Rural-urban disparities in population practising open defecation across 47 African countries: a secondary analysis using the WHO health equity assessment toolkit.

Access to safe sanitation is a human right, yet millions in Africa practice open defecation, risking disease and environmental harm. Despite global efforts, rural-urban disparities remain, especially in low-income countries. This study examines these disparities to inform policies for equitable sanitation access. To assess rural-urban disparities in open defecation across 47 African countries using WHO and HEAT data, identifying patterns for equitable sanitation policies and SDG 6 progress. This study assessed disparities using WHO and HEAT data, focusing on open defecation prevalence, defined

Unaccompanied refugee minors' early life narratives of physical abuse from caregivers and teachers in their home countries.

The early life narratives of 34 unaccompanied refugee minors, especially their reports of interpersonal violence, were analyzed using Interpretative Phenomenological Analysis (IPA). The youth originated from eight countries, with Afghanistan, Eritrea, and Sri Lanka being the most frequent origins, and they arrived to Norway before the age of 15. Four of these youth were girls. The physical violence took place at home and/or at school and could be extremely harsh. Approximately half of the youth expressed some type of ambivalence toward the perpetrator. In analyzing how the youth understood the

Schistosomiasis mansoni in Yemen: A review.

Schistosomiasis is a major public health problem in Yemen. In 1922 Schistosomiasis mansoni was first reported to be found in Yemeni patients. In 1951 the first population survey was carried out in Taiz and revealed the presence of Biomphalaria biossyi arabica where 35% of the snails were heavily infected with S. mansoni and in San'a no snails were infected. Although S. mansoni and S. hematobium infections have been found in most parts of Yemen Arab Republic (YAR) it seems that the disease is not a public health problem in some parts of the country such as Hodeidah, Al-Beidah, Mareb, and Al-Gou

Fighting with Spirits: Migration Trauma, Acculturative Stress, and New Sibling Transition-A Clinical Case Study of an 8-Year-Old Girl with Absence Epilepsy.

In this article, we discuss the impact of migration and acculturation processes on the cultural, personal identity, and mental health of children who immigrate to a Western, multicultural environment, and the challenges clinicians in such environments face, when confronted with non-Western idioms of distress and healing practices. We do that by presenting a challenging clinical case of an 8-year-old girl who presented with very disorganized behavior, which matches a culturally accepted construct of spirit possession, in the context of migration trauma, acculturative stress, and new sibling tra

Primary Oral Tuberculosis of Tongue, Tonsil, and Labial Mucosa: A Rare Case Report.

INTRODUCTION: Tuberculosis (TB) is worldwide a leading cause of morbidity, mortality, and a major public health problem particularly in resource constrained countries. Extrapulmonary tuberculosis (EPTB) accounts for about 20% of all TB cases. Primary oral cavity TB is not only rare form of EPTB but also exists with its diagnostic challenges. To our knowledge, a small number of primary oral TB cases have been reported in literature, which is likely due to the limited diagnostic modalities and its rarity. CASE PRESENTATION: We report a 40-year-old female patient presented to Dekemhare hospital

Fitness Costs of pfhrp2 and pfhrp3 Deletions Underlying Diagnostic Evasion in Malaria Parasites.

BACKGROUND: Rapid diagnostic tests based on detection of histidine-rich proteins (HRPs) are widely used for malaria diagnosis, but parasites carrying pfhrp deletions can evade detection and are increasing in frequency in some countries. Models aim to predict conditions under which pfhrp2 and/or pfhrp3 deletions will increase, but a key parameter-the fitness cost of deletions-is unknown. METHODS: We removed pfhrp2 and/or pfhrp3 from a Malawian parasite clone using gene editing approaches) and measured fitness costs by conducting pairwise competition experiments. RESULTS: We observed significa

Shrinking the lymphatic filariasis map: update on diagnostic tools for mapping and transmission monitoring.

