Arthrodesis is a well-recognized salvage procedure in patients with septic destruction of knee joints. It offers the opportunity for restoring weight bearing capability and significant pain relief as well as eradication of infection, but at the expense of joint motion. 26 children with 29 lower limb fractures, between the age of 2 and 15 years, were treated with an external fixator. The average time to union and removal of the fixator was 71 days for femoral and 73 days for tibial fractures. Fourteen patients with Ruedi/Allgower types II and III distal tibia fractures received indirect reduction followed by application of a Monticelli-Spinelli hybrid external fixation system. At our institution, the small AO external fixator has been utilized on the wrist and hand on over 200 patients since 1979. Disregarding fractures of the distal radius, the main indications were fracture dislocations of the wrist, Kienböck's disease, combined injuries with bone defects of the wrist and metacarpals, and basilar fractures of the thumb metacarpal. Small external fixators have been developed for use in the hand and wrist. The main indications are stabilization of open or infected fracture or non-union of the metacarpals and phalanges; length maintenance in segmental bone loss; and distraction-lengthening of the fingers. External fixation in fractures of the distal radius has been used for almost 80 years. The main objective is to achieve reduction and maintain the reduction throughout treatment. The fixator concept described as the dynamic fixator allows reduction in three planes and allows for the wrist to move after a period of rigid fixation. In a prospective, controlled study 58 patients aged under 60 years with Colles' fractures were treated either by a forearm plaster or by the application of an external fixator. In 94% of those treated by a fixator it was possible to insert the distal pins of the frame into the fracture fragment, the fixation obtained being sufficient to forgo additional splintage. The external fixator proved more effective at holding the manipulated position, and the radiological loss of position during fracture union was minimal compared with that seen in patients treated in plaster.

Thirty-three consecutive patients with complete ruptures of the calcaneal tendon were treated by external fixation. The patients were assessed both objectively and subjectively, and the results classified as excellent, good, fair and poor. In 30 patients the result was excellent or good. There were no infections or re-ruptures. Two patients with fair results had sural nerve injury. The only patient with a poor result had Sudeck's atrophy. It is felt that this operation satisfies the need for a new technique which is simple and combines the advantages of both surgical and non-surgical treatment

From 1977 to 1980 fixation with an external fixateur was performed in 33 injuries of the upper extremity: ten fractures of the hand, 22 Colles' fractures, and one osteotomy for Madelung's deformity.
