Telephone CPR Instructions in Emergency Dispatch Systems: Qualitative Survey of 911 Call Centers.

INTRODUCTION: Out-of-hospital cardiac arrest (OHCA) is a leading cause of death. The 2010 American Heart Association Emergency Cardiovascular Care (ECC) Guidelines recognize emergency dispatch as an integral component of emergency medical service response to OHCA and call for all dispatchers to be trained to provide telephone cardiopulmonary resuscitation (T-CPR) pre-arrival instructions. To begin to measure and improve this critical intervention, this study describes a nationwide survey of public safety answering points (PSAPs) focusing on the current practices and resources available to prov

Path Optimization of Medical Waste Transport Routes in the Emergent Public Health Event of COVID-19: A Hybrid Optimization Algorithm Based on the Immune-Ant Colony Algorithm.

In response to the emergent public health event of COVID-19, the efficiency of transport of medical waste from hospitals to disposal stations is a worthwhile issue to study. In this paper, based on the actual situation of COVID-19 and environmental impact assessment guidelines, an immune algorithm is used to establish a location model of urban medical waste storage sites. In view of the selection of temporary storage stations and realistic transportation demand, an efficiency-of-transport model of medical waste between hospitals and temporary storage stations is established by using an ant col

Incidence and factors associated with bystander cardiopulmonary resuscitation in out-of-hospital cardiac arrest in an Italian region.

INTRODUCTION: Out-of-hospital cardiac arrest (OHCA) is a major cause of mortality, and early bystander cardiopulmonary resuscitation (CPR) is critical for survival. This study assessed OHCA incidence and factors associated with bystander-initiated CPR in the Piedmont region of Northern Italy. METHODS: A retrospective cross-sectional analysis was conducted using 2022 administrative data from an Emergency Medical Dispatch Centre serving 851,538 residents. All confirmed OHCA cases were included. Demographic, clinical, and contextual variables were extracted. Descriptive statistics summarized cas

A retrospective review of advanced life support interfacility transfers of the public sector emergency medical service in the Western Cape Province, South Africa.

BACKGROUND: The need for critical care transfers (CCTs) has increased in recent years owing to the growing prevalence of high-acuity patients who require access to specialised care and/or resources that are not readily available at the facility where they find themselves. During their transfer from one facility to another, critically ill and injured patients commonly require ongoing care, monitoring and interventions that can only be provided by transfer teams with advanced training and appropriate equipment. In South Africa (SA), these transfers are undertaken mainly by advanced life support

Concrete silo collapse: emergency medical services response to a mass casualty incident.

BACKGROUND: Major incidents evolving from occupational accidents are very infrequent in Scandinavia and therefore, case reports are called for. On 26 November, 2024, a fatal occupational accident took place during the construction of a concrete silo in the small rural town of Flemløse (population 574), Denmark. Three people died and six were injured as the result of a collapsing concrete roof during construction. We aim to describe the incident response by the emergency medical services (EMS), to identify areas of improvement, and to evaluate the adherence to current national major incident gu

Prehospital identification of acute ischaemic stroke with large vessel occlusion: a retrospective study from western Norway.

BACKGROUND: In 2019, the emergency medical services (EMS) covering the western Norway Regional Health Authority area implemented its version of the prehospital clinical criteria G-FAST (Gaze deviation, Facial palsy, Arm weakness, Visual loss, Speech disturbance) to detect acute ischaemic stroke (AIS) with large vessel occlusion (LVO). For patients with gaze deviation and at least one other G-FAST symptom, a primary stroke centre (PSC) may be bypassed and the patient taken directly to a comprehensive stroke centre (CSC) for rapid endovascular treatment (EVT) evaluation. The study aim was to inv

Racial and ethnic differences in bystander cardiopulmonary resuscitation for opioid-associated out-of-hospital cardiac arrests.

BACKGROUND: A number of research studies expose race and ethnic disparities in bystander cardiopulmonary resuscitation (bCPR) among people experiencing out-of-hospital cardiac arrest (OHCA). There is limited research on the degree of racial and ethnic bCPR disparities among people experiencing opioid-associated OHCA (OA-OHCA). The purpose of this study is to evaluate these differences. METHODS: This retrospective cohort study investigated bCPR patterns among adult OA-OHCA patients using U.S. National Emergency Medical Services Information System (NEMSIS) dataset years 2017 through 2025. General and Specific Stress Factors as Potential Predictors of Work Ability Among Pre-Hospital Emergency Medical Personnel.

