Bile acids (BAs) are critical mediators of metabolic and immune regulation, influencing both liver and intestinal function. Their homeostasis, maintained through the enterohepatic circulation, is pivotal for immune-metabolic balance. BAs activate key receptors, including Farnesoid X Receptor (FXR) and TGR5, to modulate inflammation. FXR exerts anti-inflammatory effects by suppressing NF-κB signaling and cytokine production, whereas TGR5 primarily regulates NLRP3 inflammasome activation. Dysregulated BA signaling, driven by microbial dysbiosis, exacerbates inflammatory diseases like non-alcohol

Chronic diarrhea and abdominal pain after radiotherapy continue to be a problem in cancer survivors. Gut microbiomes are essential for preventing intestinal inflammation, maintaining intestinal integrity, maintaining enterohepatic circulation, regulating bile acid metabolism, and absorption of nutrients, including fat-soluble vitamins. Gut microbiome dysbiosis is expected to cause inflammation, bile acid malabsorption, malnutrition, and associated symptoms. Postradiotherapy, Firmicutes and Bacteroidetes phylum are significantly decreased while Fusobacteria and other unclassified bacteria are i

In a 5-day-old full-term, dehydrated boy with bilious vomiting and a cephalhaematoma, bilirubin encephalopathy was diagnosed at a serum bilirubin level of 395 mumol/l. The patient was rehydrated intravenously and treated with phototherapy and an exchange transfusion, after which the serum bilirubin level decreased. The neurological condition normalised during his stay in the hospital. Hyperbilirubinaemia was caused by an increased enterohepatic circulation due to a high intestinal obstruction and resorption of the cephalhaematoma. Toxicity was caused by dehydration and fasting. Even with new b

Metabolism of trichloroethylene (TRI) and its major metabolite, trichloroacetic acid (TCA), by gut content and gut microflora cultures was studied to gain an insight into the role of enterohepatic circulation in TRI metabolism. TRI and TCA were incubated anaerobically with rat and mouse cecal contents and TCA was additionally incubated anaerobically and aerobically with microflora cultures from mice. Although TRI was not metabolized by rat or mouse cecal contents. TCA was metabolized to dichloroacetic acid (DCA) by cecal contents. DCA formation in microflora cultures was dependent on initial T

The major host defense mechanisms against bacterial overgrowth in the small bowel are the normal propulsive activity of the bowel itself and gastric acid secretion. Microbial interactions are a major factor in regulating the indigenous bacterial flora. Studies of the bacterial enzymes of the gut suggest that changes in diet may lead to marked changes in the colonic flora. Antibiotics affect the composition of the colonic microflora. The microflora also influence the degradation of mucin, the conversion of urobilin to urobilinogen, of cholesterol to coprostanol, and the production of short chai

A primate model is described that permits separate but simultaneous sampling of both bile and pancreatic fluid without altering the normal enterohepatic circulation. Simultaneous biliary and pancreatic dose response curves are presented to contrast the production of bile with both pancreatic flow and the secretion of pancreatic protein and bicarbonate. The data show that in the cholecystectomized animal both cholecystokinin and secretin increase bile flow at doses that produce appropriate submaximal responses from the pancreas. Both these hormones, therefore, are physiologically important in r

Bile acids (BAs) have gained mainstream attention since the discovery of their key role as signalling molecules in health and disease. The apical sodium-dependent transporter (ASBT) protein located in the terminal ileum plays an important physiological role in the enterohepatic circulation of BAs and therefore essential for the BA homeostasis. Primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC), the 2 most common cholestatic liver diseases are characterised by altered BA flow and BA composition, which contribute to disease progression and symptom (pruritus) development.

BACKGROUND: Fibroblast growth factor 19 (FGF19) is an enterohepatic hormone the synthesis of which is stimulated by bile acid activation of the nuclear farnesoid X receptor (FXR) in ileal enterocytes. Increased production of FGF19 downregulates hepatocyte bile acid synthesis and gluconeogenesis, while concurrently upregulating hepatocyte glycogenesis and gallbladder (GB) filling. The physiologic impact of this regulatory cycle is illustrated in cholecystectomized humans, in whom the disturbed meal-related flux of GB bile decreases serum FGF19 concentrations.
OBJECTIVE: Determine if serum FGF1

Farnesoid X receptor (FXR) is a ligand-activated transcription factor involved in the control of bile acid (BA) synthesis and enterohepatic circulation. FXR can influence glucose and lipid homeostasis. Hepatic FXR activation by obeticholic acid is currently used to treat primary biliary cholangitis. Late-stage clinical trials investigating the use of obeticholic acid in the treatment of nonalcoholic steatohepatitis are underway. Mouse models of metabolic disease have demonstrated that inhibition of intestinal FXR signalling reduces obesity, insulin resistance and fatty liver disease by modulat

