Persistent dysarthria with apraxia associated with a combination of lithium carbonate and haloperidol.

Reported is a 19-year-old manic-depressive patient who developed persistent dysarthria with coexisting apraxia while on a combination of high dose haloperidol and lithium carbonate. The speech disability occurred as a solitary symptom in a patient with normal serum lithium levels and no other signs or symptoms of lithium toxicity and persisted after lithium was discontinued and the neuroleptic changed. There were several factors which favored an association between the speech disability and the drug therapy. These included improvement during a drug-free trial: the absence of a prior history of

Phonemic behavior of aphasic subjects without dysarthria or apraxia of speech.

Thirty adult aphasic subjects without apraxia of speech or dysarthria were tested for their ability to produce phonemes in single test words and in spontaneous contextual speech. Results indicated that 75% of the total phonemic errors were due to a whole-word phenomenon apparently associated with faulty processing of the word rather than faulty production of the phoneme. True phonemic errors comprised 25% of the total errors or about 2% of all responses. Phoneme substitutions were by far the most frequent error (61%). Of the 30 subjects, 28 made no phonemic errors in spontaneous contextual spe

Digital profiling of dysarthria in late- and early-onset Parkinson's disease.

BackgroundDigital speech analysis affords robust markers of Parkinson's disease (PD). However, most studies target late-onset PD (LOPD), neglecting early-onset PD (EOPD) -an increasingly prevalent subtype. This proof-of-concept study tackles such gap.MethodsWe used machine learning to discriminate persons with EOPD (with symptom onset before age 50) and LOPD (with symptom onset after age 50) from healthy controls (HCs) through prosodic and articulatory features from natural speech.ResultsMaximal classification between patients and HCs was afforded by combined prosodic and articulation features

A novel Swin transformer based framework for speech recognition for dysarthria.

Dysarthria frequently occurs in individuals with disorders such as stroke, Parkinson's disease, cerebral palsy, and other neurological disorders. Well-timed detection and management of dysarthria in these patients is imperative for efficiently handling the development of their condition. Several previous studies have concentrated on detecting dysarthria speech using machine learning-based methods. However, the false positive rate is high due to the varying nature of speech and environmental factors such as background noise. Therefore, in this work, we employ a model based on the Swin transform

Volumetric and diffusion MRI abnormalities associated with dysarthria in multiple sclerosis.

Up to half of all people with multiple sclerosis experience communication difficulties due to dysarthria, a disorder that impacts the motor aspects of speech production. Dysarthria in multiple sclerosis is linked to cerebellar dysfunction, disease severity and lesion load, but the neuroanatomical substrates of these symptoms remain unclear. In this study, 52 participants with multiple sclerosis and 14 age- and sex-matched healthy controls underwent structural and diffusion MRI, clinical assessment of disease severity and cerebellar dysfunction and a battery of motor speech tasks. Assessments o

Dysarthria Detection with Deep Representation Learning for Patients with Parkinson's Disease.

Dysarthria is a very common motor speech symptom in Parkinson's disease impairing normal communications of patients. Detection of dysarthria could assist clinical diagnosis and intervention of Parkinson's disease, provide monitoring approach for treatment-related side effects, and lead to effective speech therapy to prevent further communication and social deficits. Applying machine learning techniques to speech analysis for patients offers more resource-efficient, accessible and objective tools for screening and assessment of dysarthria. In this study, we constructed a multi-task speech datas

Isolated dysarthria as the sole manifestation of myasthenia gravis: a case report.

Myasthenia gravis (MG) is an acquired autoimmune disease. Its clinical manifestations comprise ptosis, diplopia, dysarthria, dysphagia, limb weakness, and in severe cases, respiratory muscle involvement. Dysarthria as an exclusive initial and primary complaint in MG is rare and seldom reported. In this paper, we report a case of type IIIb MG with isolated dysarthria as the only clinical manifestation and we review the relevant literature. The patient was a 62-year-old man who presented with episodes of slurred speech for 20 days that had worsened in the previous 9 days. His medical history com

Ataxia and dysarthria due to an ABCA2 variant: Extension of the phenotypic spectrum.

INTRODUCTION: Ataxias are heterogeneous disorders that are caused by variants in a large number of genes. The study was conducted to identify the molecular basis of a movement disorder in a consanguineous Pakistani family. METHODS: We performed clinical assessments and magnetic resonance imaging of the older of two siblings affected with a movement disorder. Molecular analyses included whole-exome sequencing in order to delineate the underlying pathology of the disorder. Segregation of variants with the phenotype was checked by Sanger sequencing. RESULTS: Symptoms of the two affected subject

When the word doesn't come out: A synthetic overview of dysarthria.

