Incidental findings of thickening luminal gastrointestinal organs on computed tomography: an absolute indication for endoscopy.

OBJECTIVES: Incidental findings of thickened luminal GI organs (LGIO) on CT are not uncommon. However, the significance of these findings is unclear. Because of the lack of scientific data, there are no clinical guidelines for the evaluation of these radiologic abnormalities. Our objective was to determine whether endoscopic evaluation of these findings revealed significant abnormalities. METHODS: This study evaluated all incidental findings of thickened LGIO in a large medical center from October, 1997 to March, 1999 that were followed by endoscopic examinations. RESULTS: Ninety-six percent

THE RADIOGRAPHIC AND ENDOSCOPIC ANATOMY AND DIGESTIVE MECHANISMS OF CAPTIVE AFRICAN PENGUINS (SPHENISCUS DEMERSUS).

The anatomy of the avian gastrointestinal (GI) tract is uniquely suited to each species' dietary requirements. African penguins (Spheniscus demersus) are charismatic and popular exhibit animals. As their prevalence grows, there is a need to understand their unique digestive tract to diagnose abnormalities. Reference material specific to the digestive tract of piscivores is scant, and knowledge of the GI tract of a healthy penguin is based on information from other birds. The purpose of this study is to determine the normal gross anatomy, transit time, and histopathologic structures of the peng

Advances in the diagnosis of GERD.

Gastroesophageal reflux disease (GERD) is considered one of the most frequent chronic gastrointestinal diseases globally with high costs due to treatment and investigations.First line therapy is with proton pump inhibitors, those who do not respond to initial treatment usually require further investigations such as upper gastrointestinal endoscopy or ambulatory 24-hours esophageal pH monitoring. The total time of exposure to acid and the DeMeester score represent the most useful parameters associated with conventional pH-metry, because they can identify gastroesophageal reflux disease.Although

Use of SpyGlass for peroral cholangioscopy in the diagnosis and treatment of hepatobiliary diseases in over five years follow-up: A single centre experience.

BACKGROUND/AIMS: The use of cholangioscopy for the diagnosis and treatment of hepatobiliary diseases is gradually becoming more common. We aimed to review our peroral cholangioscopy interventions, using the first-generation SpyGlass Direct Visualization System (SDVS) and summarize our experience in terms of procedures and results. MATERIALS AND METHODS: Forty-one patients who underwent this procedure at our Gastroenterology Clinic between February 2010 and October 2014 were included in this study. Patients were monitored for a median (IQR) of 44 (range 38-72) months. Demographic characteristi

Diagnostic and therapeutic endoscopy in children and adolescents with cancer.

UNLABELLED: Chemotherapy regimes in children with cancer often cause gastrointestinal side effects. Only limited data exist on the use of endoscopy in this group of patients. METHODS: Retrospective review over a time period of 8.5 years identified 57 endoscopies (49 upper endoscopies, four colonoscopies, three sigmoidoscopies, one rectoscopy) in 38 patients (mean age 12.8 years). Seventeen children (45%) had hematological malignancies and 21 (55%) had solid tumors. In 12 children, platelet count was <50 × 10(9)/L and 10 children were neutropenic (ANC < 1 × 10(9)/L; absolute neutrophil count).

Endo-Hepatology: Paradigm Shift in Endosonographic Diagnosis for Liver Disease to Digestive Endoscopy.

Assessment of liver disease has traditionally relied on a fragmented approach involving cross-sectional imaging, noninvasive fibrosis assessment, liver biopsy, and endoscopy for complications of portal hypertension. Although effective, this strategy often requires multiple procedures and lacks integration. Advances in endoscopic ultrasound (EUS) have led to the emergence of endo-hepatology, an integrated model enabling comprehensive evaluation and management of liver disease within a single endoscopic platform. This review highlights recent advances in endo-hepatology and the expanding diagnos

Endoscopic Step-Up Approach in Management of Necrotizing Pancreatitis.

