Peripheral Vestibular Organ Degeneration After Temporal Bone Fracture: A Human Otopathology Study.

OBJECTIVES/HYPOTHESIS: Vestibular symptoms are a common sequela of temporal bone fractures (TBFs). The mechanisms of injury to the peripheral vestibular system following TBF, however, are not well described. Herein, we aimed to investigate the histopathology of the peripheral vestibular system in patients who sustained TBFs. STUDY DESIGN: Retrospective human specimen analysis. METHODS: Specimens from the National Temporal Bone Pathology Registry with (cases) and without (controls) TBFs were evaluated. Specimens were analyzed by light microscopy for vestibular hair cell and/or dendritic degen

[Electrocochleography in experimental animal model with acute hydrolabyrinth].

A desirable animal model of acute hydrolabyrinth was made by injecting artificial endolymph into the cochlear duct of guinea pigs. In 12 animals with intact Reissner membranes, the endolymphatic potential kept normal after injection of artificial endolymph, but their ECochG showed a rise of SP amplitude, a decrease in AP amplitude, an increase of SP/AP ratio, and a delay of N1 latency. In 7 animals with ruptured membranes, the EP significantly decreased. ECochG showed a decrease in hearing in all test frequencies, a distortion of SP-AP wave or even a loss of AP. Based on the results of our exp

Temporal bone findings in a family with branchio-oto-renal syndrome (BOR).

A family group with confirmed branchio-oto-renal (BOR) syndrome was investigated in this study. Computerized tomography of the temporal bones has demonstrated that the malformations of the inner ear consist of hypoplastic structural changes within the cochlea with reduced vertical diameters, and absent or hypoplastic semicircular canals and normal endolymphatic ducts. It is concluded that in the present cases, the Mondini malformation of the cochlea is not associated with the BOR syndrome.

Bast's valve: what we know and what we don't.

PURPOSE: To synthesize current knowledge on the utriculo-endolymphatic valve (UEV, Bast's valve)-a flap-like structure at the junction of the utricular and endolymphatic ducts and to address the central question of whether it represents a passive anatomical remnant or an active regulator of endolymphatic homeostasis with implications for disorders such as Ménière's disease. METHODS: A critical narrative review of anatomical, imaging, experimental, computational, developmental, comparative, and pathological studies was conducted. Literature was identified through PubMed/MEDLINE, Embase, Web of

Ductus Reuniens and Its Possible Role in Menière's Disease.

OBJECTIVE: After 160 years the true underlying cause of Meniere's disease remains enigmatic. The aim of our study is to discuss the possible implication of an obstruction of the ductus reuniens as a cause in Menière's disease. METHODOLOGY: We first conducted an historical study of the description of the ductus reuniens. We then reviewed the literature regarding ductus reuniens obstruction in animal experiments, human post-mortem studies and living ear imaging. We completed its description by modern microCT imaging. Limited knowledge on the fate of dislodged saccular otoconia is summarized. Th

Transection and occlusion of lateral semicircular canal in guinea pigs.

Audiologic and histopathologic examinations were made after occlusion of the lateral semicircular canal in eight guinea pigs. The lateral semicircular canal was drilled out, and then the canal lumens were plugged with muscle pieces. After a serial recording of auditory brain stem responses for 2 months, histologic specimens of the temporal bones were prepared in the lateral semicircular canal plane. One animal developed profound hearing loss due to suppurative labyrinthitis. The other seven animals showed no significant threshold elevation during this period. Histopathologic examination reveal

Glycerol-induced changes in the cochlear responses of the guinea pig hydropic ear.

Pigmented guinea pigs were chronically implanted with bilateral round-window electrodes. Endolymphatic hydrops was induced by obstruction of the endolymphatic duct on one side. At 1-5 months post-surgery, glycerol was orally administered to each animal and the acute effect on cochlear and eighth nerve responses was investigated. Doses from half to four times the clinical dose of glycerol failed to improve the CAP audiogram in operated ears and were often found to raise the thresholds. Glycerol was found to diminish the amplitude of the summating potential as well as that of the compound action

History of Meniere's disease and its clinical presentation.

The term Meniere's disease is used to define either the classic triad of vestibular and cochlear symptoms and aural pressure from known or unknown causes or its clinical variants, vestibular and cochlear Meniere's disease. Some variants evolve after years into typical forms, whereas others do not. Some symptoms (positional vertigo) have been long underestimated in previous reports. The more we study our patients and correlate clinical findings and the natural history with pathologic studies on temporal bones and laboratory research, the more we will understand Meniere's disease and its causes.

[Surgical strategy in oncologic diseases of organs of the pancreatobiliary zone complicated by mechanical jaundice].

In patients operated upon for oncological diseases of organs of the pancreatobiliary zone complicated by mechanical jaundice the lethality was decreased in a group of patients with purulent cholangitis and high biliary hypertension by means of using a two-stage method of operative treatment, use of the dosaged decompression of the bile ducts with a gradually increased diameter of the drain from 4 to 7-8 mm being most effective. A good effect of the application of complex antibacterial therapy was also noted when using a direct endolymphatic administration and intracholedochal perfusion of anti

Communication between cochlear perilymph and cerebrospinal fluid through the cochlear modiolus visualized after intratympanic administration of Gd-DTPA.

