An Assistive Role of a Machine Learning Network in Diagnosis of Middle Ear Diseases.

The present study aimed to develop a machine learning network to diagnose middle ear diseases with tympanic membrane images and to identify its assistive role in the diagnostic process. The medical records of subjects who underwent ear endoscopy tests were reviewed. From these records, 2272 diagnostic tympanic membranes images were appropriately labeled as normal, otitis media with effusion (OME), chronic otitis media (COM), or cholesteatoma and were used for training. We developed the "ResNet18 + Shuffle" network and validated the model performance. Seventy-one representative cases were selec

High-mobility group box protein 1 expression in inflammatory diseases of the middle ear.

High-mobility group box 1 (HMGB1) is a nuclear non-histone protein, playing a critical role as a mediator between innate and acquired immunity; when released extracellularly, it coordinates the cellular stress response (under necrosis, bacterial lipopolysaccharide stimulation) and acts as an inflammatory marker and cytokine. The aim of the study was to demonstrate whether HMGB1 is over-expressed in chronic middle-ear pathologies and whether the entity of expression and the localization are correlated with the degree of the inflammatory reaction, thus suggesting that HMGB1 may play a crucial ro

[Treatment of patients with acute and chronic suppurative middle ear diseases using ozone preparations].

Methods of clinical application of gaseous ozone and ozonated solutions are described with special reference to the treatment of patients with acute and chronic suppurative middle ear diseases. Results of otoscopy, microbiological and cytological studies confirm therapeutic efficiency of the above techniques as accelerating recovery of patients with this pathology. It is recommended to include ozonotherapy as an additional method in the combined treatment of acute and chronic suppurative otitis media.

[Pressure relations between endocranial and intracochlear fluid spaces in patients with inner ear diseases].

Intracranial pressure is transmitted to the perilymph of the cochlea via the cochlear aquaeduct and via Reissner's membrane to the endolymph. The "Tympanic Membrane Displacement Analyser (TDA)" is a new device that may be a useful non-invasive method to show intracranial and intracochlear pressure changes indirectly measured by tympanic membrane displacement during stapedial reflex contraction. The TDA was utilised in 20 normal persons and in 29 patients with unilateral diseases of the inner ear. No significant differences in the tympanic membrane displacement were found between patients with

[Nimodipine (Bay e 9736) a new therapy concept in diseases of the inner ear?].

In an open study 30 patients with the symptom of tinnitus were treated for 12 weeks with nimodipin (Bay e 9736) 3 X 30 mg/die. 12 patients showed sudden deafness with a history of one day up to more than 20 years. In six cases we observed complete remission, in two cases remission up to 70-80%, in two cases slight improvement. Four patients showed no side reaction to the treatment of nimodipin. Ten patients suffered from so-called degenerative inner ear deafness. 3 (n = 10) showed complete remission, one nearly 90% improvement, another patient showed 60% improvement, one slight improvement, in

Ossiculoplasty in the Setting of Erosive Middle Ear Disease.

Erosive middle ear disease (EMED) involves a spectrum of otologic conditions that involve chronic inflammatory processes, including chronic otitis media with or without cholesteatoma, that contribute to osseous erosion of the middle ear and ossicular chain. Treatment of EMED involves the eradication of the disease followed by ossicular chain reconstruction to optimize hearing outcomes. There are many challenges regarding ossiculoplasty in EMED, including the potential for microscopic residual disease, variability of ossicular chain status following disease eradication, and concern for recurren

Effects of Body Weight on Middle Ear Disease.

Body weight can have various effects on middle ear health. Pediatric obesity is associated with an increased risk and recurrence of otitis media with effusion. Adult obesity is associated with an increased risk of temporal bone cerebrospinal fluid leak and encephalocele, and weight loss can cause symptoms of patulous Eustachian tube dysfunction. With rising rates of obesity and widespread adoption of glucagon-like peptide 1 receptor agonists, it is increasingly important that patients are appropriately counseled on how weight changes affect the middle ear. In this review, we examine the litera

Sinonasal and ear manifestations of Erdheim-Chester disease.