Motor speech disorders are common in a number of neurological conditions including diseases involving impairment of the pyramidal, extrapyramidal, and cerebellar pathways, cranial nerves, muscular apparatus, neuromuscular plaque, and of cognitive, symbolic and mnestic activities. The diagnosis of speech disorders, namely the dysarthrias, involves the assessment of characteristic structural cerebral, prosodic, phonetic and phonemic changes, often flanked by concomitant functional, clinical, neuroradiological, neurophysiological and behavioral impairment. This paper presents a brief outline of t

[A case of sigmoid colon cancer with temporary dysarthria associated with irinotecan].

A40 -year-old woman visited our hospital with adenocaricinoma of the sigmoid colon with multiple liver metastases and ovarian metastasis. Because of a stenosis of the primary tumor, she underwent a colostomy before chemotherapy. 5-fluorouracil and irinotecan and leucovorin(FOLFIRI)was selected as first-line chemotherapy. At the start of chemotherapy, just after the end of irinotecan and leucovorin administration, the patient developed dysarthria. There were no neurological abnormalities or hematological abnormalities. The treatment was temporarily discontinued, and the dysarthria completely di

The impact of stroke-related dysarthria on social participation and implications for rehabilitation.

INTRODUCTION: Each year an estimated 30,000-45,000 UK individuals experience stroke-related dysarthria (impairment of movements required to produce speech). Many will experience persistent dysarthria long after discharge from stroke services. Although we have some insight into the impact of other communication impairments, we have very limited information on the impact of dysarthria on social participation. PURPOSE: To explore the impact of dysarthria on social participation following stroke. METHODS: We report data from in-depth semi-structured interviews with 24 individuals with stroke-rel

A phonation therapy approach for Mandarin-English bilingual clients with dysarthria.

This study aims to measure the speech intelligibility of Mandarin-English speakers with dysarthria before and after phonation therapy, in order to determine the effectiveness of this approach. A within-group design was used with two case studies which allowed one to measure therapy variables (single word and sentences); language variables (Mandarin and English); and speech production variables (respiration, phonation, articulation, resonance, and prosody). Both participants demonstrated highly significant improvement in Mandarin intelligibility scores after therapy compared with minimal change

Myotonia and flaccid dysarthria in patients with adult onset myotonic dystrophy.

BACKGROUND: Myotonia and weakness are the most important components of dysarthric speech in myotonic dystrophy. OBJECTIVE: To specify and quantify possible defects in speech execution in patients with adult onset myotonic dystrophy. METHODS: Studies on speech production were done on 30 mildly affected patients with myotonic dystrophy. Special attention was paid to myotonia. Because muscle activity can result in a decrease of myotonia, speech characteristics were measured before and after warm up. The possibility that warming up causes increased weakness was also assessed. RESULTS: As with o

Voice dysfunction in dysarthria: application of the Multi-Dimensional Voice Program.

Phonatory dysfunction is a frequent component of dysarthria and often is a primary feature noted in clinical assessment. But the vocal impairment can be difficult to assess because (a). the analysis of voice disorder of any kind can be challenging, and (b). the voice disorder in dysarthria often occurs along with other impairments affecting articulation, resonance, and respiration. A promising assessment tool is multi-parameter acoustic analysis, such as the Multi-Dimensional Voice Program (MDVP). Part 1 of this paper recommends procedures and standards for the acoustic analysis of voice, incl

[A 63 year-old man with progressive gait disturbance and dysarthria].

We report a 63-year-old man with progressive gait disturbance and dysarthria. The patient was apparently well until the age of 62 (February, 1990) when he noted unsteadiness of gait. Two months later, dysarthria appeared. He was admitted to Juntendo Izunagaoka Hospital on April 23, 1990. Neurologic examination revealed a mentally sound man with normal higher cerebral functions. Cranial nerves were unremarkable except for scanning speech. His gait was ataxic with positive Romberg sign. No motor weakness was noted, however, he had hypotonia and cerebellar ataxia. Deep tendon reflexes were retain

Resolution of dysarthria in multiple sclerosis by treatment with weak electromagnetic fields.

It has been reported that 50% or more of patients diagnosed with multiple sclerosis (MS) exhibit speech impairment (dysarthria) which in some cases can be exceedingly disabling. Currently there is no effective medical treatment for the dysarthria of MS which occurs as a result of lesions to the cerebellum and its outflow tracts. It was reported recently that extracranial application of brief AC pulsed electromagnetic fields (EMFs) in the picotesla (pT) range intensity produced in patients with MS sustained improvement in motor functions including cerebellar symptomatology. This communication c
