[Endolymphatic sac tumour as an infrequent cause of Ménière's syndrome].

Endolymphatic sac tumours are uncommon. They have been classified as adenocarcinomas with a low degree of malignancy and no metastases have yet been documented. We report on a female patient with Von Hippel-Lindau disease and Ménière's syndrome suffering from an endolymphatic sac tumour. Diagnosis and early treatment are essential to preserve hearing, so long-term monitoring is recommended when this clinical combination is encountered.

A further critical assessment of the efficacy of endolymphatic sac surgery.

This paper further assesses the efficacy of two selected techniques of endolymphatic sac surgery (ELS) employing implants for the treatment of classic Meniere's disease and endolymphatic hydrops: the Arenberg inner ear valve implant and the newly developed Huang/Gibson inner ear shunt. Long-term (8-10 years) results for 57 patients given the Arenberg inner ear valve implant show that our demonstrated high success rate in achieving control of symptoms over the short term (1 year) is long-lasting and that although previously reported hearing gains have decreased in some cases, the overall long-t

Ultrastructural evidence of a merocrine secretion in the human endolymphatic sac.

The results of a light and transmission electron microscopic analysis of an endolymphatic sac (ES) from a patient suffering from episodic vertigo, tinnitus, and hearing loss are presented. A biopsy of the intraosseous portion of the ES was obtained during a translabyrinthine approach to section the vestibular nerve in the internal acoustic meatus. The material consisted mainly of tubular epithelial structures filled with heavily stained material. Pathologically dilated and degranulated rough endoplasmic reticuli and disaggregation of polyribosomes with accumulation of solitary ribosomes in the

Endolymphatic sac surgery in congenital malformations of the inner ear.

A retrospective analysis of 40 patients (49 ears) with congenital progressive sensorineural hearing loss who underwent endolymphatic sac surgery was performed. The inner ears were radiographically abnormal in 57% of operated ears. In the remaining cases, subtle malformations beyond the resolving power of radiographic studies were suspected. Early postsurgical hearing loss (defined as a loss greater than 10 dB in three-tone average or greater than 15% in speech discrimination score) was found in 29% of operated ears (14/49). However, only two of these patients lost all of their residual hearing

Vascularity of endolymphatic sac in Meniere's disease. A histopathological study.

The vascularity of the endolymphatic sac (ES) in the vestibular aqueduct (VA) and of periaqueductal bony channels was studied in 18 temporal bones from patients with Meniere's disease and in the same number of control bones with no endolymphatic hydrops and no premortem history of otologic disease. The control bones were selected on the basis of having VAs of the same sizes as the hydropic bones. This was important in order to eliminate any possibility that differences in degree of vascularity of the ES could have been due simply to differences in the degree of development of the VA. All speci

Vascular permeability of the stria vascularis and morphology of the endolymphatic sac in two types of experimental endolymphatic hydrops.

We produced two kinds of experimental endolymphatic hydrops. One was induced by obliteration of the endolymphatic sac (ES), and the other was induced by both systemic keyhole limpet hemocyanin immunization and secondary keyhole limpet hemocyanin challenge into the ES. The vascular permeability of the stria vascularis in acute and chronic phases was compared between both models by light and electron microscopy using the tracer method of horseradish peroxidase. Both models showed that the number of strial capillaries showing horseradish peroxidase leakage was significantly higher in the acute ph

von Hippel-Lindau disease: surveillance strategy for endolymphatic sac tumors.

PURPOSE: : Up to 16% of patients with the hereditary von Hippel-Lindau disease develop endolymphatic sac tumors of the inner ear. Early diagnosis and treatment of endolymphatic sac tumors can prevent audiovestibular morbidity, but optimal endolymphatic sac tumor surveillance strategy has yet to be determined. We aimed to evaluate endolymphatic sac tumor surveillance to determine the best surveillance strategy. METHODS: : In a national prospective study, 40 VHL mutation carriers were interviewed about audiovestibular symptoms and had audiological examinations and magnetic resonance imaging of

Extraosseous endolymphatic sac low-grade adenocarcinoma mimicking posterior fossa meningioma.

Low-grade adenocarcinoma arising from the endolymphatic sac is an uncommon tumor that is characterized clinically by hearing loss and radiologically by temporal bone destruction. The authors report a case of low-grade adenocarcinoma of endolymphatic sac origin that mimicked a posterior fossa meningioma in both clinical and neuroimaging examinations. In this case, the most unusual and interesting feature was the lack of auditory symptoms and temporal bone destruction. The lesion occurred in a 21-year-old man who presented with headaches that had been increasing in frequency for 6 months and was

Outcome-based assessment of endolymphatic sac surgery for Meniere's disease.

This study reports the preliminary results of an outcomes approach to analysis of the effectiveness of endolymphatic sac decompression (ELSD). Using the medical outcomes survey's 36-item, short-form health survey (SF-36), we assessed the quality of life in 33 patients with disabling Meniere's disease undergoing ELSD for medically intractable vertigo. Results indicate that patients with 1995 American Academy of Otolaryngology--Head and Neck Surgery (AAO/HNS) class A or B outcomes showed no significant difference from population norms on the SF-36, whereas patients with AAO/HNS classes C to E ou

[Long-term result of endolymphatic sac drainage for Ménière's disease].

Ten cases of Ménière's disease were operated with endolymphatic mastoid shunt. All patients were followed up 3 to 5 years postoperatively. The results showed that all patients obtained satisfactory relief of Vertigo, and in most of the cases the symptom of the tinnitus was diminished or vanished, only one patient demonstrated hearing loss. The advantages of this surgery are: shorter hospitalization, fast recovery and a low potential for serious intracranial complications. The relation between the type of endolymphatic sac and the result of operation was discussed.

Ménière's disease: endolymphatic sac decompression compared with sham (placebo) decompression.

The placebo effect in surgery for Ménière's disease was investigated in a double-blind, controlled surgery by comparing the effect of a regular endolymphatic shunt with the effect of a purely placebo operation (regular mastoidectomy). Thirty patients with typical Ménière's disease participated in the study. They were selected for surgery because of unsuccessful medical treatment and were chosen randomly for each treatment group. The patients filled in daily dizziness questionnaires for 3 months before and 12 months after surgery, registering nausea, vomiting, vertigo, tinnitus, hearing impairm
