Clinical and Ethical Approaches to Medical Futility in End-of-Life Care: A Scoping Review.

Medical futility (MF) remains a complex and debated concept in clinical practice, particularly among patients with terminal illness or limited life expectancy, largely due to inconsistencies in its definition and variability in its application, which complicate clinical decision-making. A scoping review was conducted following the methodologies proposed by Arksey, Levac, the Joanna Briggs Institute, and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines to analyze evidence on therapeutic futility in terminally ill adults

Artificial intelligence in pediatric diabetes: clinical benefits, developmental risks, and ethical challenges.

UNLABELLED: Artificial intelligence (AI) is rapidly transforming both pediatric healthcare and everyday childhood experiences. Children with diabetes represent a unique population at the intersection of medical AI, including automated insulin delivery (AID) systems, clinical decision support tools, and predictive glucose monitoring, and consumer AI, such as chatbots and AI-based educational platforms. This paper examines the benefits, risks, and unresolved dilemmas associated with this dual AI exposure. This narrative review addresses medical and consumer AI applications in pediatric diabetes.

Risk models for sudden cardiac death in cardiomyopathies: clinical, methodological and ethical challenges.

Sudden cardiac death (SCD) remains a devastating but potentially preventable outcome in inherited cardiomyopathies. Although its absolute incidence is low, the possibility of preventing SCD through implantable cardioverter-defibrillators renders accurate arrhythmic risk stratification a central clinical priority. Risk prediction is evolving from phenotype-based stratification towards individualized approaches integrating genotype, imaging biomarkers, and clinical variables. This review critically evaluates available SCD risk prediction models across cardiomyopathy subtypes, including well-vali

The Challenges of Compassion Bias in Clinical Decision-Making: A Case-Based Ethical Analysis.

Bias in clinical decision-making is commonly understood as arising from negative stereotypes or cognitive shortcuts that undermine patient-centered care. Less attention has been paid to forms of bias that emerge from clinicians' well-intentioned efforts to alleviate suffering. This article introduces the concept of compassion bias, defined as a distortion in clinical judgment that occurs when empathy and a desire to avoid harm unintentionally constrain objectivity, patient autonomy, or the range of treatment options offered. We present the case of a young woman with advanced neurologic decline

Geriatric palliative care: clinical imperatives, ethical challenges and public health opportunities.

It is estimated that Angelman syndrome (AS) accounts for up to 6% of all children presenting with severe mental retardation and epilepsy. The main clinical features of AS may not be apparent early in life. Clinical findings present in all patients include developmental delay, which becomes apparent by 6-12 months of age, severely impaired expressive language, ataxic gait, tremulousness of limbs, and a typical behavioral profile, including a happy demeanor, hypermotoric behavior, and low attention span. Seizures, abnormal electroencephalography, microcephaly, and scoliosis are observed in >80%

Where is the exit? The ethical importance of exit plans in clinical trials with neural implants.

BACKGROUND: As clinical trials involving implantable neural devices (INDs) increase in frequency and attract greater public attention, it is paramount to ensure they are conducted in alignment with fundamental ethical guidelines. Particular focus must be placed on the often underexplored aspect of trial termination for INDs. OBJECTIVE: To systematically review the ethical challenges encountered in clinical trials for INDs at the juncture of trial termination. METHODS: We conducted a rapid review using PubMed with two specific search queries, including all publications addressing ethical issu

The ethical inadequacy of uninformed surrogate consent: advancing respect for persons in clinical research.

In clinical research, decision-making capacity is often equated with unspecified conceptions of autonomy, and autonomy is often equated with personhood. On this view, the loss of decision-making capacity is seen as a loss of autonomy, and the loss of autonomy subsumes a loss of personhood. An ethical concern arises at the intersection of those philosophical considerations with the legal considerations in informed consent. Because persons with inadequate decision-making capacity cannot provide legally effective consent, enrollment in research can occur only if a surrogate gives permission on th

Exposure to Noise or Music in Clinical Trials: A Scoping Review on Ethical and Methodological Considerations.

