[Ischemic heart disease in patients with renovascular hypertension: clinical picture, diagnosis and choice of a suitable method of treatment].

The article analyses the results of examination of 234 patients with vasorenal hypertension and ischemic heart disease. The specific features of the clinical picture, electrocardiographic picture, the results of the bicycle ergometry test, scintigraphy of the myocardium with 201Te at the peak of the exercise, and aorto- and coronarography were revealed and described. The results of the captopril test in these patients were substantiated and analysed and on basis of the obtained data stage-by-stage operative treatment was recommended. It was proved that aorto-coronary shunt must be formed in su

Metabolic changes in patients with mitochondrial myopathies and effects of coenzyme Q10 therapy.

We used a standardized bicycle ergometry protocol with a stepwise increasing workload (30-100 W) to evaluate various metabolic factors for the diagnosis and metabolic monitoring of mitochondrial encephalomyopathies. All patients (n = 9) showed pathological venous lactate/pyruvate (L/P) ratios, which normalized in three patients after 6 months of coenzyme Q10 (CoQ) therapy. Thus, the L/P ratio proved to be the clinically most useful parameter in the evaluation and monitoring of mitochondrial diseases, showing higher sensitivity than lactate measurements only. CoQ may exert a favourable effect i

Cardiopulmonary fitness assessment on maximal and submaximal exercise testing in patients with Fabry disease.

The cardiopulmonary exercise test (CPET) is a valuable tool to assess a patient's aerobic fitness and cardiac function, including the response to stress. There have been few studies using CPET to evaluate cardiopulmonary exercise capacity in patients with Fabry disease. We performed a retrospective chart review of patients with Fabry disease from 2001 to 2016, compared to age, gender, and size-matched normal controls. A total of 18 patients were evaluated using the Bruce protocol (treadmill) and 11 patients were evaluated with the ramp protocol (cycle ergometer). The Fabry group demonstrated s

[Pacemaker therapy in heart failure].

Drug therapy is the standard therapy for heart failure. The current state of the art does not permit pacemaker therapy as a general recommendation for left ventricular failure. Numerous controversial reports have been published on the success of pacemaker therapies for congestive heart failure. Iskandrian reported on tachycardiac atrial stimulation in 1986. Since 1990 several papers have been published on DDD-pacemakers with short AV-time. From the data available to us, congestive heart failure is generally not considered a new pacemaker indication. In carefully selected patients, however, an

"Silent" myocardial ischaemia in essential hypertension.

During 16-h electrocardiographic monitoring of 29 patients with stage II (WHO) essential hypertension and left ventricular hypertrophy, the episodes of painless "silent" myocardial ischaemia were registered in 12. In these patients a bicycle ergometry test, transoesophageal atrial pacing, isotope ventriculography at rest and during isometric exercise, and coronary angiography in 3 cases, were performed. A comprehensive evaluation of findings revealed the presence of coronary insufficiency not accompanied by pain syndrome. Absence of angiographic signs of coronary stenosis in 3 patients subject

Combined intravenous bolus amino acid supplementation and mobilization on early muscle loss in critically ill adults: A randomized controlled trial.

BACKGROUND: Muscle wasting is a hallmark of critical illness. We evaluated the effect of amino acid supplementation and mobilization on early muscle loss in critically ill patients. METHODS: We conducted a randomized parallel-group trial involving adult patients admitted to the surgical intensive care unit of a tertiary hospital. Participants were randomly assigned to two groups, both receiving standard care nutrition and mobilization. The intervention group also received a daily intravenous amino acid bolus with 45 min of cycle ergometry, starting on days 3-4 for a mean of 6 days. The primar

Association between lung ultrasound B-lines and exercise-induced pulmonary hypertension in patients with connective tissue disease.

BACKGROUND: Identification of elevation in pulmonary pressures during exercise may provide prognostic and therapeutic implications in patients with connective tissue disease (CTD). Interstitial lung disease (ILD) is common in CTD patients and subtle interstitial abnormalities detected by lung ultrasound could predict exercise-induced pulmonary hypertension (PH). METHODS AND RESULTS: Echocardiography and lung ultrasound were performed at rest and bicycle exercise in CTD patients (n = 41) and control subjects without CTD (n = 24). Ultrasound B-lines were quantified by scanning four intercostal

High-Intensity Interval Training Is Effective at Increasing Exercise Endurance Capacity and Is Well Tolerated by Adults with Cystic Fibrosis.

BACKGROUND: To optimize outcomes in people with cystic fibrosis (CF), guidelines recommend 30 to 60 min of moderate-intensity aerobic exercise on most days. Accumulating this volume of exercise contributes importantly to the substantial treatment burden associated with CF. Therefore, the main aim of this study was to investigate the effects of low-volume high-intensity interval training (HIIT) on exercise capacity in people with CF. METHODS: This randomized controlled trial included people with CF aged ≥15 years, who were allocated to either eight weeks of thrice-weekly 10-min sessions of HII

Does a brief functional assessment in the emergency department predict outcomes of patients admitted with heart failure? The FASTER-HF study.

