Adamantiades-Behçet's disease with inner ear involvement.

Adamantiades-Behçet's disease is a chronic recurrent inflammatory disorder involving the small and large vessels. Typical loci of manifestations are the mucous membranes, skin and eyes, as well as the joints and central nervous system. Other organs are not commonly involved. We present two patients, one with ocular and the other with mucocutaneous manifestation of Adamantiades-Behçet's disease. In addition, the first patient reported three episodes of sudden hearing loss while under immunosuppressive therapy for his eye involvement. The second, therapy-naive patient complained of tinnitus in h

Autoimmune inner ear disease--a real entity?

Involvement of autoimmune mechanisms in sudden-onset, rapidly progressive, bilateral inner ear disease is supported by the following evidence: (1) the inner ear contains immune cells and mediators (immunoglobulins); (2) animal models demonstrate inner ear damage after immunization with inner ear tissue; (3) experimental autoimmune inner ear disease appears to be transferable with sensitized T cells; (4) human SNHL can occur in the context of systemic immunologic disease; (5) SNHL can be improved by immunosuppressive therapy; and (6) patients with SNHL demonstrate elevated immune responses to i

Inner ear disorders due to pressure change.

We reviewed the records of 136 patients who had inner ear disorders including hearing loss and vertigo caused by pressure change. We divided them into three groups, according to the aetiology: group A, change in atmospheric pressure (diving, airplane travel, etc.); group B, rapid change in ear pressure in normal atmosphere (nose blowing, heavy lifting, etc.); and group C, blast injury. A flat initial audiogram was the most common type in groups A and B. In group C, high-tone hearing loss was the most common type of audiogram. These results correspond to findings previously reported in animal e

Inner ear involvement in mixed cryoglobulinaemia patients.

In order to evaluate the nature and prevalence of audiovestibular disturbances in mixed cryoglobulinaemia (MC), 32 consecutive MC patients were studied by a wide audiological and vestibular examination. Pure tone audiometry, impedance audiometry, brainstem response audiometry and vestibular function were performed. Patients with a previous history of ear damage due to other well-known agents were excluded from the study. In MC patients we found a rather frequent audiovestibular involvement (34.3%). Bilateral sensorineural hearing loss was found in seven MC patients (22%) and altered vestibular

[Hemodilution therapy of acute inner ear damage].

Two independently conducted randomized clinical studies (sudden deafness; acute acoustic trauma) showed that in terms of hearing gain and the elimination of tinnitus low-molecular dextran 40 and 6% hydroxyethyl starch 40/0.5 do not produce any significant difference in therapy. Clear significant results were obtained with patients who suffered from acute acoustic trauma, when only NaCl-infusions were applied. Both rheologically effective substances show side effects, but they lie in the per-mil-range. On the grounds of measured clinical results we refuse zero or next to zero therapy of patient

Clinical treatment of vascular inner ear diseases.

A multitude of general disorders of the vascular system may also affect the blood circulation of the cochlea and cause symptoms such as fluctuating or permanent hearing loss. Such is the case for arteriosclerosis combined with hypertension or hypotension, collagenosis, and diabetes. Blood disorders, like leukemia, sickle cell anemia, and polycythemia, and infectious diseases involving the blood vessels, such as lues, may also present their primary symptoms in the ear. The otorhinolaryngologist must be able to establish the correct diagnosis and refer patients requiring more general treatment t

Histopathology of chloroform-induced inner ear damage.

Inner ear function loss was caused in guinea pigs and rats by injecting chloroform into the middle ear. After symptoms for cochlear and vestibular deficit had been registered, the animals were permitted to survive for one day to five months. Ear histopathology was then studied in celloidin sections. In both species, hair cells and afferent nerve fibers were intact at all survival times. The acute stage of functional loss in guinea pigs was associated with inner ears of normal histological appearance. Within days after chloroform injection a severe otitis media developed which led to fibrous oc

Inner ear damage induced by Mycobacterium fortuitum.

We used electron microscopy to investigate Mycobacterium fortuitum-induced changes in the inner ears of mice. We found that the inner and outer hair cells had degenerated and disappeared in the organ of Corti. Changes in the lower turn of the cochlea were more severe than those of the upper turn while the changes of the outer hair cells were more severe than those of the inner hair cells. Disappearance, fusion and ballooning of the sensory hairs were observed in the vestibular organs. The bacterial extract also induced inner ear damage which was similar to that caused by live M. fortuitum.

Complicated cholesteatomas: CT findings in inner ear complications of middle ear cholesteatomas.

Patients with facial palsy and middle ear disease, which may be chronic but clinically occult, may have a cholesteatoma with extension medially along the facial canal. In two patients, axial computed tomographic (CT) scans demonstrated involvement of the medial petrous bone. Patients with vertigo and chronic middle ear disease may have a cholesteatoma with a "fistula" between the middle and inner ears. Although the fistula usually involves the lateral semicircular canal, the cholesteatoma may pass through the oval window. In two patients, coronal CT scans showed extension to the oval window in

A syndrome with mixed deafness, Mozart ear, middle and inner ear dysplasias.

