Drugs containing ergotamine are commonly used in the treatment of migraine. Hypersensitivity to these drugs may be triggered off even with intake of recommended doses, inducing peripheral ischemia. Contributing factors to ergotism are concurrent fever, liver disease and drugs such as erythromycin or propranolol. We present a case history and different methods of treatment.

19 patients with severe stenoses or occlusions of the internal carotid artery showed spontaneous recanalization at follow-up. The cause of the stenotic process was spontaneous or traumatic dissection, fibromuscular dysplasia, ergotism or arteriosclerotic disease. Total recanalization occurred in carotid artery dissections coinciding with the best clinical outcome. In fibromuscular dysplasia and arteriosclerotic process, recanalization was seldom found, incomplete, and the clinical course was less favourable.

BACKGROUND: Complete withdrawal from headache medication is the treatment of choice for medication-overuse headache. Discontinuation of the overused headache medication, however, results in the development of withdrawal headache, often associated with nausea, vomiting, and sleep disturbances.
METHOD: In a prospective study of 95 patients, the authors investigated the duration and severity of withdrawal headache after overuse of various headache drugs, including single and combination analgesics, ergots, and triptans. All patients underwent standard inpatient withdrawal therapy for 14 days.

Claws and nails belong to the epidermal skin appendages, with nails developing later than claws during primate development. Both have also been depicted in fine arts. Claws can be found in illustrations of the cavemen and the devil. Nails became a subject of painting and sculptures during the Renaissance. We discuss several examples of fine arts from a dermatologic point of view, involving nail afflictions associated with ergotism, lichen planus, trachyonychia, and onychogryphosis.
