Meniere's disease.

Meniere's disease (MD) is a disorder of the inner ear that causes vertigo attacks, fluctuating hearing loss, tinnitus and aural fullness. The aetiology of MD is multifactorial. A characteristic sign of MD is endolymphatic hydrops (EH), a disorder in which excessive endolymph accumulates in the inner ear and causes damage to the ganglion cells. In most patients, the clinical symptoms of MD present after considerable accumulation of endolymph has occurred. However, some patients develop symptoms in the early stages of EH. The reason for the variability in the symptomatology is unknown and the re

Tricellulin deficiency affects tight junction architecture and cochlear hair cells.

The two compositionally distinct extracellular cochlear fluids, endolymph and perilymph, are separated by tight junctions that outline the scala media and reticular lamina. Mutations in TRIC (also known as MARVELD2), which encodes a tricellular tight junction protein known as tricellulin, lead to nonsyndromic hearing loss (DFNB49). We generated a knockin mouse that carries a mutation orthologous to the TRIC coding mutation linked to DFNB49 hearing loss in humans. Tricellulin was absent from the tricellular junctions in the inner ear epithelia of the mutant animals, which developed rapidly prog

Hemorrhage in the endolymphatic sac: a cause of hearing fluctuation in enlarged vestibular aqueduct.

OBJECTIVE: Most of the patients with enlarged vestibular aqueduct (EVA) experience sudden hearing deterioration, but the exact mechanism is unclear. We analyzed magnetic resonance (MR) images and the cellular components of endolymph obtained from the endolymphatic sac in patients with EVA, in order to demonstrate the cause of sudden hearing loss. METHODS: A total of 25 patients (50 ears) with EVA, who had severe to profound hearing loss, were included in this retrospective clinical study. MR examinations were performed by a 3.0-T MR system using an 8-channel sensitivity-encoding head coil. We

SLC4A11 prevents osmotic imbalance leading to corneal endothelial dystrophy, deafness, and polyuria.

Maintenance of ion concentration gradients is essential for the function of many organs, including the kidney, the cornea, and the inner ear. Ion concentrations and fluid content in the cornea are regulated by endothelial cells that separate the collagenous avascular corneal stroma from the anterior eye chamber. Failure to maintain correct ion concentrations leads to swelling and destruction of the cornea. In the inner ear, the stria vascularis is responsible for generating proper ion concentrations in the endolymph, which is essential for hearing. Mutations of SLC4A11 in humans lead to syndro

Blockage of longitudinal flow in endolymphatic hydrops.

The external shunt operation on the endolymphatic sac is based on the concept that it will drain excess endolymph and thus arrest the progression of endolymphatic hydrops. We performed histological studies on 46 temporal bones from 13 cases of bilateral and 20 cases of unilateral Meniere's disease to evaluate the status of the pathways of longitudinal flow of endolymph to the sac. The endolymphatic ducts were blocked in 8 specimens (17%), the endolymphatic sinuses in 9 (19.5%), the utricular ducts in 12 (26%), the saccular ducts in 7 (15%), and the ductus reuniens in 27 (59%). These blockages

Purinergic signaling in the inner ear.

Epithelial cells of the inner ear coordinate their ion transport activity through a number of mechanisms. One important mechanism is the autocrine and paracrine signaling among neighboring cells in the ear via nucleotides, such as adenosine, ATP and UTP. This review summarizes observations on the release, detection and degradation of nucleotides by epithelial cells of the inner ear. Purinergic signaling is thought to be important for endolymph ion homeostasis and for protection from acoustic over-stimulation.

Endolymphatic hydrops impairs inner ear uptake and distribution of intratympanically injected gadolinium mixed with dexamethasone in patients with Meniere's disease.

PURPOSE: To investigate how endolymphatic hydrops (EH) affects the entry of intratympanic gadolinium in the inner ear of patients with Meniere's disease (MD). MATERIALS AND METHODS: This study included 166 MD patients. MRI was performed 24 h after intratympanic injection of dexamethasone and 20-fold diluted Gd-DTPA. The region of interest with the strongest enhancement in the cochlea and vestibule was selected to calculate the signal intensity ratio (SIR), representing transport speed. The enhancement area in the vestibular perilymph, indicating the distribution volume of Gd-DTPA, was measure

The Auditory Nerve Overlapped Waveform (ANOW) Detects Small Endolymphatic Manipulations That May Go Undetected by Conventional Measurements.

Electrocochleography (ECochG) has been used to assess Ménière's disease, a pathology associated with endolymphatic hydrops and low-frequency sensorineural hearing loss. However, the current ECochG techniques are limited for use at high-frequencies only (≥1 kHz) and cannot be used to assess and understand the low-frequency sensorineural hearing loss in ears with Ménière's disease. In the current study, we use a relatively new ECochG technique to make measurements that originate from afferent auditory nerve fibers in the apical half of the cochlear spiral to assess effects of endolymphatic hydro

Communication pathways to and from the inner ear and their contributions to drug delivery.

The environment of the inner ear is highly regulated in a manner that some solutes are permitted to enter while others are excluded or transported out. Drug therapies targeting the sensory and supporting cells of the auditory and vestibular systems require the agent to gain entry to the fluid spaces of the inner ear, perilymph or endolymph, which surround the sensory organs. Access to the inner ear fluids from the vasculature is limited by the blood-labyrinth barriers, which include the blood-perilymph and blood-strial barriers. Intratympanic applications provide an alternative approach in whi

Dysfunction of the cochlea contributing to hearing loss in acoustic neuromas: an underappreciated entity.