A 10-year-old boy presenting a disorder of multiple malformations with an otologic component consisting of mixed deafness, Mozart ear, thin stapes, small oval window and agenesis of the semicircular canals, was studied. A comparative analysis of similar malformation-entities has permitted the delineation of a previously undescribed syndrome.

A perspective from magnetic resonance imaging findings of the inner ear: Relationships among cerebrospinal, ocular and inner ear fluids.

Visualization of endolymphatic hydrops has been performed using magnetic resonance imaging (MRI) after intratympanic or intravenous gadolinium (Gd) injection. Our recent findings indicate that just as the prevalence of asymptomatic glaucoma is greater than that of symptomatic glaucoma, there are also many cases of asymptomatic endolymphatic hydrops. It is assumed that the asymptomatic endolymphatic hydrops that precedes Ménière's disease is found more frequently using MRI than with other techniques. Gd in the inner ear moves into the cerebrospinal fluid (CSF) via the internal auditory meatus.

Transplantation of Ears Provides Insights into Inner Ear Afferent Pathfinding Properties.

Numerous tissue transplantations have demonstrated that otocysts can develop into normal ears in any location in all vertebrates tested thus far, though the pattern of innervation of these transplanted ears has largely been understudied. Here, expanding on previous findings that transplanted ears demonstrate capability of local brainstem innervation and can also be innervated themselves by efferents, we show that inner ear afferents grow toward the spinal cord mostly along existing afferent and efferent fibers and preferentially enter the dorsal spinal cord. Once in the dorsal funiculus of the

Neurotransmission of the cochlear inner hair cell synapse--implications for inner ear therapy.

The cochlear inner hair cells (IHCs) are connected to afferent type I auditory neurons and use probably L-glutamate as a neurotransmitter. This IHC synapse receives efferent input from the lateral part of the efferent olivocochlear system with neurons originating in the brainstem and terminating below IHCs synapsing with the afferent type I dendrites. A number of substances have been proposed to function as neurotransmitter or neuromodulator in the lateral efferent system: acetylcholine, gamma-aminobutyric acid (GABA), dopamine, enkephalin and dynorphin. With the aid of microiontophoretic tech

Hybrid Ear Cubes for local controlled dexamethasone delivery to the inner ear.

A new type of miniaturized implants for local controlled drug delivery to the inner ear is proposed: Hybrid Ear Cubes. They are composed of two main parts: (i) a cylinder, which is placed into a tiny hole (<0.4 mm) drilled into (or close to) the oval (or round) window, and (ii) a cuboid, which is placed into the middle ear. The drug is released at a pre-programmed rate into the perilymph: (i) via the cylindrical part of the implant, which is in direct contact with this liquid, and (ii) via diffusion from the cuboid through the oval/round window. Importantly, the cylindrical part assures a reli

Mucosal melanocytic lesion in the middle ear extending to the inner ear and nasopharynx.

Benign dendritic melanocytic proliferation is usually observed in the skin. We report an extremely rare case of a melanocytic lesion in the middle ear mucosa. Only 3 cases of melanocytic lesions in the middle ear have been reported. Our report is the only one that describes an adult with a melanocytic lesion in the middle ear that extended to the inner ear and nasopharyngeal mucosa. A 23-year-old female presented with profound sensory neural hearing loss and recurrent discharge from the right ear. Examination revealed a blue right tympanic membrane with a small perforation. [The effect of OSAHS on middle ear and inner ear vestibule function advances].

Obstructive sleep apnea hypopnea syndrome(OSAHS) as a common frequentlyoccurring disease, it can cause repeated episodes of hypoxaemia and hypercapnia during sleep. With long period of hypoxaemia, obvious pathological changes and dysfunction emerged in heart, brain and lung then all kinds of clinical symptoms appear. Because of the middle ear and inner ear themselves anatomical characteristics and blood supply of regulating mechanism, they often has been damaged before the other important organ damage. As scholars have indepth study of the auditory system complications in patients with OSAHS,

Toxic inner ear lesion following otitis media with effusion: a comparative CT-study regarding the morphology of the inner ear.

Viral infections of the upper respiratory airways can lead to a delayed viral otitis media (VOM) caused by a diffusion of viruses/virus particles through the round window membrane and resulting in sensorineural hearing loss. The treatment of choice is immediate paracentesis, evacuation of all fluids from the middle ear cavity, and haemorrheological infusions. However, in some cases, persistent symptoms may be an indication for a surgical approach using mastoidectomy. In high-resolution computed tomography, an extended small-sized pneumatisation of the mastoid cells with complete shading was fo

Echoes of Renewal: Rebuilding the Mammalian Inner Ear.

Hearing loss and dizziness are the most common sensory disorders worldwide. About 5% of the world's population is affected by disabling hearing loss, and the prevalence is projected to increase over the next several decades. Cochlear hair cells are essential in converting sound vibrations into electrical signals, which are then relayed centrally via the spiral ganglion neurons and lead to sound perception. The irreversible loss of cochlear hair cells is the most common pathology underlying sensorineural hearing loss in humans. In stark contrast, nonmammalian species such as chicken and zebrafi