BACKGROUND: Evaluation of patients with acute decompensated heart failure includes symptom review, biomarker measurement and comorbidity assessment. Early objective evaluation of functional status is generally not performed. AIM: To investigate whether a simple low-impact functional assessment and measurement of sarcopenia would be safe, feasible and predictive of hospital length of stay and all-cause 30-day hospital readmission. METHODS: We administered 3-minute bicycle ergometry and hand grip strength tests at admission and discharge to patients for whom a decision to admit for heart failu

Highland mountain hiking and coronary artery disease: exercise tolerance and effects on left ventricular function.

Physical exercise has become a well-established concept in the secondary prevention of coronary artery disease. We investigated the exercise requirements of extensive highland mountain hiking (8.7 km, 470 m to 1220 m over sea level, average incline 8.5%, mean walking velocity < 3 km x h-1) in 11 regularly exercising male patients with history of MI and stable coronary artery disease (CAD; mean age +/- SD:61.0 +/- 3.9 yr) and 9 age-matched male healthy controls (CO; mean age +/- SD:61.2 +/- 5.0 yr). All subjects underwent continuous ECG monitoring; arterial blood pressure and blood lactate conc

Benefits and limitations of rate adaptive pacing under laboratory and daily life conditions in patients with minute ventilation single chamber pacemakers.

Rate adaptive pacing has been shown to improve hemodynamic performance and exercise tolerance during acute testing. However, there remain concerns about its benefit in daily life and possible complications incurred by unnecessary pacing. This double-blind crossover study compared the benefit of rate adaptive (SSIR) versus fixed rate (SSI) pacing under laboratory and daily life conditions in 20 rate incompetent patients with minute ventilation single chamber pacemakers (META II). The heart rate (HR) response during three different exercise tests (treadmill, bicycle ergometry, walking test) was

Thyrotoxicosis-induced prinzmetal variant angina.

We describe a postmenopausal women with new onset of variant angina caused by thyrotoxicosis due to Graves' disease. During exercise bicycle ergometry at 50 Watts, the patient developed typical angina with ST segment elevation in the precordial leads. A coronary angiogram revealed normal coronary arteries. Graves' disease with overt hyperthyroidism was diagnosed. After achieving an euthyroid state with administration of propylthiouracil, the symptoms resolved completely and the patient had a normal exercise capacity without electrocardiographic changes. Thus, we conclude that in patients with

Diagnostic and functional exercise testing: test selection and interpretation.

Exercise stress tests are useful in many areas of medical practice and research. The results extend the clinical significance of information obtained from other sources (ie, detailed history, thorough physical examination, resting electrocardiogram, chest radiograph, and basic laboratory analyses) and serve as a diagnostic, prognostic, and therapeutic guide. Leg or arm ergometry is commonly used to assess a patient's functional status, diagnose relative myocardial ischemia, and investigate physiologic mechanisms of cardiac symptoms. The responses may also be used to determine the effects of in

Bilateral deficit in handgrip strength is not associated with rowing ergometer performances.

The bilateral deficit (BD) is the reduced force in bilateral versus unilateral contractions. This study investigated BD in handgrip strength of 13 trained rowers and its relation to rowing ergometer performance (500 m, 1000 m, 2000 m). No difference was found between bilateral (95.4 ± 13.3 kgf) and unilateral forces (95.2 ± 14.7 kgf; p = 0.902), indicating no BD (0.4 ± 4.3%). The bilateral index did not correlate with performance in 500 m (r = -0.097), 1000 m (r = -0.576), or 2000 m (r = -0.399). These findings suggest that trained rowers do not exhibit BD, and this measure is not associated w

Effects of the real-time feedback and knee taping on lower-extremity function during ergometer pedaling in subjects with tibiofemoral varus alignment.

BACKGROUND: The effect of the Posterior X Taping (PXT) used for subjects with Tibiofemoral Varus Malalignment (TFRV) aimed to control excessive tibiofemoral rotations is still unclear. Further, it is critical to use evidence-based therapeutic exercises to prevent non-contact injuries, especially in repetitive movements. OBJECTIVE: To investigate whether the PXT and real-time feedback (RTF) interventions would improve lower extremity functions during the pedaling task in subjects with TFRV. METHODS: Twenty-four male recreational athletes with TFRV participated in this study; Kinematic and mus

Setting boundaries: Utilization of time to boundary for objective evaluation of the balance error scoring system.

Subjective evaluations of balance performance, like the modified Balance Error Scoring System (mBESS), are highly popular. Alternatively, quantitative measures may offer additional clarity in identifying balance dysfunction. A novel measure to define balance impairments is time to boundary (TTB), which represents the amount of time available to make corrective postural adjustments prior to the centre of pressure (CoP) reaching the edge of the base of support. The purpose of this investigation was to assess TTB and traditional measures of CoP displacement of young adults performing the mBESS on