OBJECTIVE: Hearing loss is a common symptom in patients with cochleovestibular schwannoma. Clinical and histologic observations have suggested that the hearing loss may be caused by both retrocochlear and cochlear mechanisms. Our goal was to perform a detailed assessment of cochlear pathology in patients with vestibular schwannoma (VS). STUDY DESIGN: Retrospective analysis of temporal bone histopathology. SETTING: Multi-center study. MATERIAL: Temporal bones from 32 patients with unilateral, sporadic VS within the internal auditory canal. MAIN OUTCOME MEASURES: Sections through the cochlea

Two-phase endolymphatic hydrops: a new dynamic guinea pig model.

The classical guinea pig model for Meniere's disease, in which endolymphatic hydrops was achieved by destruction of the endolymphatic sac and obliteration of the endolymphatic duct, is a non-physiological profound model with shortcomings in relation to Meniere's disease as seen in patients. We developed a more subtle animal model; the two-phase endolymphatic hydrops. This model is based on a combination of chronic endolymphatic sac dysfunction, induced by slight destruction of the most distal part of the endolymphatic sac, and acute stress-induced endolymph production by stimulation of the Na/

[The development and application of MRI of the inner ear after gadolinium injections in the diagnosis of Meniere's disease].

Due to the lack of objective examination method, the diagnosis of Meniere's disease mainly relies on subjective symptoms. MRI of the inner ear after gadolinium injections had become a new technology in otology in recent years. The image can clearly distinguish perilymph from endolymph in the labyrinth and can be applied to the imaging diagnosis, evaluation of treatment and the pathogenesis research of Meniere's disease. This article is a review of the development and the application in clinical research of this new technology.

Endolymphatic sac enhancement. Principles of diagnosis and treatment.

Principles of diagnosing and treating Meniere's disease using endolymphatic sac enhancement are described. Based on a current understanding of the natural history and pathogenesis of this disease, the terminology, diagnosis, and thereby are discussed. Techniques (methods) and surgical objectives of endolymphatic sac enhancement are designed to counteract the pathogenesis (endolymph malabsorption). Current surgical methods and results are presented for 100 patients selected at random from a larger group. Discussion of this method of treatment and reporting criteria follow.

Effect of 4 kHz tone exposure on the guinea pig inner ear: relation in the change of cochlear microphonics, action potential, electrochemical potential and K+ ion concentration induced by noise exposure.

Using 110 male albino guinea pigs, and applying electrophysiological methods, we investigated the effects of 4 kHz tone at moderate sound pressure level on the inner ear at different exposure times (6, 12, 24 and 48 h). Longer than 12 h exposure to 4 kHz tone had a considerable effect on the hearing, inducing a decrease in the maximum output voltage of cochlear microphonics and an increase in the K+ concentration in the endolymph.

Effects of glycerol on the inner ear fluid electrolytes of guinea pigs--oral and intravenous administration.

Under sodium pentobarbital anesthesia (25-35 mg/kg), serum, CSF and inner ear fluids from scala tympani perilymph, scala vestibuli perilymph, and scala media endolymph were collected from normal guinea pigs after the oral administration of glycerol (50%, 12 ml/kg) or the intravenous injection of glycerol (1.0 ml/kg). The sodium and potassium concentrations were assessed by microflame photometry. The electrolyte dynamics of the inner ear fluids were compared after these two routes of administration. Serum: There was no significant change in Na or K levels after either route of administration. C

Pathophysiologic theories in the etiology of Meniere's disease.

The etiology of the attacks of vertigo that occur in Meniere's disease is discussed in this article. None of the current theories remain tenable in view of recent findings concerning the physiology and pathophysiology of the cochlea and endolymphatic sac. A new theory suggests that a narrowed duct becomes obstructed by debris that is cleared by a combination of the secretion of hydrophillic proteins within the sac and a hormone, saccin, that increases the volume of endolymph within the cochlea. It is proposed that the sudden restoration of longitudinal flow initiates the attacks of vertigo.

Critical role of SDF-1/CXCR4 signaling pathway in stem cell homing in the deafened rat cochlea after acoustic trauma.

Previous animal studies have shown that stromal cell-derived factor-1 (SDF-1)/CXC chemokine receptor-4 (CXCR4) signaling pathway plays an important role in the targeted migration of bone marrow-derived mesenchymal stem cells (BMSCs) to the injured area. In the present study, we aimed to investigate the potential role of chemotactic SDF-1/CXCR4 signaling pathway in the homing of transplanted BMSCs to the injured cochlea after noise-induced hearing loss (NIHL) in a rat model. White noise exposure (110 dB) paradigm was used for hearing loss induction in male rats for 6 hours in 5 days. Distortion

Regulation of Fn14 Receptor and NF-κB Underlies Inflammation in Meniere's Disease.

Meniere's disease (MD) is a rare disorder characterized by episodic vertigo, sensorineural hearing loss, tinnitus, and aural fullness. It is associated with a fluid imbalance between the secretion of endolymph in the cochlear duct and its reabsorption into the subarachnoid space, leading to an accumulation of endolymph in the inner ear. Epidemiological evidence, including familial aggregation, indicates a genetic contribution and a consistent association with autoimmune diseases (AD). We conducted a case-control study in two phases using an immune genotyping array in a total of 420 patients wi
