Active sac management for prevention of type II endoleaks after endovascular aneurysm repair.

Background: Type II endoleaks (T2EL) remain the most common complication after endovascular aneurysm repair (EVAR). Aneurysm sac regression is a predictor for better treatment outcomes compared to sac stability and growth. T2EL are associated with aneurysm sac regression and prophylactic embolization of the sac or side branches may result in lower T2EL incidence. This review aims to assess the current evidence on whether prophylactic treatment strategies provide improved clinical outcomes after EVAR. Materials and methods: A systematic search was performed of the Scopus, PubMed and Web of Scie

Exploring Type IIIb Endoleaks: A Literature Review to Identify Possible Physical Mechanisms and Implications.

Endoleaks are common complications following endovascular aneurysm repair (EVAR). They can be classified into low-pressure and high-pressure endoleaks. High-pressure endoleaks, which include Type I and Type III endoleaks, pose a significant risk of rupture and require urgent treatment. The aim of our study is to review published case reports and case series to assess the impact of Type IIIb endoleaks in EVAR and to identify possible mechanisms contributing to these endoleaks. This review targeted case reports and case series published between January 1998 and December 2022. A total of 62 case

Late type III endoleaks are common in early generation Endologix AFX stent grafts.

OBJECTIVE: Early generation Endologix AFX endovascular abdominal aortic aneurysm (AAA) stent graft devices are at risk of developing type IIIa (intercomponent/overlap-related) as well as type IIIb (fabric tear) endoleaks over time. Current follow-up studies are limited to short (<24 months) and midterm (24-48 months) reports. The purpose of this study was to identify the incidence of type III endoleaks associated with these endografts over a long-term follow-up (>4 years) period and discuss current management strategies. METHODS: A retrospective, single-institution cohort analysis of all Endo

Translumbar Embolization of Type II Endoleaks: 12 Years of Experience at a Regional Vascular Centre.

BACKGROUND AND AIMS: The management of persistent type II endoleaks (T2E) is often problematic for the endovascular specialist, with a lack of clear evidence for the best approach for embolization. The aim of this study was to evaluate the safety and efficacy of translumbar embolization (TLE) for T2E following endovascular aneurysm repair (EVAR). METHODS: This retrospective review included 27 embolizations performed on 23 patients with a median age of 78 (range 67-94 years; male: female 15:9), during the period September 2006 to July 2018. Primary outcome was freedom from aneurysm sac growth

Treatment of Type II Endoleaks with a Novel Agent: Precipitating Hydrophobic Injectable Liquid (PHIL).

OBJECTIVE: Endovascular aortic aneurysm repair has become the principle technique used to treat abdominal aortic aneurysms. However, its long-term success is limited by complications, including type II endoleaks from lumbar arteries and the inferior mesenteric artery. Treatment of these endoleaks can be difficult. The most commonly performed procedure is embolisation via mesenteric or iliolumbar vessels. Embolic agents currently used include coils, glue and Onyx, all of which have their limitations. Our aim was to assess a novel embolic agent precipitating hydrophobic injectable liquid (PHIL)

Atypical Endoleak Associated With Inadvertent Cannulation of the Ipsilateral Gate in an Ovation Trimodular Stent-Graft.

We report on a 54-year-old man who presented with an atypical, proximal, intraoperative endoleak after endovascular aortic repair with an Ovation endograft for a 65-mm-diameter abdominal aortic aneurysm. The endografting was complicated by inadvertent bilateral iliac limb insertion into the right gate without cannulation of the left gate. The endoleak was treated by brachial approach: Through the open left gate, the outflow inferior mesenteric artery was coil embolized and the inflow left gate was closed with an Amplatzer plug. Follow-up computed tomography over 3 years showed absence of any e

Endoleak as a complication of endoluminal grafting of abdominal aortic aneurysms: classification, incidence, diagnosis, and management.

The inability to obtain or maintain a secure seal between a vessel wall and a transluminally implanted intra-aneurysmal graft is a complication unique to the evolving technique of endovascular aneurysm exclusion. Because the term "leak" has long been associated with aneurysm rupture, the term "endoleak" is proposed as a more definitive description of this phenomenon. Embracing both persistent blood flow into the aneurysmal sac from within or around the graft (graft related) and from patent collateral arteries (nongraft related), endoleak can be classified as primary or secondary depending on t

Automated Morphological Phenotyping Identifies Anatomic Patterns Associated with Early Type I Endoleak after Endovascular Aortic Repair.

OBJECTIVE: Early type I endoleak (T1EL) following endovascular aortic repair (EVAR) remains a clinically important complication. Risk stratification is traditionally based on binary morphological thresholds, but their ability to predict early failure is limited. This study aimed to use fully automated volume segmentation (FAVS) to identify anatomic phenotypes associated with early T1EL. METHODS: This was a multicentre, retrospective cohort study. Pre-operative computed tomography angiography of patients undergoing EVAR was analysed using FAVS, with matched National Vascular Registry clinical

Post-TEVAR type IA endoleak caused hemoptysis in a case of mid-thoracic aortic saccular aneurysm.

