A novel percutaneous approach to retrieve an ingested extra-esophageal foreign body.

We report a case of an 8-year-old boy who presented to our emergency department with progressive onset of dysphagia and odynophagia after eating barbecued steak that evening. Radiographs revealed a metal bristle from a barbecue brush at the level of the proximal esophagus. The otolaryngologist attempted to retrieve this bristle using flexible esophagoscopy, but unfortunately it pushed the bristle extra-esophageal. In order to avoid major open surgery with associated morbidity, a novel percutaneous image-guided minimally invasive percutaneous approach was used to successfully retrieve the brist

A Hem-o-Lok-Induced Tracheoesophageal Fistula Cured by Temporary Airway Stenting Modified With Three-Dimensional Printing.

We present the case of a fistula between the trachea and the esophagogastric anastomosis in a 58-year-old man after esophagectomy. A Hem-o-Lok clip (Teleflex, Morrisville, NC) used at the time to ligate the azygos vein was discovered by esophagoscopy and bronchoscopy to be eroding into both structures. A covered self-expandable Y-shaped metallic tracheobronchial stent was customized with a three-dimensional printed airway model and inserted in place with endoscopic injection of fibrin glue to the anastomotic site. The stent was withdrawn 64 days after implantation, with complete healing of the

Thoracoscopic correction of persistent right aortic arch in a dog.

A 15-week-old, male intact, miniature schnauzer presented for signs consistent with persistent right aortic arch (PRAA). Esophagram and esophagoscopy confirmed this diagnosis. Following selective intubation, the constricting ligamentum arteriosum was visualized and completely resected via thoracoscopy. No complications were noted with this procedure. Advantages of thoracoscopy for management of persistent right aortic arch observed in this case were better visualization of the ligamentum arteriosum, minor postoperative discomfort, and minimal intraoperative hypothermia. Therefore, thoracoscopy

Portal hypertension as an early manifestation of carcinoma of the pancreas.

Massive hematemesis followed by 48 hours the onset of obstructive jaundice in a previously asymptomatic 38-year-old male patient. Splenomegaly was the only abnormal physical finding. Esophagoscopy visualized bleeding varices. The liver scan was normal. Dilated intra- and extrahepatic bile ducts and a possible retroperitoneal mass were demonstrated by ultrasonography. Laparotomy disclosed the presence of a carcinoma of the head and body of the pancreas that obstructed the common bile duct and the portal vein. This case report should draw attention to the vast spectrum of rare presentations of p

Ingestion of lye and other corrosive agents--a study of 86 infant and child cases.

Eighty-six cases of caustic ingestion seen over a 10 year period at the Montreal Children's Hospital are presented in an analytical retrospective fashion. All the patients were hospitalized. Ingestion was more common in males than females. Sixty-two patients were less than four years of age. Thirty-five of the 86 cases had esophageal burns. Esophagoscopy was performed on all cases. There was no perforation following endoscopy. Steroids and antibiotics were used on all the cases. Six patients developed stricture formation requiring bougienage.

[Primary malignant melanoma of the esophagus].

We report the case of primary malignant melanoma of oesophagus in a 63-year-old man who presented with a 8-week history of dysphagia. Esophagoscopy demonstrated a polypoid mass expanding in the mid-oesophagus without causing obstruction. Based on histological and immunohistochemical studies, the diagnosis of primary esophageal malignant melanoma was made. Radical resection was not possible because of regional tumour extension at the time of diagnosis. The patient underwent percutaneous endoscopic gastrostomy. Death occurred 4 months later because of acute respiratory failure.

Endoscopic obliteration of a recurrent tracheoesophageal fistula with enbucrilate and polidocanol in a child.

