[Diagnosis and treatment of seven primary mediastinal yolk sac tumors].

OBJECTIVE: Primary mediastinal yolk sac tumor, which is also known as endodermal sinus tumor, is a rare but lethal neoplasm and its prognosis is very dismal. The current treatment for this tumor is controversial, and chemotherapy combined with resection of residual lesions is adopted sometimes. We summarized the experience of seven primary mediastinal yolk sac tumors treated with platinum-based chemotherapy and extended resection in Peking University First Hospital. METHODS: Clinicopathological data of the patients with primary mediastinal yolk sac tumor who received operation in Peking Unive

Primary yolk sac tumor originating from the endometrium: A case report and literature review.

RATIONALE: Yolk sac tumors (YSTs) are malignant germ cell tumors that secrete alpha-fetoprotein (AFP). These tumors commonly develop in infants, young children, and young women and often originate in the gonads. Primary endometrial YST is a very rare malignancy, and a primary endometrial YST in the absence of abnormal AFP levels is even rarer. PATIENT CONCERNS: A 38-year-old woman presented with the chief complaint of prolonged menstruation and increased menstrual bleeding with a duration of more than 2 months. DIAGNOSES: Postoperative pathology confirmed a diagnosis of endometrial YST with

Intracardiac yolk sac tumor and dysrhythmia as an etiology of pediatric syncope.

This report describes a new etiology of pediatric syncope. Epilepsy, brain anomalies, infection, electrolyte abnormalities, and trauma are commonly identified etiologies of seizures in the pediatric population. We report here a child with third-degree heart block and right ventricular outflow tract obstruction related to an intracardiac tumor presenting with syncope and seizure-like activity. Echocardiography revealed a large (3 x 8-cm) intracardiac mass filling the right atrium, extending across the tricuspid valve into the right ventricle and crossing the atrial septum into the left atrium,

Primary yolk sac tumors of the mesentery. A report of two cases.

Two yolk sac tumors that arose in the mesentery of the jejunum and the mesentery of the transverse colon of two male patients, aged 2 and 17 years, are reported. Both patients had abdominal masses. The tumors measured 9 and 11 cm in maximum dimension. One of them grew into the bowel lumen. Microscopically, both neoplasms exhibited several of the typical patterns of yolk sac tumor and stained immunohistochemically for alpha-fetoprotein. Both patients received chemotherapy postoperatively and are alive, but follow-up is short. The subject of extragonadal yolk sac tumors is reviewed, and histogen

Ovarian Sertoli-Leydig cell tumor, endometrioid-like yolk sac tumor, and Y-chromosomal material.

Sertoli-Leydig cell tumors (SLCTs) are rare neoplasms, accounting for less than 0.2% of ovarian tumors. The endometrioid-like variant of yolk sac tumor (YST) is very rare, and the most extensive series reported only 8 cases. We present a case of ovarian SLCT with endometrioid-like YST in a patient with a 46,XX karyotype with Y-chromosomal material. A 26-year-old woman had undergone a right salpingo-oophorectomy for SLCT with endometrioid-like YST. Chromosomal analysis revealed a 46,XX karyotype with Y-chromosomal material insertion into chromosome 1. The patient's father and sister, and 7 othe

Postchemotherapy phenotypes of GCNIS-derived testicular germ cell tumors, focusing on yolk sac tumor variants, trophoblastic tumors, and somatic-type malignancies.

Postchemotherapy retroperitoneal lymph node dissection (PC-RPLND) specimens from patients with testicular germ cell tumors (TGCTs) often reveal a complex mix of residual tumor components and treatment-related changes. This review focuses on the morphologic spectrum of yolk sac tumor (YST) variants, trophoblastic tumors, and somatic-type malignancies in PC-RPLNDs, which continue to pose significant diagnostic and clinical challenges. Accurate identification of these entities is critical, as they carry distinct prognostic implications and influence management decisions. A multidisciplinary appro

Endometrial, Ovarian, and Peritoneal Involvement by Endometrioid Carcinoma, Yolk Sac Tumor, and Endometriosis: Molecular Evidence for a Shared Precursor.

Recent studies have provided molecular confirmation that a subset of yolk sac tumors is somatically derived. Somatically derived yolk sac tumors are typically diagnosed in older women and are often seen adjacent to epithelial proliferations (such as endometriosis or endometrioid carcinoma) with which they share mutations. Here, we present a case of a postmenopausal woman with a yolk sac tumor and endometriosis in the right ovary, endometriosis with glandular crowding and reactive changes in the left ovary, endometrial endometrioid carcinoma, and yolk sac tumor involving the serosa of the colon

Yolk Sac Tumor in an Infant with Androgen Insensitivity Syndrome: A Case Report and Review of the Literature.

Background: Androgen insensitivity syndrome (AIS) is a disorder of sexual differentiation caused by complete or partial resistance to the biological action of androgens. The common malignant tumors associated with this syndrome are seminomas. However, the risk of malignancy in childhood remains low. Case Report: A 8-month-old child with a female phenotype and a 46, XY karyotype, presented with bilateral inguinal hernia. The patient underwent right radical inguinal orchiectomy with high ligation of the spermatic cord and laparoscopic percutaneous extra-peritoneal herniorrhaphy. Final pathology

Pure yolk sac tumour, post-pubertal type, arising from cryptorchid testes.