To calculate the prevalence of sinonasal and ear involvement in an Erdheim-Chester disease (ECD) population, to describe the different ear, nose and throat (ENT) manifestations and to study the association between ENT involvement, other organ involvement, and BRAF mutations. We led a retrospective monocentric study in the national referral center for ECD. One hundred and sixty-two patients with ECD and ENT data were included between January 1, 1980 and December 31, 2020. Ear and nose clinical and radiological findings were noted. We described and studied the prevalence of ENT involvement in EC

Endoscopic Management of Pediatric Chronic Ear Disease.

Pediatric chronic ear disease and its sequelae often necessitate surgical intervention, such as ear tube insertion, ossiculoplasty, tympanoplasty, and cholesteatoma removal. Although these procedures have traditionally been performed with the microscope, use of rigid endoscopes provides an alternative method for visualization. The endoscope offers improved visualization of the middle ear space and adjacent structures and can either be used alone to perform surgery through the ear canal or together with the microscope if mastoidectomy is required. Endoscopic ear surgery can reduce the need for

Neurological manifestations of ear disease in dogs and cats.

There are four major neuroanatomical structures associated with the ear that, when damaged, result in different neurologic clinical signs. These structures are the facial nerve, the ocular sympathetic tract, the vestibular receptors, and the cochlea. The clinical signs associated with disorders of each structure are discussed, followed by a summary of the diseases that should be considered in each case. The article begins with a description of the neuroanatomy of each of these structures.

Foot and ear disease--the dermatophytid reaction in otology.

Chronic otitis external (COE) is occasionally refractory to treatment and may persist for years. In such cases, a dermatophytid (id) reaction from a fungal infection in a remote location should be suspected. Immunotherapy with dermatophyte (Trichophyton, Oidiomycetes, and Epidermophyton [T.O.E.]) extracts and dust mite, based on serial endpoint titration skin test results, as well as a yeast elimination diet, is the treatment of choice. Fourteen cases of id reaction are described. The primary fungal source was not always obvious, but immunotherapy and diet led to complete resolution of the COE

Detection of inner ear disease autoantibodies by immunoblotting.

To define further the character of autoantibodies against the inner ear in patients with inner ear disease, Autoantibodies in sera from 82 patients with inner ear disease were investigated by immunoblotting. The inner ear antigens were extracted from Hartley guinea pigs. Brain, kidney, lung, heart and liver extracts were also prepared. Antibodies against the inner ear were found in 32 of 82 (39%) patients with inner ear disease. These sera reacted with the 30 and 58 kDa bands of the inner ear extracts. The 30 kDa band was detected in sera from patients with various inner ear diseases, while th

Clinical diagnosis of immune inner-ear disease.

The purpose of this study was to develop a high-risk clinical profile of patients with autoimmune (immune-mediated) inner-ear disease. The records of 52 patients diagnosed over the past 5 years were reviewed. Age, sex, bilateral versus unilateral involvement, otologic symptoms, concomitant systemic immune disease, and presenting clinical diagnoses were recorded. The presenting diagnoses were Cogan's syndrome, Meniere's syndrome, Dandy's syndrome without hearing loss, or progressive sensorineural hearing loss without dizziness. Because Cogan's and Dandy's syndromes were relatively uncommon, the

The microbial environment of the ear canal in health and disease.

Otitis externa is an important disease of dogs and, to a lesser degree, cats. The yeast Malassezia canis is the most common organism demonstrated in ear infections. Coagulase-positive staphylococci are the most common bacteria isolated, often occurring with M. canis. Pseudomonas aeruginosa and Proteus are gram-negative bacteria that are frequently encountered. Other bacteria, such as beta-hemolytic streptococci, enterococci, Escherichia coli, and Corynebacterium are also important. Malassezia canis, coagulase-positive staphylococci, and Pseudomonas aeruginosa commonly occur as monoinfections,

Unilateral Meniere's disease: is the contralateral ear normal?

Certain investigations in patients with unilateral Meniere's disease may on occasion show abnormalities in the completely symptomless contralateral ear. These tests include transtympanic electrocochleography, the acetazolamide cochlear hydration test, vestibular aqueduct tomography, and caloric testing. Eventually these ears may well become symptomatic. Previous studies have shown that otoadmittance changes are a sensitive indicator of glycerol-induced intracochlear pressure alterations in hydropic ears, but do not occur in patients without Meniere's disease. Otoadmittance parameters were eval
