Repeated Recurrence of Spontaneous Cervical Epidural Hematoma Associated with Habitual Strength Training.

Spontaneous spinal epidural hematoma is a clinical entity as a hematoma occurring in the spinal epidural space with unknown etiology. It is known to sometimes show spontaneous regression, and recurrence is rare and repeating even rarer. We had an experience of repeated recurrent spontaneous spinal epidural hematoma, eventually leading to surgery. A 25-year-old man, a habitual exerciser of strength training, had a sudden onset of cervical back pain during sleep the same night after training. Radiological examinations revealed a cervical epidural hematoma but no vascular anomalies, and the patie

Pneumomediastinum: A Rare Complication of Epidural Analgesia.

Pneumomediastinum is a rare complication following epidural block using the loss of resistance (LOR) technique with air. It is speculated to result from the opening of potential space connecting the epidural space and the posterior mediastinum via intervertebral foramina through fascial planes. To date, only two cases of pneumomediastinum after epidural block have been reported. An incidental finding of pneumomediastinum two days after the procedure has not been reported before. Epidural block as a cause should be considered among multiple causes while interpreting the imaging of this life-thr

Challenges with labour epidural placement in a parturient with a lumbar arachnoid cyst: a case report.

This case report describes the management of a patient with a lumbar arachnoid cyst requesting epidural labour analgesia. The patient was otherwise healthy but had suffered from multiple dural punctures during attempted epidural placement in a previous pregnancy. Magnetic resonance imaging following the dural punctures revealed the cyst, which was at the level of the L3-4 interspace, effacing the posterior epidural space at that level. Multidisciplinary management in this subsequent pregnancy, involving radiology and anaesthesia specialists, allowed ultrasound-guided identification of a higher

Transpedicular Approach for Ventral Epidural Abscess Evacuation in the Cervical Spine.

OBJECTIVE: Spinal epidural abscess may require prompt surgical intervention. Ventral cervical abscesses pose a particular challenge regarding the approach for surgical evacuation. The aim of this article was to describe the technical nuances of a posterior transpedicular cervical approach for evacuation of a ventral epidural abscess. METHODS: After a standard laminectomy, a foraminotomy was performed to identify the exiting nerve root. Then the medial aspect of the pedicle below the nerve was drilled. This allowed the insertion of a dissector to reach the ventral epidural space and drain the

Partial displacement of epidural catheter after patient position change: A case report.

Epidural catheter migration is a well-known cause of failed anesthesia and complications. One of the factors that affect catheter movement is when patients change their position after skin fixation. We report a case of an epidural catheter placed without evidence of intravascular or subdural insertion that produced an insufficient block. A 36-year-old woman presented for ankle surgery under epidural anesthesia. Epidural block was conducted at the L3-4 intervertebral space with a catheter threaded 3 cm into the epidural space with the patient in a back flexion and lateral position. The total vo

[The presence of air in epidural hematoma due to congenital bone defect in the mastoid tegmen--a case report].

Presence of air in acute epidural hematomas has been described by several authors. Although the air trapped in the epidural space is thought to be derived from the air-containing structure, there are some cases in which there is an absence of any demonstrable fracture involving air-containing structure. We report a very rare case of air in an epidural hematoma which was not related to fracture involving air-containing structure. The patient was a 30-year-old man whose left temporal bone was struck during a fall. CT scan revealed acute epidural hematoma with a large amount of air in the epidura

Magnetic resonance imaging of the lumbar spine after epidural and nerve root injection therapy: evaluation of soft tissue changes.

OBJECTIVE: Magnetic resonance imaging (MRI) was performed to evaluate soft tissue changes after nerve root and epidural injections in lumbar spine in patients with radiculopathy. MATERIALS/METHODS: Patients underwent a multiple-shot injection protocol to nerve root/epidural space. The MRI protocol included T1-weighted, T2-weighted, STIR, and contrast-enhanced, fat-saturated T1-weighted sequences before and after treatment. RESULTS: In all treated patients, a diffuse wedge-shaped edema in paravertebral tissues with a slight contrast enhancement was seen. Two small hematomas in the paravertebr

Pneumorrhachis associated with bronchial asthma, subcutaneous emphysema and pneumomediastinum.

Pneumorrhachis is defined as the presence of air in the epidural space or subarachnoid space. The air may migrate along fascial planes from the posterior mediastinum, through the neural foramina, and into the epidural space. Pneumorrhachis is rare, and even more so in the paediatric population. Pneumorrhachis in itself usually is asymptomatic, does not tend to migrate and reabsorbs spontaneously. The combination of pneumomediastinum with epidural pneumorrhachis without thoracic trauma has rarely been reported in the literature. The present case report describes the presence of pneumomediastinu

Epidural Contrast Patterns and Clinical Implications: An Educational Review.

PURPOSE OF REVIEW: The purpose of this educational review is to describe the contrast spread patterns that indicate accurate needle placement in the epidural space and spread patterns associated with erroneous needle insertion. RECENT FINDINGS: Epidural injections are minimally invasive and commonly used for patients with acute and chronic back pain that does not respond to conservative management. Imaging with contrast is frequently used during this procedure to improve accuracy and reduce adverse events. Contrast spread patterns are an important tool that can help identify where the needle

Epidural cyst with cauda equina syndrome after epidural anesthesia.

A 40-year-old woman without remarkable medical history received epidural anesthesia for uterine cervix conization. Six hours after the operation, cauda equina syndrome occurred. Magnetic resonance imaging of the spine revealed epidural fluid accumulation around L5, as well as L4/5 herniated intervertebral disc found incidentally. Surgical decompression was performed with H-reflex monitoring. Epidural injection could result in cystic accumulation complicated with cauda equina syndrome.

Epidural navigation in type 1 spontaneous intracranial hypotension: technique and anatomy.

BACKGROUND AND PURPOSE: Spontaneous intracranial hypotension (SIH) with ventral dural tears is associated with spinal longitudinal extradural collections (SLECs). Direct fluoroscopic characterization of the anterior epidural compartment and SLEC morphology in vivo remains limited. We describe wire-catheter epidural navigation and imaging in patients with SIH and SLECs. METHODS: Between June 2023 and December 2025, 20 consecutive patients with type 1 SIH and MRI-confirmed SLECs underwent attempted wire-catheter epidural navigation from a lumbar interlaminar approach. Using a hydrophilic guidew

Epidural anatomy examined by cryomicrotome section. Influence of age, vertebral level, and disease.

BACKGROUND AND OBJECTIVES: Cryomicrotome section is a means of anatomic examination with minimal artifact ideally suited to delineating details of tissue relationships in the epidural space. In the past, healthy adult lumbar levels have been studied by this method. This report extends observations to other regions of the vertebral column, other age groups, and some abnormal conditions. METHODS: The bodies of 26 adults were frozen in toto soon after death, and the bodies of 2 children were frozen after embalming. Unstained anatomy was revealed by sectioning, and the exposed surface was photogr

[Pathogenetic significance and diagnosis of adhesive processes in the radicular syndromes of lumbar osteochondrosis].

A study of reactive adhesive processes in the epidural space, radicles, and membranes of the spinal cord in 309 patients with lumbar osteochondrosis with radicular syndromes revealed hernia of lumbar disks in 296 patients (95.8%). In 45.6% of the patients disk hernias were attended by epiduritis. In 13 patients surgery revealed no disk hernia; one of them had aneurysm of the epidural space vessels, the rest had marked manifestations of epiduritis. In disk hernias with attendant epiduritis damage to two or three neural radicles was observed twice as frequently as in disk hernias without epiduri
