individuals on antidepressants after remission or recovery of major depressive disorder is a common strategy to prevent relapse or recurrence. Preventive cognitive therapy (PCT) has been proposed as an alternative to maintenance antidepressant treatment, but whether its addition would allow tapering of antidepressants or enhance the efficacy of maintenance antidepressant treatment is unclear. We aimed to compare the effectiveness of antidepressants alone, with PCT while tapering off antidepressants, or PCT added to antidepressants in the prevention of relapse and recurrence

Medico-legal rounds: Medico-legal issues and breaches of "standards of medical care" in opioid tapering for alleged opioid addiction.

OBJECTIVES: The objectives of this medico-legal case report are the following: 1) To present an example of a medico-legal problem that developed as a result of reports of alleged opioid addiction that resulted in a decision to taper a chronic pain patient (CPP) from opioids; 2) To present both the plaintiff's and defendant's expert witness opinions as to if and how the care of that patient fell below the "standard of medical care"; and 3) Based on these opinions, to develop some recommendations on how, in the future, pain medicine physicians and other physicians should proceed in similar circu

'We need more support and doctors that understand the process of tapering …': A content analysis of free-text responses to a questionnaire on discontinuing long-term benzodiazepine receptor agonist use.

BACKGROUND: Many individuals worldwide continue to take benzodiazepine receptor agonists (BZRAs) long term (≥3 months). The aim of this study was to conduct a content analysis of the views and experiences of discontinuing long-term BZRA use as documented in the free-text responses of respondents to an online questionnaire examining mediators of behaviour change relating to the discontinuation of long-term BZRA use. DESIGN: The questionnaire was disseminated via online BZRA support groups to community-based adults with either current or previous experience of long-term BZRA use. The four free-

A case of severe and prolonged γ-hydroxybutyrate (GHB) withdrawal syndrome successfully managed with a slow benzodiazepine and baclofen taper.

INTRODUCTION: γ-hydroxybutyrate (GHB) is a GABA-B agonist that rapidly produces effects that are likened to both alcohol and MDMA/ecstasy. GHB use can lead to neuroadaptation with a characteristic withdrawal syndrome. There is currently a paucity of data on the progression of GHB withdrawal, however, due to the drug's short half-life it is generally considered to be typically 5-7 days, although some cases can be severe and complicated by life threatening delirium. Here, we present a case of severe GHB withdrawal, which recurred on multiple occasions over 56 days, despite initial clinical stabi

Proton Therapy Mediates Dose Reductions to Brain Structures Associated With Cognition in Children With Medulloblastoma.

PURPOSE: Emerging evidence suggests proton radiation therapy may offer cognitive sparing advantages over photon radiation therapy, yet dosimetry has not been compared previously. The purpose of this study was to examine dosimetric correlates of cognitive outcomes in children with medulloblastoma treated with proton versus photon radiation therapy. METHODS AND MATERIALS: In this retrospective, bi-institutional study, dosimetric and cognitive data from 75 patients (39 photon and 36 proton) were analyzed. Doses to brain structures were compared between treatment modalities. Linear mixed-effects

Photon-counting CT for diagnosis of acute pulmonary embolism: potential for contrast medium and radiation dose reduction.

OBJECTIVE: To evaluate the image quality of an ultra-low contrast medium and radiation dose CT pulmonary angiography (CTPA) protocol for the diagnosis of acute pulmonary embolism using a clinical photon-counting detector (PCD) CT system and compare its performance to a dual-energy-(DE)-CTPA protocol on a conventional energy-integrating detector (EID) CT system. METHODS: Sixty-four patients either underwent CTPA with the novel scan protocol on the PCD-CT scanner (32 patients, 25 mL, CTDIvol 2.5 mGy·cm) or conventional DE-CTPA on a third-generation dual-source EID-CT (32 patients, 50 mL, CTDIvo

Evaluation of thyroid radiation dose during abdominal Computed Tomography procedures and dose reduction effectiveness of thyroid shielding.

INTRODUCTION: During abdominal Computed Tomography (CT) studies, vicinity organs receive a dose from scatter radiation. The thyroid is considered an organ at greater risk due to high radiosensitivity. METHODS: The primary objective of this study was to determine the entrances surface dose (ESD) to the thyroid during abdominal CT studies and to evaluate the efficiency of dose reduction by lead shielding. The calibrated thermoluminescence dosimeter (TLD) chips were used to measure the ESD during 180 contrast-enhanced (CE) and non-contrast-enhanced (NC) abdominal CT studies in the presence and a

Dose reduction potential in diagnostic single energy CT through patient-specific prefilters and a wider range of tube voltages.

PURPOSE: Various studies have demonstrated that additional prefilters and/or reduced tube voltages have the potential to significantly increase the contrast-to-noise ratios at unit dose (CNRDs) and thereby to significantly reduce patient dose in clinical CT. An exhaustive analysis, accounting for a wide range of filter thicknesses and a wide range of tube voltages extending beyond the 70 to 150 kV range of today's CT systems, including their specific choice depending on the patient size, is, however, missing. Therefore, this work analyzes the dose reduction potential for patient-specific selec

Comparison of viral control between two tenofovir dose reduction regimens (300 mg every 48 hours versus 300 mg every 72 hours) in chronic hepatitis B patients with moderate renal impairment from tenofovir-induced renal dysfunction.

Long-term use of tenofovir disoproxil fumarate (TDF) can induce renal dysfunction that requires TDF dose reduction. Previous studies showed that systemic drug use exerts a threefold higher risk of moderate renal impairment. This study aimed to compare viral control between two tenofovir dose reduction regimens in chronic hepatitis B (CHB) patients with moderate renal impairment from TDF-induced renal dysfunction. This noninferiority, randomized controlled study was conducted at the Faculty of Medicine Siriraj Hospital, Mahidol University, Bangkok, Thailand. Virologically suppressed CHB patient

Efficacy and safety of an early response-based tapering regimen of tocilizumab in children with systemic juvenile idiopathic arthritis.

INTRODUCTION: Systemic juvenile idiopathic arthritis (sJIA) is a distinct disease subset, with a poorer prognosis compared with other JIA subsets. Tocilizumab has an important role in the management of sJIA refractory to standard initial therapy. However, no specific guidelines exist for the tapering of tocilizumab therapy in sJIA, which could have implications on the overall cost and side effects of treatment. METHODS: This was an observational study which included 21 children with refractory sJIA, who were initially put on injection tocilizumab every 2 weekly, with subsequent dosing tapered

Therapeutic issues in oral glucocorticoid use.

The therapeutic use of glucocorticoids are extensive despite numerous adverse effects related to their use. The fear of hypothalamic-pituitary-adrenal (HPA) axis suppression and adrenal crisis often prolongs glucocorticoid therapy unnecessarily. Glucocorticoids are the most common cause of drug-induced osteoporosis and cataract formation. Other effects of concern to the primary care provider include increased blood pressure, blood glucose, and cholesterol. Glucocorticoid therapy also may lead to avascular necrosis, growth retardation, and myopathy. Practical dosing strategies such as alternate
