Partial sequence analysis of B2L gene of Brazilian orf viruses from sheep and goats.

We herein describe the partial nucleotide sequencing and phylogenetic analysis of the B2L gene of seventeen Brazilian orf viruses (ORFV). Seventeen viruses were recovered from outbreaks of contagious ecthyma in sheep and goats in four states in Southern and Northeast country, and three from commercial vaccines. Most analyzed viruses were associated with outbreaks of classical contagious ecthyma, with lip, nostrils and labial commissure involvement, yet udder/teat, feet, vulvar and disseminated lesions were also reported in some cases. Nucleotide sequence analysis revealed a high degree of B2L

Pseudomonas sepsis with Noma: an association?

We report here a 2.5-year-old male child with community-acquired Pseudomonal sepsis showing the characteristic lesions of ecthyma gangrenosum. The child had development of gangrenous changes of the nose and face - the 'cancrum oris' or 'Noma'. We highlight the possible association of Pseudomonas sepsis and Noma, with malnutrition playing a central role in causing both the diseases.

Methylmalonic aciduria presenting with recurrent multiple molluscum contagiosum lesions.

Methylmalonic aciduria is a rare metabolic disorder with autosomal recessive inheritance. Patients with methylmalonic aciduria develop a variety of skin manifestations: superficial scalded skin lesions, alopecia, psoriasiform eruptions, acrodermatitis enteropathica-like lesions, fulminant ecthyma gangrenosum and ichthyosis. We report a Japanese boy, diagnosed with methylmalonic aciduria, who suffered from recurrent multiple molluscum lesions around the eyes.

Palatopharyngeal and lower extremity soft tissue loss in an infant secondary to Pseudomonas gangrenous cellulitis.

Ecthyma gangrenosum, the rapidly fulminating tissue infarction of Pseudomonas sepsis, presents as one of the more spectacular disease processes. A case involving sloughing of the soft palate, pharyngeal walls, and 40% of one extremity in an infant is presented. Fever and diarrhea are the usual nondescript precursors. The lesions are impressive for their lack of visible inflammatory response or granulation tissue. Early debridement is mandatory for resolution of the toxic condition.

[Locoregional polymorphous Pseudomonas aeruginosa skin infection].

A 72-year-old diabetic woman was referred with a painful chronic leg ulcer associated with venous and arterial insufficiency. She then developed a polymorphous skin infection due to Pseudomonas aeruginosa, with ecthyma gangrenosum, subcutaneous abscess and panniculitis of the homolateral inferior limb, without general sepsis. Although P. aeruginosa infection may induce polymorphous skin lesions, they are most often observed in immunocompromised patients following septicaemia.

Non healing legs ulcers infected with Stenotrophomonas maltophilia.

Stenotrophomonas maltophilia (S.M.) is a Gram-negative bacillus, naturally resistant to cephalosporins and carbapenems, which can colonize different sites and may be responsible for serious infections for which treatment is a real challenge. This was rarely reported as cutaneous pathogenic organism causing cellulitis-like lesions, paronychia, mucocutaneous ulcers and ecthyma gangrenosum in immunocompromised individuals. We report a case of non-healing legs ulcer infected with Stenotrophomonas maltophilia.

Pseudomonas aeruginosa infections of intact skin.

Pseudomonas aeruginosa infections of healthy skin are uncommon. We report four cases of P. aeruginosa infections of intact skin. These cases illustrate the clinical spectrum of these cutaneous infections: localized, mild epidermal infections (the green nail syndrome and webbed space infections), moderately serious infections (cutaneous folliculitis and otitis externa), and, in immunocompromised patients, extremely serious infections (malignant otitis externa, perirectal infection, and ecthyma gangrenosum).

[Infectious cutaneous gangrene in infants].

The authors review 6 cases of severe cutaneous gangrene complicating infectious diseases in more or less debilitated infants. The evoked diagnoses were 1 case of septicemic gangrene, 4 cases of extensive cutaneous necrosis of the infant, and 1 severe case of ecthyma gangrenosum. However, the authors believe this classic nosological background while being precise, is restricting and poses the problem of knowing how to classify necrotizing fasciitis.

[Mucormycosis cutanea- clinical case presentation souvenir etiopatogenia diagnosis and treatment].

Mucormycosis is serious emerging infection, caused by saprophytic fungi of the order Mucorales, which mainly affects immunocompromised patients. Presentation forms can be rinosinusal, pulmonary, gastrointestinal, disseminated and localized wounds and burns. The cutaneous presentation is caused by inoculation of spores into the dermis with the subsequent development compatible with gangrenous ecthyma. Our patient was treated with surgical lesion and antifungal cleaning. Presenting hemodynamic complication secondary to the administration of posaconazole.

A 4-month-old male baby with an orf lesion on his nose: a rare case report.