Lithocholic acid (LCA) is a monohydroxy bile acid produced by intestinal flora, which has been found to be associated with a variety of hepatic and intestinal diseases. LCA is previously considered to be toxic, however, recent studies revealed that LCA and its derivatives may exert anti-inflammatory and anti-tumor effects under certain conditions. LCA goes through enterohepatic circulation along with other bile acids, here, we mainly discuss the effects of LCA on the gut-liver axis, including the regulation of gut microbiota, intestinal barrier, and relevant nuclear receptors (VDR, PXR) and G

Here we report therapeutic drug monitoring of cyclosporine in a kidney transplant recipient lacking enterohepatic circulation. The patient developed steroid-resistant nephrotic syndrome at age 14 years, and was medicated with an oral cyclosporine microemulsion. However, her cyclosporine trough level was unexpectedly elevated, and subsequent investigations showed that she was deficient in drug metabolism as a result of the congenital absence of the portal vein. Her renal function gradually decreased and she became dialysis-dependent at the age of 21 years, and kidney transplantation was planned

A 57 year old white male with oncogenous osteomalacia due to a mixed mesenchymal tumor was evaluated by sequential histologic and metabolic studies over a period of 33 mos prior to identifying the location of the tumor. On the basis of these studies we conclude: (i) disorders of the enterohepatic circulation and/or acceleration of metabolism of calcitriol are not responsible for its diminished level in oncogenous osteomalacia, (ii) the Von Kossa stain is preferred to the modified Masson in evaluating osteomalacia, (iii) avascular necrosis of the femoral head may be part of the syndrome, (iv) h

Controversy continues to surround the appropriate form and timing of nutrition support for the patient with circulatory shock. Clinical studies have demonstrated improvements in outcome with the administration of enteral nutrition to critically ill patients; however, the provision of enteral nutrition to critically ill patients with ongoing shock remains controversial. This article reviews gut perfusion during normal states and during circulatory shock as well as alterations in perfusion when enteral feeding is provided. Pharmaconutrients studied during ischemia and reperfusion are discussed.

The gut and the liver are the key organs in nutrient absorption and metabolism. Bile acids, drugs, and toxins undergo extensive enterohepatic circulation. Bile acids play a major role in several hepatic and intestinal diseases. Endotoxins deriving from intestinal Gram-negative bacteria are important in the pathogenesis of liver and systemic diseases. Chronic liver diseases can influence gastrointestinal motility, which together with other factors may contribute to bacterial overgrowth and in patients with ascites to an increased risk of spontaneous bacterial peritonitis. Patients with end-stag

The liver is the junction of several inter-organ metabolic cycles which are essential for the homeostasis of mammalian metabolism. Two of these are described in greater detail and their role in control of lipid metabolism will be presented. The fatty acid-triglyceride cycle is of particular importance for our understanding of the mechanisms governing serum lipid levels. This is due to the fact that the lipoprotein secreted by the liver in the course of this metabolic cycle - very low density lipoprotein - has a relatively long half-life in the plasma compartment. Data have been collected from

The authors have reviewed their cases of neonatal obstructive malformations of the intestinal tract and analyzed the incidence of association with indirect hyperbilirubinemia. 39 patients in four years are considered. In 23 of them there is a bilirubin level greater than 5 mg/100 ml. A great number of high bilirubin levels (greater than 15 mg/100 ml) is associated with volvulus, malrotations and intestinal atresias. Bilirubin levels between 11 and 15 mg/100 ml are often connected with meconium ileus while ano-rectal atresias have a low bilirubin level (less than 5 mg/100 ml). The reported data

Lambda-cyhalothrin (LCT) is a widely used pyrethroid insecticide frequently detected in environmental and food-associated matrices, yet its effects on host bile acid metabolism remain unclear. Male C57BL/6 mice were orally exposed to LCT for 28 days and analyzed using integrated bile acid metabolomics, hepatic and ileal gene-expression profiling, 16S rRNA sequencing, and shotgun metagenomics. LCT reduced hepatic total bile acids but increased plasma and fecal bile acids, indicating compartment-specific bile acid redistribution. This response was accompanied by hepatic Cyp7a1/Cyp27a1 downregula

Partial external biliary diversion in patients with progressive familial intrahepatic cholestasis and Alagille syndrome reduces plasma bile salt levels by modifying their enterohepatic circulation. When this surgical technique is performed before the onset of cirrhosis, the symptoms may improve. It can also modify hepatic fibrosis evolution and, thus, disease progression by avoiding or delaying liver transplantation. We present eight patients undergoing partial external biliary diversion: five patients were diagnosed with progressive familial intrahepatic cholestasis, and three of them with Al

The gut-liver axis is one of the major contributors to the transport of products from the intestine or intestinal microbes with the progression of liver regeneration. However, the influence of proteins from the hepatic portal vein (HPV), the bridge of enterohepatic circulation, on liver regeneration is unclear. For first time, we applied a quantitative proteomics approach to characterize the molecular pathology of the HPV sera of mice with antibiotic-induced intestinal flora disorder during acute liver injury. The biological processes of lipid metabolism and wound healing were enriched in the