The endoscopic step-up approach in the management of necrotizing pancreatitis involves sequential steps of intervention at different time points in the clinical course of the disease. EUS -guided drainage of walled-off necrosis is the first step of the endoscopic step-up approach. Lumen-apposing metal stents are preferred over plastic stents for safe and effective drainage because of their wide caliber. Successive steps in the endoscopic step-up approach include direct endoscopic necrosectomy and/or irrigation using naso-cystic tube in symptomatic patients, primarily based on necrotic debris c

[Features of gastroesophageal reflux disease in elderly patients (literature review).].

In recent years, there has been an increase in the incidence of gastroesophageal reflux disease (GERD) in the elderly not only in our country, but also worldwide. In this regard, the issues of early diagnosis and development of an individualized approach to the treatment of this disease are of particular relevance. When addressing these issues, it is necessary to take into account a number of features in patients of older age groups, which include polymorbidity and polypharmacotherapy in combination with anatomical, physiological and functional age-related changes. These features are primarily

Potential versatile uses of a novel ultra-thin peroral cholangioscope.

Tonozuka and colleagues report the usefulness of a newly developed ultra-thin mother-baby type peroral cholangioscope with a tip external diameter of 2.3 mm for a case of biliary stricture in which conventional peroral cholangioscope insertion was challenging. The novel scope allows simple and low-cost peroral cholangioscopy, making it highly versatile.

Asymptomatic benzocaine spray-induced methaemoglobinaemia in preoperative sedation for oesophagogastroduodenoscopy.

Methaemoglobinaemia is defined as elevated methaemoglobin in the blood which is characterised by conversion of some of the reduced ferrous iron elements [Fe2+] to the oxidised ferric [Fe3+] form which does not have capacity to bind and transport oxygen resulting in functional anaemia. Causes can be genetic mutations or acquired by medications such as dapsone, nitrates or benzocaine. Benzocaine is currently being used as a topical anaesthetic agent before certain procedures. We report a case of benzocaine spray-induced methaemoglobinaemia in a patient who underwent oesophagogastroduodenoscopy f

Retained Food During Esophagogastroduodenoscopy Is a Risk Factor for Gastric-to-Pulmonary Aspiration.

BACKGROUND: Residual food (RF) during esophagogastroduodenoscopy (EGD) is thought, but not proven, to be a risk factor for gastric-to-pulmonary aspiration. AIMS: The aims of this study were to determine the prevalence of RF during EGD, to investigate whether RF was associated with an increased risk of aspiration, especially when monitored anesthesia care (MAC) or general anesthesia (GA) were administered, and to determine whether aspiration associated with RF led to a more severe clinical outcome. METHODS: Patients undergoing EGD between October 2012 and September 2018 were identified. Patie

A Computer-Aided Method for Digestive System Abnormality Detection in WCE Images.

Источник пометил публикацию как отозванную

Wireless capsule endoscopy (WCE) is a powerful tool for the diagnosis of gastrointestinal diseases. The output of this tool is in video with a length of about eight hours, containing about 8000 frames. It is a difficult task for a physician to review all of the video frames. In this paper, a new abnormality detection system for WCE images is proposed. The proposed system has four main steps: (1) preprocessing, (2) region of interest (ROI) extraction, (3) feature extraction, and (4) classification. In ROI extraction, at first, distinct areas are highlighted and nondistinct areas are faded by us

Clinical features and time trends associated with an endoscopically normal esophagus in active eosinophilic esophagitis.

BACKGROUND: A proportion of patients with active eosinophilic esophagitis (EoE) have a normal-appearing esophagus on esophagogastroduodenoscopy (EGD). We aimed to determine the associations between the baseline clinical features and the endoscopically normal esophagus in EoE, as well as time trends in reporting. METHODS: In this retrospective study of active EoE cases from 2002 - 2018, patients with and without esophageal endoscopic abnormalities were compared. Multivariable logistic regression identified the independent predictors of a normal EGD. The proportion of patients with a normal EGD

Choledochocele observed on direct peroral cholangioscopy using an ultra-slim endoscope.

Highlight Ikarashi and colleagues report a case of choledococele treated by endoscopic sphincterotomy and observed on direct peroral cholangioscopy. Histological examination reveals duodenum-like mucosa without atypical changes in the choledochocele. Direct peroral cholangioscopy using an ultra-slim endoscope may be a useful technique for diagnosis and surveillance of choledochocele.
