Cochlear implantation with a dexamethasone-eluting electrode array: First-in-human safety and performance results.

Cochlear implantation is the standard of care for individuals with severe-to-profound sensorineural hearing loss. However, implantation itself can degrade residual hearing, for example due to insertional trauma and subsequent inflammatory processes. One potential method to mitigate this loss of residual hearing is through the local and sustained delivery of anti-inflammatory drugs released from the electrode array. To this end, a dexamethasone eluting electrode array (FLEX28 DEX) was developed by MED-EL. Here we present the results from a first-in-human feasibility study of the CIDEXEL system

Hearing preservation outcomes with motorized cochlear implant electrode insertion: matched-cohort observations.

BACKGROUND: Cochlear implants (CIs) are an established treatment for severe sensorineural hearing loss and are increasingly used in patients with substantial residual hearing. Preservation of residual hearing is associated with improved outcomes, including speech understanding in noise, natural sound perception, and spatial hearing. Manual electrode insertion, however, is limited by hand tremor and abrupt maneuvers, which can cause intracochlear trauma. Motorized insertion tools (MITs) have been developed to enable slow, continuous, and highly controlled electrode advancement. METHODS: We con

Acute human defibrillation performance of a subcutaneous implantable cardioverter-defibrillator with an additional coil electrode.

BACKGROUND: The subcutaneous implantable cardioverter-defibrillator (S-ICD) delivers 80 J shocks from an 8 cm left-parasternal coil to a 59 cm3 left lateral pulse generator (PG). A system that defibrillates with lower energy could significantly reduce PG size. Computer modeling and animal studies suggested that a second shock coil either parallel to the left-parasternal coil or transverse from the xiphoid to the PG pocket would significantly reduce the defibrillation threshold. OBJECTIVE: The purpose of this study was to acutely assess the defibrillation efficacy of parallel and transverse co

Inverted clip technique for cochlear implant electrode array fixation: how I do it.

BACKGROUND: Electrode migration has been identified as a complication primarily related to straight electrodes. To address this issue, the use of a fixation clip has been suggested as an alternative to stabilize the electrode lead and reduce the risk of migration. METHODS: A modified approach to the standard fixation clip procedure is introduced. Steps and technical considerations are presented in the video attached. CONCLUSIONS: This novel configuration offers enhanced fixation of the electrode array without requiring additional surgical time and without posing any additional risk to the fa

Robotics, automation, active electrode arrays, and new devices for cochlear implantation: A contemporary review.

In the last two decades, cochlear implant surgery has evolved into a minimally invasive, hearing preservation surgical technique. The devices used during surgery have benefited from technological advances that have allowed modification and possible improvement of the surgical technique. Robotics has recently gained popularity in otology as an effective tool to overcome the surgeon's limitations such as tremor, drift and accurate force control feedback in laboratory testing. Cochlear implantation benefits from robotic assistance in several steps during the surgical procedure: (i) during the app

Cochlear Implant Insertion Axis Into the Basal Turn: A Critical Factor in Electrode Array Translocation.

HYPOTHESIS: An inappropriate insertion axis leads to intracochlear trauma during cochlear implantation (CI). BACKGROUND: Few studies assessed the relationship between the insertion axis and the electrode scalar location. METHODS: Preimplantation cone-beam CT (CBCT) was performed on 12 human temporal bones. In five temporal bones, an optimal insertion axis was planned, due to the impossibility to attain the ST centerline from the posterior tympanotomy, because of facial canal position. In the seven other temporal bones, an inaccurate insertion axis was intentionally planned (optimal axis+15 d

Dexamethasone Reduces the Foreign Body Reaction to Intraneural Electrode Implants in the Peripheral Nerve of the Rat.

Intraneural electrodes must be in intimate contact with nerve fibers to have a proper function, but this interface is compromised due to the foreign body reaction (FBR). The FBR is characterized by a first inflammatory phase followed by a second anti-inflammatory and fibrotic phase, which results in the formation of a tissue capsule around the implant, causing physical separation between the active sites of the electrode and the nerve fibers. We have tested systemically several anti-inflammatory drugs such as dexamethasone (subcutaneous), ibuprofen and maraviroc (oral) to reduce macrophage act

[Changes in verbal and nonverbal memory associated with bilateral hippocampal electrode implantation for epilepsy surgery].

INTRODUCTION: An appropriate localization of ictal onset zone in refractory temporal lobe epilepsy favors an adequate outcome associated with surgical treatment. When video-electroencephalogram (video-EEG) and magnetic resonance imaging do not provide accurate data to locate ictal onset zone, the use of subdural or deep intracranial electrodes is indicated. Hippocampal electrode placement could generate functional changes in an unaffected hippocampus. AIM: To describe mnesic changes in patients admitted for epilepsy surgery, with previous bilateral hippocampal implantation using depth electro

[Implanting the conducting electrode in rat and investigating its effect on the rat's penicillin-induced seizure].