Background/Objectives: Retention of healthcare professionals in the workforce, their employment, and the improvement of working conditions largely depend on identifying the factors that influence their departure and their health. The study was conducted during the period from January to June 2021. This study aimed to examine the association between specific work-related stressors and work ability. The initial hypothesis was that general and specific occupational stressors negatively associate with work ability among healthcare professionals in emergency medical intervention teams. Methods: The

Adult and Pediatric Emergency Medical Services Ambulance Response Times.

BACKGROUND: The routine use of lights and sirens (L&S) by emergency medical services (EMS) may be associated with increased risk of ambulance crashes. Recently, the National Association of EMS Physicians and American College of Emergency Physicians, among others, released a joint statement recommending decreased use of L&S when appropriate. OBJECTIVES: Given the recent shifting culture in recommended L&S use, our objective was to describe current EMS response times across the U.S., stratified by urbanicity and patient age. METHODS: This was a cross-sectional evaluation of the 2023 National E

Prevalence of insomnia, fatigue and symptoms of mental health problems among emergency medical service nurses: a cross-sectional study.

BACKGROUND: Emergency medical service nurses worldwide face continuous high-stress situations caused by critical incidents that can overwhelm them emotionally and affect their daily functioning and sustainable employability. Repeated exposure to these incidents negatively impacts their mental health. The COVID-19 pandemic has further exacerbated these issues, with high prevalence rates of insomnia and fatigue among emergency medical service nurses serving as key predictors of mental health problems. Until now little is known about the mental consequences of the COVID-19 pandemic on EMS nurses.

Characteristics of Prehospital Electrocardiogram Use in North Carolina Using a Novel Linkage of Emergency Medical Services and Emergency Department Data.

Objective: Prehospital electrocardiography (ECG) is recommended for patients with suspected acute coronary syndrome (ACS), yet only 20-80% of chest pain patients receive a prehospital ECG. Less is known about prehospital ECG use in patients with less common complaints (e.g., fatigue) suspicious for ACS who are transported by emergency medical services (EMS). The aims of this study were to determine: (1) the proportion of patients with chest pain and less typical complaints, and (2) patient characteristics associated with prehospital ECG use in patients transported by EMS to emergency departmen

Paramedics' Perspectives on the Hospital Transfers of Nursing Home Residents-A Qualitative Focus Group Study.

Emergency department (ED) visits and hospital admissions are common among nursing home residents (NHRs). Little is known about the perspectives of emergency medical services (EMS) which are responsible for hospital transports. The aim of this study was to explore paramedics' experiences with transfers from nursing homes (NHs) and their ideas for possible interventions that can reduce transfers. We conducted three focus groups following a semi-structured question guide. The data were analyzed by content analysis using the software MAXQDA. In total, 18 paramedics (mean age: 33 years, male n = 14

When Bystanders Skip CPR: Insights from a Retrospective Study.

Background and Objectives: Out-of-hospital cardiac arrest (OHCA) is one of the most clinically significant conditions and can lead to rapid death if cardiopulmonary resuscitation (CPR) or defibrillator use is not performed. Despite this, in approximately half of cases of witnessed OHCA, bystanders do not initiate resuscitation maneuvers. The factors underlying this phenomenon are varied but not yet fully understood. The aim of our analysis is to identify predictors that may indicate a higher likelihood of not performing resuscitation maneuvers. Materials and Methods: Data from cardiac arrests

Effect of time of day on prehospital care and outcomes after out-of-hospital cardiac arrest.

BACKGROUND: More than 300 000 out-of-hospital cardiac arrests (OHCA) occur each year in the United States. The relationship between time of day and OHCA outcomes in the prehospital setting is unknown. Any such association may have important implications for emergency medical services resource allocation. METHODS AND RESULTS: We performed a retrospective review of cardiac arrest data from a large, urban emergency medical services system. Included were OHCA occurring in adults from January 2008 to February 2012. Excluded were traumatic arrests and cases in which resuscitation measures were not