We present a difficult case of a recurrent tracheoesophageal fistula following primary surgical repair of esophageal atresia. After four unsuccessful attempts to close the fistula, which included three thoracotomies and one endoscopic obliteration using fibrin glue, successful recurrent tracheoesophageal fistula closure was attained with the endoscopic application of enbucrilate (Histoacrylate) combined with polidocanol. Enbucrilate was applied into the lumen of the fistula by bronchoscopy, and polidocanol (Sclerovein) was injected into the esophageal submucosa around the fistula by esophagosc

Gastroesophageal reflux disease. When antacids aren't enough.

Gastroesophageal reflux disease is a very common condition that is usually manifested by heartburn or regurgitation. Reflux esophagitis, caused by mucosal exposure to the backflow of caustic gastric contents, is primarily a result of lower esophageal sphincter dysfunction. Diagnostic workup varies but commonly includes esophagoscopy, 24-hour esophageal pH monitoring, and radiography of the upper gastrointestinal tract. Treatment, which progresses from simple life-style changes and antacids to histamine2 receptor antagonists to omeprazole (Prilosec) or surgery, is tailored to individual needs a

[Palliative treatment of cancer of the esophagus using an endoluminal prosthesis].

Between 1973 and 1984, 27 patients with a cancer of the esophagus or the cardia and suffering dysphagia underwent palliation with an endoluminal prosthesis. Intubation under esophagoscopy or through a gastrotomy, or both, allowed the placement of 23 Celestin tubes and 4 Mousseau-Barbin prostheses. The early postoperative death rate was 11%. The overall morbidity was 37%. It included such complications as tube displacement (18.5% of patients), tracheal compression by the tube (7.4%), obstruction of the prosthesis (11%), esophageal fistula (7.4%), aspiration (7.4%) and wound infection (7.4%). Gastroesophageal reflux and severe mental retardation.

Recurrent vomiting is common in children with severe mental retardation and leads to significant morbidity with malnutrition, anemia, and aspiration pneumonitis. Spasms of the abdominal muscles and diaphragm, uncoordinated peristalsis, and central nervous system disorders are causes of dysphagia and continuous gastroesophageal reflux. It is desirable that mentally retarded children with vomiting have a barium swallow and esophagoscopy as early as possible. Fundoplication should be performed before complications develop. Spasms with aspiration followed by apnea, in particular, are life-threaten

Fiberoptic endoscopic diagnosis and treatment of a congenital esophageal diaphragm.

A 9-month-old infant suffering from dysphagia to solids presented with esophageal bolus obstruction. Using an adult fiberoptic esophagoscope, a congenital web was found at the junction of the middle and distal thirds of the esophagus. Previous direct rigid esophagoscopy and roentgenologic examination had not visualized the web. The web was successfully ruptured and dilated by inserting a guide wire under fiberscopic control and then inserting metal olives, followed later by blind mercury bougienage. This report illustrates the use of adult fiberoptic instruments for diagnosis and treatment in

A case of antenatally undiagnosed laryngeal atresia with tracheo-esophageal fistula with airway obstruction at birth and its management.

Laryngeal atresia is a rare congenital anomaly that is usually diagnosed by antenatal ultrasound, however, if undiagnosed presents with desaturation after birth. A term neonate presented with airway obstruction after birth with multiple failed attempts at intubation and was rescued by proseal laryngeal mask airway (LMA). An esophagoscopy using an Ambuscope utilizing a modified connector assembly revealed an opening on the anterior wall of the esophagus with no esophageal atresia, leading to a diagnosis of H-type tracheo-esophageal fistula (TEF) with laryngeal atresia. The ability to ventilate

Upper Esophageal Sphincter Dilation for Recalcitrant Dysphagia Secondary to Dermatomyositis.

Dermatomyositis is a rare multisystem autoimmune disorder occasionally accompanied by dysphagia. It is typically treated with immune modulating agents; however, dysphagia is often unresponsive to these. Previous reports have demonstrated the utility of videoflouroscopy and manometry in understanding the etiologies of dysphagia to inform a procedural target, historically the cricopharyngeus muscle. We present a case of dermatomyositis and dysphagia resistant to medical management in a patient found by videoflouroscopy and manometry to have severe oropharyngeal dysphagia, esophageal dysmotility

A hamartoma presenting as an intramural upper oesophageal tumour.