To investigate the effects of conducting electrodes implanted into the penicillin-induced epilepsy foci in rats on seizures, 30 male SD rats were randomly divided into experimental group (EG) and control group (CG). Afer penicillin(PNC) administration, metal electrode was implanted into the epilepsy focus in EG. Behavior evaluations were given after PNC injection in both groups and electroencephalogram recordings were taken respectively. Immunohistochemical methods were used to test the expressions of P38 and Calcium channel a 1A subunit (CACNA1A) at the serial time-point. We found that EG sho

[DDD-pacemaker implantation in a 7-week-old infant: epicardial electrodes in intrapleural position].

Permanent pacing for complete heart block in newborns and small infants is a challenge concerning the mismatch between body size and pacemaker dimensions. As the abdominal position of the large pacemaker carries a considerable risk of healing disturbances, the intrapleural position of single chamber pacemakers is accepted as well. We report about a 7 week old infant with congestive heart failure due to a complete atrioventricular septal defect. The immature valve tissue led to recurrent, severe mitral valve incompetence and necessitated several valvuloplasty procedures and finally the implanta

Cardiovascular alterations induced by chronic transvenous implantation of an automatic defibrillator electrode catheter in dogs.

Six dogs (three large, three small) had defibrillator electrode catheters implanted transvenously via the right jugular vein for 17 weeks. The dogs remained healthy throughout the study. At necropsy, the lead-induced cardiovascular alterations were characterized by (1) formation of a thin white fibrous sheath over portions of the lead along its course in the veins, right atrium and right ventricle; (2) adhesion of parts of the sheathed segments of the lead to the adjacent venous intima or endocardium of the right atrium, tricuspid valve and right ventricle; (3) endocardial fibrosis with foci o

Reaction times of cochlear implant users to a change in electrode or modulation frequency in the presence of a masker.

Spatial and temporal selectivity vary substantially between cochlear implant users, resulting in large differences in speech perception. Direct electrode stimulation offers precise control to capture this variability. However, most studies using direct stimulation have not incorporated interfering stimuli, which may not reflect the complex nature of speech and ambient noise. To address this gap, we introduce the Electrode Interaction Reaction Time (ELI-RT) paradigm, which uses amplitude-modulated, interleaved pulses on two closely spaced electrodes at clinical pulse rates. ELI-RT includes two

Quantitative Investigation of Implant Effect of Intracranial EEG Electrodes on Cortico-Cortical Evoked Potentials.

Intracranial EEG (iEEG) electrodes allow for direct sampling of neural signals from cortical substructures that provide valuable insight for the treatment of medically-refractory epilepsy and other neurological conditions. Changes in neural behavior (i.e., seizure frequency or motor function) have been documented as the result of the implantation of the intracranial electrodes, termed the implant effect, but the mechanism responsible for these changes is not well understood. Implant revision (i.e., the implantation of new iEEG electrodes to sample additional brain regions) during pre-surgical

Diving technique of cochlear implant electrode insertion for hearing preservation: how I do it.

BACKGROUND: Preservation of intracochlear structures and residual hearing has become a major concern in modern cochlear implant. Consequently, many efforts have been made to minimize intraoperative trauma, especially while cochlear fenestration and electrode insertion. METHODS: Building on the core concept of "soft surgery", a modified approach, described as diving technique for cochlear implant electrode array insertion is proposed. Steps and technical points are presented with figures, video and review of relevant anatomy. CONCLUSIONS: This novel diving technique is operationally feasible

Automatic localization of cochlear implant electrodes using cone beam computed tomography images.

BACKGROUND: Cochlear implants (CI) are implantable medical devices that enable the perception of sounds and the understanding of speech by electrically stimulating the auditory nerve in case of inner ear damage. The stimulation takes place via an array of electrodes surgically inserted in the cochlea. After CI implantation, cone beam computed tomography (CBCT) is used to evaluate the position of the electrodes. Moreover, CBCT is used in research studies to investigate the relationship between the position of the electrodes and the hearing outcome of CI user. In clinical routine, the estimation

Robotic Milling of Electrode Lead Channels During Cochlear Implantation in an ex-vivo Model.

Objective: Robotic cochlear implantation is an emerging surgical technique for patients with sensorineural hearing loss. Access to the middle and inner ear is provided through a small-diameter hole created by a robotic drilling process without a mastoidectomy. Using the same image-guided robotic system, we propose an electrode lead management technique using robotic milling that replaces the standard process of stowing excess electrode lead in the mastoidectomy cavity. Before accessing the middle ear, an electrode channel is milled robotically based on intraoperative planning. The goal is to f