PURPOSE: Intratympanic injection of gadolinium diethylenetriaminepentaacetic acid (Gd-DTPA) has been reported as a procedure to visualize endolymphatic hydrops of Meniere's disease. We frequently noted that cerebrospinal fluid (CSF) in the internal auditory canal (IAC) was also enhanced after this procedure. The purpose of this study was to evaluate how frequently this occurs and to investigate the specific features of patients who lack this communication. MATERIALS AND METHODS: A total of 25 patients with clinically suspected endolymphatic hydrops underwent the procedure. After 24 h, three-d

Microvasculature of normal and hydropic labyrinth.

The microvasculature of the inner ear in guinea pigs and humans was observed with a scanning electron microscope using corrosion casting method. Alterations in the inner ear vasculature which occurred in association with experimental endolymphatic hydrops were also investigated. The results thus obtained are summarized as follows: 1. In the cochlea and vestibule, the arteries, coiled arterioles, and the veins are endowed with their respective characteristic morphologic features and play a role in the regulatory mechanisms of circulation. 2. The point in humans which is most different from guin

Meniere's disease: new concepts, new treatments.

Our understanding of Meniere's disease is being revamped. For decades, the condition was thought to be caused by excessive fluid retention (hydrops) in the endolymphatic spaces of the inner ear, which led to tears or ruptures of the membranous structures that affect hearing and balance. More recently, research has shown that hydrops is not always associated with Meniere's and ought not to be considered the ultimate cause of its symptoms. New theories center on the fact that Meniere's disease may not have a single cause but may well be a common endpoint of a variety of anatomic or physiologic v

Cochleostomy or endolymphatic sac surgery for advanced Meniere's disease.

The operation of cochleostomy is described. The surgical technique is vital for the true assessment of results. If the cochlear duct is punctured, the risk to hearing is unacceptably high. If only the osseous spiral lamina is punctured, the results are acceptable in the short term, though the rationale cannot be explained. In the management of difficult patients with more advanced Meniere's disease, some variant of cochleostomy should probably be included in the therapeutic options. The results of endolymphatic sac surgery are significantly better in a matched group of patients.

Electrophysiological measures of cochlear function in guinea pigs with long-term endolymphatic hydrops.

Experimental endolymphatic hydrops was induced in guinea pigs by obliteration of the endolymphatic sac and duct. From 3 to 24 months after this operation, cochlear action potential (AP) audiograms and AP tuning curves were measured. The purpose of this study was to establish parallels, if any, between this supposed animal model of Menière's disease and the auditory symptoms of the disease in man. In some animals, low and middle frequency AP threshold elevations were observed whilst higher frequency regions maintained normal sensitivity. Other animals developed flat or very gradually sloping AP

Detailed MR imaging assessment of endolymphatic hydrops in patients with SLC26A4 mutations.

OBJECTIVE: Mutations in SLC26A4 represent the second most common mutations in deafness patients. The majority of patients with SLC26A4 mutations have a large vestibular aqueduct (LVA). Recently, some reports showed the presence of endolymphatic hydrops (ELH) in patients with LVA on the basis of high-resolution enhanced 3T-MRI. However, detailed evaluation has not been performed. We provide the first report on ELH in LVA patients with biallelic SLC26A4 mutations. In this study, we focused on 1) the findings of ELH in LVA patients with biallelic SLC26A4 mutations, and 2) the findings of the endo

Significance of the development of the inner ear third window effect after endolymphatic sac surgery in Ménière disease patients.

OBJECTIVES/HYPOTHESIS: The goal of this study was to identify the clinical significance of the low-frequency air-bone gap (LFABG) that often develops after endolymphatic sac surgery. STUDY DESIGN: Prospective study. METHODS: Sixteen patients who had been diagnosed with definite Ménière disease and underwent endolymphatic sac surgery were studied. The surgical outcome was evaluated based on the 1995 guidelines of the American Academy of Otolaryngology-Head and Neck Surgery. The number of patients who developed LFABGs (a mean air-bone gap >10 dB HL at 250, 500, and 1,000 Hz) after surgery was

OBJECTIVE: To investigate a method of quantification of the endolymphatic hydrops in experimental animal model. METHODS: Thirty guinea pigs were divided into three groups at random. In control group, there were ten guinea pigs without operation on both ears. Endolymphatic hydrops was induced by endolymphatic sac obliteration through extradural posterior cranial fossa approach in right ear, including 4-week postoperative group(n = 10) and 8-week postoperative group(n = 10). The area of scala vestibuli (SV) and scala media(SM) of each turn on both cochlear midmodiolar sections was measured, res

Effects of endolymphatic mastoid shunt operation for patients with Menière's disease.

During the past 10 years, the endolymphatic mastoid shunt operation was carried out on 108 patients with Meniere's disease: 54 men and 54 women between 22 and 72 years old. According to criteria AAOO proposed in 1972, 86 cases (79.6%) belonged to class A, 19 cases (17.7%) to class B and 3 cases (2.7%) to class C. Forty patients took the body sway test before and after the operation. Four of the 40 patients were found to have Meniere's disease on the contralateral side within 12 months after the operation and one patient was found to have a complicating psychogenic disease. The abnormal body sw

Pathophysiologic theories in the etiology of Meniere's disease.

The etiology of the attacks of vertigo that occur in Meniere's disease is discussed in this article. None of the current theories remain tenable in view of recent findings concerning the physiology and pathophysiology of the cochlea and endolymphatic sac. A new theory suggests that a narrowed duct becomes obstructed by debris that is cleared by a combination of the secretion of hydrophillic proteins within the sac and a hormone, saccin, that increases the volume of endolymph within the cochlea. It is proposed that the sudden restoration of longitudinal flow initiates the attacks of vertigo.
