Reusable vs single-use flexible cystoscopes in outpatient urology: a real-world micro-costing and user evaluation.

BACKGROUND AND OBJECTIVE: Flexible cystoscopy is integral to outpatient urology, particularly for surveillance of non-muscle-invasive bladder cancer. Real-world economic data comparing reusable (RU) and single-use (SU) cystoscopes in German outpatient practice are scarce. We evaluated procedure-level costs and multidisciplinary user perceptions under routine conditions. METHODS: A micro-costing analysis was performed in a German outpatient urology centre (337 cystoscopies/year). Capital investment (5-year amortisation), maintenance, reprocessing, consumables, and labour were included. Baselin

Ensuring patient safety during cystocopy: risk assessment of device reprocessing through healthcare failure mode and effects analysis.

INTRODUCTION: Flexible cystoscopy is a widely performed urological procedure, with millions conducted annually. However, cystoscope contamination rates are relatively high, ranging from 2 to 21%, and infections may go undetected. Proper reprocessing of cystoscopes is a critical step in ensuring the safe use of these devices, which underscores the importance of studying it more thoroughly. The aim of this article is to describe the stages of cystoscope reprocessing in the Urology Department of a tertiary Hospital, identifying the potential failures of the system before they occur, quantifying i

Comparison of Physician Perception Between Single-Use Versus Reusable Flexible Bronchoscope for Basic and Advanced Bronchoscopic Procedures Using Low Fidelity Simulation.

BACKGROUND: Single-use flexible bronchoscopes (SUFB) offer potential advantages over reusable flexible bronchoscopes (RFB), including cost-effectiveness, reduced infection risk, and improved accessibility. Although widely used for basic procedures in ICU settings, their performance in advanced procedures remains understudied. METHODS: A single-center pilot study was completed to compare the user experience of VathinH-SteriScope SUFB and Olympus BF-1TH190 RFB among general and interventional pulmonologists using low-fidelity simulation with a mannequin. Participants evaluated scope performance

"Bring your own" reusables to surgery: an environmental sustainability quality-improvement initiative.

BACKGROUND: Operating rooms (ORs) produce more than 30% of a hospital's total waste, with plastic accounting for 20% of that waste. Our project aimed to reduce OR plastic waste by encouraging patients at London Health Sciences Centre sites to bring reusable bags for their belongings and reusable water bottles. METHODS: We used the Plan-Do-Study-Act (PDSA) quality-improvement method, conducting 3 PDSA cycles, first at Nazem Kadri Surgical Centre, and then at Victoria Hospital and University Hospital. Educational interventions informed staff and encouraged patients to bring their own reusable i

Guidelines in Practice: Sterilization.

Proper sterilization of instruments and medical devices is important in ensuring patient safety and preventing surgical site infections. Adhering to the manufacturer's written instructions for use of an item, its packaging, and the sterilizer is essential to ensure instruments and medical devices are properly sterilized. Quality assurance measures should be used to evaluate and monitor sterilization processes and increase patient safety. The recently updated AORN "Guideline for sterilization" provides perioperative nurses with guidance for processing, transporting, and storing reusable medical

Re-Sterilisation of Single-Use Telemetric Devices.

Implantable telemetric transponders for contactless measurement of physiological parameters are often used in animal-based research. After explantation, single-use devices cannot be re-implanted because of non-validated functionality and necessary re-sterilisation. This is disadvantageous because the battery life would enable a second implantation cycle in another animal. To save costs and time taken for the manufacturer's refurbishing process, we validated and implemented a re-sterilisation protocol for single-use transponders using hydrogen peroxide gas. The described protocol was establishe

Reprocessing safety issues associated with complex-design orthopaedic loaned surgical instruments and implants.

BACKGROUND: The acquisition of reusable surgical instruments (RSIs) through loaner system is a worldwide phenomenon, particularly in orthopaedic surgeries. Loaner sets contain high-complex design RSIs, which are very difficult to clean, and also include single use implants, such as screws, that remain in the surgical tray and are subjected to multiple reprocessing until they are implanted. Inadequate cleaning of SI and various exposures of single-use implants to physical, chemical and biological agents can compromise their quality/safety and promote biofilm formation. The difficulty in cleanin

Comparison study of home catheter cleaning methods.

Three methods of cleaning urinary catheters for reuse at home by clients on intermittent catheterization programs were studied. Reused catheters were collected from clients, autoclaved, and then incubated in a culture of Escherichia coli broth. Three different isolates of E. coli were used at concentrations ranging from 4.8 x 10(5) to 1.0 x 10(8) colony-forming units per milliliter. The catheters were then rinsed with tap water for 1 minute and soaked in one of three cleaning solutions for 30 minutes. The three cleaning solutions studied were 0.6% hydrogen peroxide, bleach in a 1:4 solution wi

Reducing environmental contamination during reprocessing of highly contaminated flexible bronchoscopes from the intensive care unit.

BACKGROUND: Flexible bronchoscopes (FBs) used in intensive care units (ICUs) frequently harbour pathogenic micro-organisms, creating early reprocessing risks for environmental contamination and staff exposure. METHODS: In a six-month evaluation of 832 ICU FBs sampled immediately post-use with metagenomic next-generation sequencing, 55.4% carried one or more pathogens, including multi-drug-resistant bacteria, fungi and airborne agents. We instituted an enhanced reprocessing protocol centred on sealed transfer using double biohazard bags, a two-person dedicated handling method that separates co

Duodenoscope-associated infections: a review.

Flexible digestive endoscopes are used for the management of various conditions with hundreds of thousands of therapeutic procedures performed worldwide each year. Duodenoscopes are indispensable tools for the delivery of minimally invasive vital care of numerous pancreaticobiliary disorders. Despite the fact that nosocomial infections after endoscopic retrograde cholangiopancreatography (ERCP) have always been among the most frequently cited postprocedural complications, recent emergence of duodenoscope-transmitted multiple drug-resistant bacterial infections has led to intense research and d

[Perspective of the transition from reusable instruments to single use ancillary in orthopedic surgery].

OBJECTIVES: Since 2009, single-use (SU) ancillaries for total knee arthroplasty (TKA) and total hip arthroplasty (THA) have been marketed to replace reusable ancillaries. The concept is not innovative but their use in orthopedics is still uncommon. An assessment has been done for the use of SU ancillary in Assistance publique-hôpitaux de Paris with the consequences for the patient, the surgeon and the hospital. METHODS: A technical and a clinical review has been done with those devices in 2015. The economic and organizational impacts were identified and submitted for opinion to committee expe

Persistent residual contamination in endoscope channels; a fluorescence epimicroscopy study.

BACKGROUND AND STUDY AIMS: The increasing demand for endoscopic procedures poses new contamination challenges, given developing antimicrobial resistance worldwide and potential viral or prion diseases in populations at risk. We examined working channels from reusable luminal endoscopes used in recent years. METHODS: Very sensitive fluorescence epimicroscopy was used to examine working channels from 6 decommissioned and 6 factory-new channels, as received, or following spiking and washing in the laboratory. RESULTS: After a single contamination and wash test cycle, new channels retained appro

Injection drug use, unsafe medical injections, and HIV in Africa: a systematic review.

The reuse of injecting equipment in clinical settings is well documented in Africa and appears to play a substantial role in generalized HIV epidemics. The U.S. and the WHO have begun to support large scale injection safety interventions, increased professional education and training programs, and the development and wider dissemination of infection control guidelines. Several African governments have also taken steps to control injecting equipment, including banning syringes that can be reused. However injection drug use (IDU), of heroin and stimulants, is a growing risk factor for acquiring H