Yolk sac tumour (YST), the most common germ cell tumour (GCT) in infants and children, accounts for 80% of GCTs in this age group. 1 It is observed in only 2.4% of adult patients in their pure form since up to 42% of mixed GCTs have some component of the yolk sac. 2 We present a 46-year-old cryptorchid man who presented with complaints of abdominal lump, pain and altered bowel habits since 1 month. CT scan of the abdomen revealed a mass arising from the pelvic cavity with feeders predominantly from the left gonadal vessel. Mass was excised via exploratory laparotomy and sent for histological e

Yolk sac tumour involving floor of mouth: case report.

Yolk sac tumour is a rare malignant tumour of germ cell origin that usually arises from the gonads. It is extremely rare in the head and neck. We present a case of a yolk sac tumour that involved the floor of the mouth in a 14-month-old Chinese girl. The diagnosis was confirmed by histological examination that showed characteristic Schiller-Duval bodies, and immunohistochemical examination that showed α-fetoprotein being expressed within the cytoplasm of the neoplastic cells. The patient died 4 months postoperatively. To our knowledge, this is the first case of a yolk sac tumour of the floor o

Pineal yolk sac tumor in a 65-year-old man.

A 65-year-old man presented with a case of pure yolk sac tumor in the pineal region. The patient was admitted with a gait disturbance and elevated alpha-fetoprotein levels in serum and cerebrospinal fluid. He underwent total resection of the tumor. Cisplatin-vinblastine-bleomycin therapy was interrupted because of severe syndrome of inappropriate secretion of antidiuretic hormone, and there was recurrence of the tumor. Five months after the operation, he died in spite of carboplatin-etoposide therapy. A review of the literature revealed no previous report of pineal yolk sac tumor in an elderly

[Diagnostic problems in testicular tumours in consultant pathology : A practical guide].

The great variety of pathological patterns in germ cell tumours, especially in yolk sac tumours but also the possibility of somatic-type malignancies, can complicate daily diagnosis. For the correct diagnosis, knowledge of morphological aspects and additional immunohistochemical staining can be helpful. Also, rare entities like sex cord stromal tumours, tumours of testicular adnexa or mesenchymal tumours of the spermatic cord can be diagnostically challenging.

Germ Cell Tumors of the Female Genital Tract.

Ovarian germ cell tumors are a histologically diverse group of neoplasms with a common origin in the primitive germ cell. The vast majority are represented by mature cystic teratoma. In the minority are malignant germ cell tumors including immature teratoma, dysgerminoma, yolk sac tumor, embryonal cell carcinoma, and choriocarcinoma. This article reviews the histologic and immunohistochemical features of the most common ovarian germ cell tumors. The differential diagnoses for each are discussed.

Extragonadal Pelvic Yolk Sac Tumor in a Postpubertal Patient: Case Report With Radiologic Correlation and Review of the Literature.

Yolk sac tumor, also referred to as endodermal sinus tumor, is an aggressive malignant germ cell neoplasm that most commonly originates in the gonad. Its occurrence in extra-gonadal sites is infrequent and is highly malignant, with only a handful of cases reported in the literature. With the help of this article, we want to make readers aware of the rare site of yolk sac tumor, its clinical presentation, and its radiological findings. As clinicians, we should be vigilant to consider differential diagnoses for an underlying etiology, like yolk sac tumor, for a patient presenting with a subcutan

Recurrence of Primary Mediastinal Yolk Sac Tumor with Brain Metastases: A Case Report and Literature Review.

Primary mediastinal yolk sac tumor (PMYST) is a rare and highly malignant germ cell tumor. We present a 24-year-old male admitted in 2018 with PMYST, superior vena cava syndrome (SVCS), and pulmonary and lymph node metastases. Following chemotherapy and partial resection, brain metastasis occurred; complete remission (CR) was achieved with chemotherapy plus cranial radiotherapy. Multiple bone metastases developed after 6 years. Re-resection revealed adenocarcinoma, consistent with chemotherapy-related malignant transformation. Despite multiple lines of chemotherapy and radiotherapy, the diseas

Extensive surgical resections for rare pleural neoplasms: a single-center experience with a yolk sac tumor and synovial sarcoma.

BACKGROUND: Pleural neoplasms are rare and can be subdivided into pleural metastasis and primary pleural neoplasms. Non-mesothelioma primary pleural neoplasms are a diverse group of extremely rare pathologies. CASE PRESENTATION: In this case series, we describe the presentation and management of two rare primary pleural neoplasms. A first case describes a primary pleural yolk sac tumor treated with neoadjuvant chemotherapy, extended pleurectomy decortication, and hyperthermic intrathoracic chemotherapy. In a second case we describe the management of a primary pleural synovial sarcoma by neoad

Clinical characteristics and prognostic models of gonadal and extra-gonadal yolk sac tumors: a population-based analysis in children and adolescents.

PURPOSE: Yolk sac tumors (YST) are a rare and aggressive germ cell tumor. We aimed to conduct a population-based cohort study and develop a nomogram to predict overall survival (OS) in pediatric patients with YST. METHODS: The Surveillance, Epidemiology, and End Results (SEER) database was used to identify all pediatric patients with YST diagnosed between 2000 and 2018. The log-rank test was used to compare survival curves. To examine the impact of each factor on overall survival, a multivariate Cox proportional hazards model was created. Based on the results of the Cox regression model, a no