Oesophageal hamartomas are extremely rare conditions especially in upper oesophagus. We report on a 20-year-old woman who presented with dysphagia and was diagnosed with a retrosternal 4.9 cm × 9.0 cm heterogenic tumour located in her upper oesophagus. Preoperative examinations included computed tomography of the chest, bronchoscopy and esophagoscopy, and no signs of malignancy were noted. She underwent surgical resection of the mass and the final histopathological diagnosis was osteochondromatous hamartoma of the upper oesophagus. No acute or long-term complications or tumour recurrence were

Nonsurgical treatment of gastroesophageal intussusception in a puppy.

A 7-week-old, female Siberian husky was presented to Murdoch University Veterinary Hospital with an acute onset of respiratory distress and regurgitation. Thoracic imaging identified an intraluminal esophageal mass with concurrent aspiration pneumonia. Esophagoscopy identified the mass as stomach, and a diagnosis of gastroesophageal intussusception was made. The intussusception was reduced endoscopically, and fixation of the stomach to the abdominal wall was performed using a tube gastropexy. Gastroesophageal intussusception is an uncommon disease in small animals and traditionally has been ma

Button battery ingestion: A serious pitfall of diagnosis of ingested foreign bodies in children from resource-limited settings - A case report.

INTRODUCTION AND IMPORTANCE: Button battery ingestion and impaction in the oesophagus can result in severe morbidity and even fatality if not diagnosed and managed urgently. Delayed or missed diagnosis due to the sometimes-vague symptomatology and limited investigations in resource-limited settings further increases the complications rate. CASE PRESENTATION: Case 1: A 2 years old male child presented with a nine months history of recurrent respiratory tract infections not responding well to medical treatment. A radio-opaque round foreign body (FB) was incidentally seen in the chest. Rigid eso

Panendoscopy for second primaries in head and neck cancer.

Four hundred patients with biopsy-proven squamous cell carcinoma of the upper aerodigestive tract were studied for evidence of a synchronous second primary tumor. The protocol of study included in most cases direct laryngoscopy, metal tube bronchoscopy, and esophagoscopy. Chest x-ray studies were performed on all patients and many had barium swallow x-ray studies. Asymptomatic second head and neck primary tumors were found only in the vallecula and esophagus among this series of patients. Metachronous second primary tumors were presented as early as one month after the original endoscopy and w

Successful treatment of post chemotherapy esophageal cicatricial atresia in a pediatric patient with anaplastic large cell lymphoma through minimally invasive esophagectomy: a case report.

BACKGROUND: Anaplastic large cell lymphoma (ALCL) is a CD30-positive T-cell lymphoma, which is a rare type of non-Hodgkin lymphoma. ALCL rarely presents in the gastrointestinal tract, and the esophageal involvement in of ALCL is extremely rare. CASE PRESENTATION: An 11-year-old boy who complained of abdominal pain and cough was diagnosed with ALK-positive ALCL on the basis of systemic lymphadenopathy findings and immunohistochemistry results of pleural effusion. Although remission was observed after chemotherapy at 5 months after diagnosis, dysphagia persisted, and esophagoscopy revealed a se

Fish Bone Foreign Body: The Role of Imaging.

Introduction  Fish bone foreign body (FFB) impaction in the upper aerodigestive tract is a common cause for emergency department referral. Its management varies in both diagnosis and treatment paradigms. Fish bone foreign bodies are more commonly found in the oropharynx in cases of patients < 40 years old, and in the esophagus in cases of patients > 40 years old. Symptoms are typically non-indicative for the location of the FFB, with the exception of foreign body sensation at/superior to the cervical esophagus. A lack of findings during the physical examination is routinely followed by imaging