INTRODUCTION AND IMPORTANCE: Aortobronchial fistula (ABF) is a rare complication of descending thoracic aortic pseudoaneurysm and massive hemoptysis. Computed tomography angiography (CTA) is the best modality for diagnosis. Urgent treatment is required for this condition. CASE PRESENTATION: A 62-year-old man with a history of smoking and 2 years of intermittent hemoptysis was diagnosed with a descending thoracic aortic pseudoaneurysm with a fistula to the bronchi. Despite the initial thoracic endovascular aortic repair (TEVAR), hemoptysis persisted, and clinical evaluations showed a proximal

Right thoracotomy approach for the management of uncommon type II endoleak after thoracic endovascular aortic repair.

An 80-year-old male with a ruptured pseudoaneurysm after aortic dissection repair underwent thoracic endovascular aneurysm repair. The aneurysm expanded from 43 mm to 89 mm over 4 years because of type II endoleak, with computed tomography angiography revealing a patent right intercostobronchial trunk and left bronchial artery on the lesser curvature of the aortic arch. These arteries were successfully dissected via right thoracotomy. During the 2-year postoperative period, the aneurysm remained stable at 80 mm. Open surgical management of the culprit vessel may be effective for type II endole

Numerical study on the biomechanics mechanism of Type II endoleak after EVAR for Abdominal Aortic Aneurysm.

Type II endoleak, a common complication following Endovascular Aneurysm Repair (EVAR), remains a leading cause of re-intervention, yet its biomechanical mechanisms are not fully understood. This study employed an idealized model of the aneurysmal sac, Inferior Mesenteric Artery (IMA), and Lumbar Arteries (LAs) to explore endoleak mechanisms using Computational Fluid Dynamics (CFD) and Fluid-structure Interaction (FSI) simulations. The results demonstrated that pressure differences between the sac and branch arteries drive Type II endoleak, with the IMA often serving as the primary inlet. Under

Early and Mid-Term Outcomes of Transcaval Embolization for Type 2 Endoleak after Endovascular Aortic Repair.

Background: Among the endovascular approaches for the management of endoleak type 2 (EL 2), transcaval embolization (TCE) has shown encouraging outcomes. However, the literature is still limited. This study aimed to present the early and mid-term outcomes of TCE for EL 2 after endovascular aortic repair. Methods: A retrospective, single-center analysis of consecutive patients managed with TCE for EL 2 after standard or complex endovascular aortic repair, from August 2015 to March 2024, was conducted. The indication for TCE was the presence of an EL 2 related to ≥5 mm sac increase, compared to

Midterm Outcomes of Endoleak Type 2 Embolization after Endovascular Aortic Aneurysm Repair Using a Neurointerventional Approach.

BACKGROUND: Type 2 endoleaks (T2ELs) have been considered a benign condition and intervention is recommended when they are associated to sac expansion. The aim of this study was to report on T2EL embolization midterm outcomes, using neurointerventional material. METHODS: A single-center retrospective analysis of consecutive patients treated with transarterial embolization, using neurointerventional material, for T2EL after standard endovascular aortic aneurysm repair (EVAR) between January 01, 2017 and July 30, 2022, was undertaken. Primary outcome was technical success and secondary outcome

Impact of the Lumbar Arteries on Aneurysm Diameter and Type 2 Endoleak after Endovascular Aneurysm Repair.

BACKGROUND: Prophylactic embolization of the inferior mesenteric artery (IMA) during endovascular aneurysm repair (EVAR) is recommended to prevent type 2 endoleak (T2EL). However, the impact of patent lumbar arteries (LAs) on T2ELs and aneurysm diameter has not been elucidated. METHODS: Fifty-seven consecutive patients who underwent EVAR at our institution between January 2013 and September 2022 and whose IMA had been occluded preoperatively or newly occluded postoperatively were included in the study. Predictive factors for aneurysm sac enlargement, sac shrinkage, and T2EL were investigated.

Utilization of Intravascular Ultrasound and Aptus Heli-Fx EndoAnchors for a Complex Type Ia Endoleak Repair.

Type Ia endoleaks are a complication of endovascular aneurysm repair that require intervention due to the increased risk of aneurysmal rupture. Many techniques have been studied in order to minimize the occurrence of these complications, as well as to aid in their repair. We introduce the utilization of intravascular ultrasound in conjunction with the endovascular EndoAnchor systems for the repair of a complex type Ia endoleak in an infrarenal aortic stent graft. This technique allowed us to have real-time feedback of EndoAnchor placement and aortic wall to stent graft apposition with a reduct

Type II endoleak and aortic aneurysm sac shrinkage after preemptive embolization of aneurysm sac side branches.

OBJECTIVE: Type II endoleak (T2EL) is the most common endoleak after endovascular aneurysm repair (EVAR). Its optimal management has been controversially discussed. Thus, preliminary selective embolization of aneurysm sac side branches (ASSBs) has been adopted to prevent T2EL. Our goal was to determine the rate of T2EL and the diameter decrease of abdominal aortic aneurysms (AAAs) after EVAR performed after preemptive embolization of ASSBs. METHODS: From September 2014 to September 2019, 139 patients with AAAs underwent percutaneous ASSB embolization before EVAR. Follow-up imaging studies wer