BACKGROUND: Ecthyma contagiosum, sometimes referred to as human orf, is a zoonotic disease caused by the orf virus that is mostly acquired by coming into contact with diseased animals such as sheep or goats. The orf virus, a DNA virus belonging to the Poxviridae family, infects epidermal keratinocytes via breaking down the skin barrier, which can be caused by burns or wounds. The accompanying characteristic skin lesions can take on a range of morphologies depending on the infection's stage; lesions that are crusted, papillomatous, maculopapular, targetoid, and nodular can occur before clearing

Necrotic skin lesions associated with disseminated candidiasis.

A 69-year-old woman with Felty's syndrome developed necrotic skin lesions associated with disseminated Candida tropicalis infection. These lesions differed from the previously described erythematous macronodules of disseminated candidiasis, although histologically there was a dermal infiltrate of yeast and pseudohyphae. Clinically, they resembled ecthyma gangrenosum associated with Pseudomonas septicemia. We believe candidiasis should be included in the differential diagnosis of large necrotic skin lesions in the compromised host.

A single black ulcer in a child with acute lymphocytic leukemia.

Ecthyma gangrenosum is an uncommon dermatological manifestation characterized by round, indurated ulcers with a central necrotic black eschar and surrounding erythema. This report describes the case of a 5-year-old girl, affected by acute lymphocytic leukemia, presenting with a black eschar on her right thigh. Such lesions should always be correctly identified to avoid potentially fatal bacteraemia. Furthermore, because of its similar clinical presentation, cutaneous anthrax must be ruled out.

[Huge gangrenous cellulitis. Clinical symptoms-differential diagnosis--therapy].

Extensive necrotizing cellulitis occurs in adults, when body resistance is reduced by serious general disorders. Differential diagnosis should exclude the following diseases: gangrenous erysipelas, necrotizing fasciitis, gangrenous ecthyma, pyoderma gangrenosum, purpura fulminans and Coumarin necrosis. In addition to antibiotic therapy, the removal of vast necrotic areas as well as plastic covering is necessary. A case of typical predisposing disorders, clinical symptoms and development of the disease is reported.

[Expanding papillomatous nodule on forearm with acute lymphangitis. Case of diagnosis].

Ecthyma contagiosum (orf) is a dermatosis commonly seen in those in contact with sheep. It is caused by Parapoxvirus ovis (orf virus), an oval epitheliotropic DNA parapox virus. The skin disease develops in stages starting as a macule or papule, becoming nodular, and then regressing. Diagnosis is based on history and histology, as well as identifying the virus through cell culture or specified polymerase chain reaction (PCR). The treatment of this self-limited disease is usually symptomatic.

Solitary Subungual Orf.

Orf-ecthyma contagiosum-is an endemic cutaneous disease caused by parapoxvirus that is transmitted via direct contact with contagious animals. The lesions are located frequently on the hands and fingers. Subungual presentation of orf is very rare. We report a case of solitary subungual orf. Suspicious subungual nodular lesions may be cases of orf, especially in endemic areas. Orf disease should be considered in the differential diagnosis of subungual nodular lesions to avoid unnecessary surgical interventions.

Septic shock in bacteraemia by Pseudomonas aeruginosa due to a minor skin injury in a lung transplant recipient.

A man in his 60s, under dual immunosuppression following bilateral lung transplantation (BiLuTx), developed bacteraemia and septic shock by Pseudomonas aeruginosa with ecthyma gangrenosum. Symptoms emerged acutely a few weeks after he had received high-dose intravenous steroid treatment for acute cellular graft rejection (ACR). The suspected route of entry of the pathogen was a minor accidental skin injury. Despite the severity of his condition, he successfully recovered after an intensive 26-day course of intravenous antibiotic therapy.

ATYPICAL PRESENTATION OF RECURRENT VARICELLA ZOSTER VIRUS INFECTION: A CASE REPORT AND REVIEW OF THE LITERATURE.

Recurrent varicella infection is rare but has been reported in immunocompromised patients. We present a patient with atypical recurrent varicella infection who had disseminated central crusting papular lesions without dermatomal distribution. Serology showed previous varicella zoster virus (VZV) infection and the lesions were positive for VZV DNA, consistent with recurrent VZV infection. Atypical recurrent varicella infection is probably an under-recognized condition. VZV infection should be considered in the differential diagnosis of ecthyma-like lesions in an immunocompromised host.

[Sepsis due to Pseudomonas aeruginosa in a previously healthy infant].

Pseudomonas aeruginosa, is an opportunistic organism widely distributed in both environmental and nosocomial settings. Invasive infections typically occur in immunocompromised patients, but this agent can also produce sepsis in the immunocompetent host. We report the case of an 8 months old infant, previous healthy, who presented septic shock by P. aeruginosa. Lesions of ecthyma gangrenosum in his limbs required surgical debridement. The patient presented transitory neutropenia and reduced C4 levels but subsequent study of immune defects was normal.