Lymphatic filariasis (LF), which is highly endemic in 73 countries worldwide, is targeted for elimination by 2020. The strategy for achieving this goal is based on 4 sequential programmatic steps: mapping, Mass drug administration (MDA) implementation, post-MDA surveillance and verification of LF elimination. All 4 stages of the implementation process are dependent on the availability of user friendly and highly sensitive rapid diagnostic tools. By the end of 2012, 59 countries had completed mapping for LF and Eritrea was the only country yet to start the process. Rolling out new diagnostic to

Development and deployment of a solid oral amphotericin B dosage form to treat visceral leishmaniasis within a pediatric population.

Visceral leishmaniasis (VL) is a severe and potentially fatal infection, with over 90% of reported cases occurring in East African countries including Chad, Djibouti, Eritrea, Ethiopia, Kenya, Somalia, South Sudan, Sudan, and Uganda, affecting mainly impoverished individuals, and creating a significant economic burden. Currently, the intravenous single-dose liposomal amphotericin B is the first choice for the treatment of VL. Recently, WHO and DNDi have suggested a combination of intravenous liposomal amphotericin B and oral miltefosine as a potential approach to treat VL. However, miltefosine

Trauma, Post-Traumatic Stress Disorder, and Mental Health Care of Asylum Seekers.

Asylum seekers in Israel from East Africa frequently experienced traumatic events along their journey, particularly in the Sinai Peninsula, where they were subjected to trafficking and torture. Exposure to trauma has implications for rights that are contingent on refugee status. This retrospective chart review aimed to characterize the types of traumas experienced by 219 asylum seekers (149 men) from Eritrea and Sudan who sought treatment at a specialized mental health clinic in Israel, and to compare the mental health of trauma victims (n = 168) with that of non-trauma victims (n = 53). Plasmodium falciparum is evolving to escape malaria rapid diagnostic tests in Ethiopia.

In Africa, most rapid diagnostic tests (RDTs) for falciparum malaria recognize histidine-rich protein 2 antigen. Plasmodium falciparum parasites lacking histidine-rich protein 2 (pfhrp2) and 3 (pfhrp3) genes escape detection by these RDTs, but it is not known whether these deletions confer sufficient selective advantage to drive rapid population expansion. By studying blood samples from a cohort of 12,572 participants enroled in a prospective, cross-sectional survey along Ethiopia's borders with Eritrea, Sudan and South Sudan using RDTs, PCR, an ultrasensitive bead-based immunoassay for antige

Coping with Trauma and Symptoms of Post-Traumatic Stress Disorder: Exploring Intentions and Lay Beliefs about Appropriate Strategies among Asylum-Seeking Migrants from Sub-Saharan Africa in Germany.

Asylum-seekers are at high risk of developing post-traumatic stress disorder (PTSD) due to frequent exposure to trauma. We investigated the coping intentions and lay beliefs about appropriate coping strategies among asylum-seekers from Sub-Saharan Africa in Germany. The study applied a methodological triangulation strategy with a vignette describing symptoms of PTSD. In a quantitative part, asylum-seekers (n = 119) that were predominantly from Eritrea (n = 41), Somalia (n = 36), and Cameroon (n = 25), and a native comparison sample (n = 120) responded to questionnaires assessing coping, trauma

Targeted deep amplicon sequencing of kelch 13 and cytochrome b in Plasmodium falciparum isolates from an endemic African country using the Malaria Resistance Surveillance (MaRS) protocol.

BACKGROUND: Routine molecular surveillance for imported drug-resistant malaria parasites to the USA and European Union is an important public health activity. The obtained molecular data are used to help keep chemoprophylaxis and treatment guidelines up to date for persons traveling to malaria endemic countries. Recent advances in next-generation sequencing (NGS) technologies provide a new and effective way of tracking malaria drug-resistant parasites. METHODS: As part of a technology transfer arrangement between the CDC Malaria Branch and the Istituto Superiore di Sanità (ISS), Rome, Italy,
