The clinical findings to notice mild elevation of intracranial pressure in an otology clinic.

Mildly elevated intracranial pressure appears to be a distinct pathology separate from idiopathic increased intracranial pressure and migraine. Many patients present with head fullness-pressure and dizziness, which is often suggestive of a clinical diagnosis of vestibular migraine. These patients may additionally have episodic vertigo as seen in endolymphatic hydrops and positional vertigo in addition to feeling dizzy. In most cases, hearing is normal. A long duration of the condition or a presence of associated ear pathologies, i.e., dehiscence, fistula, hypermobile footplate, or history of c

Psychologic evolution of patients with Menière's disease in relation to therapy.

The state of anxiety experienced by menieric patients indicates the importance of the psychologic component in this disease. Uncertainties as to the type of treatment that is actually curative for this disease have put doubts on the favorable outcome for patients referred for endolymphatic shunt procedures; some clinicians claim that in such cases surgery simply had a psychologic support function. To evaluate the effectiveness of such a statement, the authors have studied three groups of patients: patients who underwent surgery; patients who underwent medical treatment; and nonmenieric, otoscl

Middle fossa vestibular nerve section in the management of Ménière's disease.

The triad of fluctuating hearing loss, tinnitus, and episodic attacks of vertigo known as Ménière's disease can be a frustrating therapeutic problem. While most patients respond to a medical regime consisting of a low salt diet, diuretics, diazepam, and propantheline bromide, there is a significant group who remain refractory to conservative management. These individuals either continue to experience their distressing symptoms or undergo some type of surgical procedure in an attempt to seek relief. There are a multitude of such techniques available to the otologic surgeon and he must make the

[The effects of small dose of intratympanic gentamicin injection on intractable Meniere's disease].

OBJECTIVE: To explore the efficacy of small dose of intratympanic gentamicin injection on intractable Meniere's disease. METHOD: Retrospectively study the efficacy of gentamicin intratympanic injection on intractable Meniere's disease. The end-point of treatment was determined based on bedside tests (Spontaneous Nystagmus test, Head shaking test and Head Thrust test), hearing tests, or the improvement of patients symptoms. RESULT: Nineteen patients with Meniere's disease were treated with intratympanic gentamicin injection. Vertigo control was achieved in 17 patients (89%). Of them, vertigo

[Surgical treatment of morbus Menière (author's transl)].

Surgical treatment is only indicated if the medical treatment is not successful. From 409 patients an operation was only necessary in 90 cases. Only those operations for which there is a long period of experience were mentioned. The early stage of the disease with a fluctuating hearing loss is reversible. It is the stage in which saccotomy with myo-synangiosis is indicated. A follow-up study of about 5 years ago showed that vertigo disappeared in 48% of cases. The hearing remained unchanged in 48% and decreased in 52% of cases. The tinnitus was completely relieved in 8% and the fullness of the

Noise-induced hearing loss in the contralateral ear during otologic and neurotologic surgeries.

OBJECTIVE: Noise-induced hearing loss in the non-surgical ear during otologic/neurotologic surgery has not been well studied. The purpose of this study was to evaluate changes in hearing that may occur in the contralateral (i.e., non-surgical) ear after various otologic/neurotologic surgeries due to noise generated by drills. We hypothesized that otologic/neurotologic surgeries, longer in duration, would suggest longer drilling times and result in decreased hearing in the contralateral ear as evidenced by a change post-operative pure tone air conduction thresholds when compared to pre-operativ

The subcellular distribution of aquaporin 5 in the cochlea reveals a water shunt at the perilymph-endolymph barrier.

Aquaporins are membrane water channel proteins that have also been identified in the cochlea. Auditory function critically depends on the homeostasis of the cochlear fluids perilymph and endolymph. In particular, the ion and water regulation of the endolymph is essential for sensory transduction. Within the cochlear duct the lateral wall epithelium has been proposed to secrete endolymph by an aquaporin-mediated flow of water across its epithelial tight junction barrier. This study identifies interspecies differences in the cellular distribution of aquaporin 5 (AQP5) in the cochlear lateral wal

Intratympanic gentamicin therapy for Menière's disease placed by a tubal catheter with systematic isosorbide.

In 1974 (6), 6 patients with incapacitating unilateral Meniere's disease were given an empiric treatment with intratympanic gentamicin sulfate via the eustachian tube using a catheter with a small side-branch. These patients showed excellent results, gaining relief from vertigo over a 13-year period. Since 1980 (7. 8), we have treated patients suffering from Menière's disease with isosorbide. When the patients could not be controlled by this therapy, isosorbide was given in addition to intratympanic gentamicin therapy using a tubal catheter. Of 75 patients with Menière's disease who received g

Natural history vs. surgery for Menière's disease.

Menière's disease has an episodic course, and certain patients undergo spontaneous remission of their vertigo. A retrospective study of patients treated from 1974 to 1983 was undertaken to evaluate the long-term outcome of patients with Menière's disease for whom surgery was recommended, comparing those who had a surgical procedure with a similar group of patients who declined surgery. The surgical procedures performed were endolymphatic subarachnoid shunt (ELS), retrolabyrinthine vestibular neurectomy (RVN), middle fossa vestibular neurectomy (MFVN), and transmeatal cochleo-vestibular neurect

[Ablation therapy with gentamycin in the treatment of Meniere's disease].

Historically the therapy for untreatable Ménière's disease (Md) has been surgery: conservative (cochleosacculotomy and decompression or sacco-endolymphatic shunt) or radical (labyrinthectomy and vestibular neurectomy). A valid alternative to surgery is the use of ototoxic drugs such as aminoglycoside antibiotics based on the assumption that some of the drugs exert greater ototoxicity vs. the posterior labyrinth. The purpose of the present study was to verify a severe protocol for the intratympanic administration of gentamicin sulfate in the treatment of Md unresponsive to medical treatment. A

Surgical treatment of vertigo by utriculostomy: an experimental study in sheep.

The objective of this study was to develop and test an endolabyrinthic microsurgical procedure for treatment of vertigo, the utriculostomy. This involves the application of local heat for obtaining a fistula in the membranous labyrinth, so as to establish communication between the endolymphatic and perilymphatic spaces at the utricle level. Before the procedure, an experimental model using quail eggs was built for pre-evaluation, and macroscopic and histological studies were performed in the temporal bones of three healthy sheep. Following this, the utriculostomy was performed through the oval