BACKGROUND: Use of noise or music in experimental human studies requires balancing the need to avoid subjecting participants to potentially harmful noise levels while still reaching levels that will produce a measurable change in the primary outcome. Several methodological and ethical aspects must be considered. This study aims to summarize ethical and methodological aspects, and reported outcomes, of previously published experimental paradigms using loud noise/music. METHODS AND MATERIALS: Four databases (Medline, Central, Web of Science, and Scopus) and two trials registries (Clinicaltrials

Instrument fracture in periodontal therapy: ethical disclosure and clinical management - a case report.

Periodontal instrument fractures are rare events in dentistry, with limited literature available on their occurrence and management. This case report highlights an incident involving the fracture of a periodontal sickle scaler blade during manual instrumentation for the removal of calculus. The fracture occurred during instrumentation on the mesial surface of the maxillary right second molar, and the separated blade was subsequently pushed into the sulcus. A radiographic assessment was performed to verify the precise location of the fractured segment. Following confirmation, the broken blade w

Ethical and legal considerations in non interventional health clinical trials in the French context.

While the development of regulations in the conduct of research in humans, has better allowed risks and associated contraints to be framed, it has also raised further questions. French regulations currently consider that research based on questionnaires or interviews has no impact on the care of patients or on the individual, a view that is too limited and is not shared by the ethics committees charged with the protection of those involved in studies. Any research relating to a person requires his or her active involvement. The intention of the researcher can be perceived by the participant an

Revisiting selected ethical aspects of current clinical in vitro fertilization (IVF) practice.

Ethical considerations are central to all medicine though, likely, nowhere more essential than in the practice of reproductive endocrinology and infertility. Through in vitro fertilization (IVF), this is the only field in medicine involved in creating human life. IVF has, indeed, so far led to close to 10 million births worldwide. Yet, relating to substantial changes in clinical practice of IVF, the medical literature has remained surprisingly quiet over the last two decades. Major changes especially since 2010, however, call for an updated commentary. Three key changes deserve special notice:

Guidelines for ethical behavior relating to clinical practice issues in neuromuscular and electrodiagnostic medicine.

The American Association of Neuromuscular & Electrodiagnostic Medicine (AANEM) developed guidelines to formalize the ethical standards that neuromuscular and electrodiagnostic (EDX) physicians should observe in their clinical and scientific activities. Neuromuscular and EDX medicine is a subspecialty of medicine that focuses on evaluation, diagnosis, and comprehensive medical management, including rehabilitation of individuals with neuromuscular disorders. Physicians working in this subspecialty focus on disorders of the motor unit, including muscle, neuromuscular junction, axon, plexus, nerve

[Treatment against the patient's will exemplified by electroconvulsive therapy : Clinical, legal and ethical aspects].

BACKGROUND: Severe affective and psychotic disorders may be accompanied by legal incapacity. If in this case the patient refuses treatment and in parallel there is a risk of serious damage to health, treatment can be carried out against the patient's non-autonomous will under defined prerequisites. Due to its good and partly superior effectiveness in the treatment of severe and pharmacotherapy-resistant affective and psychotic disorders, electroconvulsive therapy (ECT) is an important treatment option in such constellations. AIM: Description of the general principles and prerequisites of ther

Ethical considerations in the design, execution, and analysis of clinical trials of chronic pain treatments.

INTRODUCTION: In the field of pain research, clinical trials may randomize over 500 subjects and include more than 150 sites spanning over a dozen countries. METHODS: This review examines the ethical considerations affecting clinical trial design, execution, and analysis of trials for chronic pain. The Belmont Report has been the touchstone for human studies protection efforts since 1979. Commissioned by the U.S. government in response to ethical failures in medical research, such as the Tuskegee Syphilis Study, the report emphasizes 3 basic principles: respect for persons, beneficence, and j

Acknowledgment of uncertainty: a fundamental means to ensure scientific and ethical validity in clinical research.

Recognition of the importance of uncertainty in the design of randomized, controlled trials (RCT) has reached the status of a principle. The "uncertainty principle," or less ambiguously, equipoise, holds that a patient should be enrolled in an RCT only if there is substantial uncertainty about which of the trial treatments would benefit the patient most. In fact, the "uncertainty principle" addresses the most important issue of a clinical trial--the choice of an adequate comparative control. Studies in which intervention and control group are believed to be non-equivalent violate the uncertain
