Rare Orbital Metastasis of Carcinoid Tumor Despite Long-Term Somatostatin Therapy: A Case Report.

Background and Clinical Significance: Carcinoid tumors are rare, slow-growing neuroendocrine cell neoplasms that typically affect the gastrointestinal tract. While metastasis may occur, it most commonly occurs in the liver, and orbital metastasis is extremely rare, especially while on systemic somatostatin suppression. Case Presentation: A 57-year-old man with a history of gastrointestinal carcinoid tumor treated with lanreotide for 5 years presented with a left proptotic, red eye and double vision for several months. Clinical examination revealed left proptosis, supraduction deficit, lower li

Corneal papilloma associated with papillomavirus in a one-humped camel (Camelus dromedarius).

A 15-year-old male dromedary camel with a history of chronic severe keratoconjunctivitis and corneal mass in the left eye of 6 months' duration was referred to the Veterinary Medical Teaching Hospital at Adnan Menderes University. A superficial keratectomy was performed and biopsy material submitted for histopathology. The diagnosis was corneal papilloma. There has been no recurrence of the neoplasm to date (6 months, 1 year). Corneal papilloma has not been reported previously in camels and seems to be associated with papillomavirus.

Clinical and histopathological findings of a rare sinonasal glomangiopericytoma.

Glomangiopericytoma is a rare vascular neoplasm of the nasal cavity and paranasal sinuses that occurs during the sixth or seventh decade of life. It is categorized as a borderline tumor with low malignant potential and classified as a distinct entity of sinonasal tumors with perivascular myoid phenotype by the World Health Organization (WHO). We report the case of a 50-year-old woman with nasal obstruction and severe epistaxis. The nasal sinuses computed tomography (CT), and magnetic resonance imaging (MRI) demonstrated a 3.1 cm soft tissue mass occupying the upper part of the left nasal cavit

Atypical presentation of cutaneous meningioma.

Cutaneous meningiomas are rare tumors of ectopic meningeal tissue in the dermis or subcutis and are most commonly located on the scalp and around the eyes, ears, nose, and mouth. We present a case of cutaneous meningioma with an unusual presentation on the upper arm, but with positivity for epithelial membrane antigen to support the diagnosis of meningioma. Lack of pancytokeratin AE1/3 and p63 allowed us to rule out a keratinocytic tumor, such as squamous cell carcinoma and basal cell carcinoma. The absence of smooth muscle actin, FXIIIa, CD163, and CD34 allowed us to rule out a leiomyoma, der

Choroidal Hemangioma Associated with Hepatic Hemangioma: A Case Report and Literature Review.

Circumscribed choroidal hemangioma (CCH) is a rare congenital ocular tumor type that is usually benign and asymptomatic. CCH has only been reported once previously in the literature in association with a visceral neoplasm. Here, we present a newly described association between CCH and hepatic hemangioma (HH). We report the case of a 57-year-old woman diagnosed with asymptomatic HHs who presented with a 2- to 3-month history of central scotoma and blurred vision in her left eye. Assessment identified an orange-red elevated lesion with a central serous retinal detachment and subretinal edema. Co

Peripheral primitive neuroectodermal tumor in a two-year-old paint horse.

A 2-year-old gelding presented with a history of lethargy and anorexia. Physical examination revealed pleural and abdominal fluid, as well as several masses in the scrotum. The horse became acutely dyspneic despite 7 days of supportive care. Because of the poor prognosis, the owners elected euthanasia. Gross necropsy findings included multiple masses in the scrotum and inguinal canals and along the dorsal peritoneal cavity. The neoplasm infiltrated the kidneys, liver, spleen, mesenteric lymph nodes, mesentery, and abdominal surface of the diaphragm. Histologically, the neoplasm is composed of

The diagnostic dilemma of primary central nervous system melanoma.

Melanomas are malignant neoplasms of melanocytes developing predominantly in the skin, but occasionally arising from eyes, mucous membranes, and the central nervous system (CNS). The CNS can be affected by a spectrum of melanocytic lesions ranging from diffuse neurocutaneous melanosis, to a focal and benign neoplasm (melanocytoma), and to an overtly malignant tumor (melanoma). Primary melanocytic lesions involving the CNS are typically concentrated in the perimedullary and high cervical region. Primary CNS melanoma cannot be reliably distinguished from metastatic melanoma on neuroimaging and h

Transient Visual Obscurations and Systemic Symptoms Unveiling Glial Fibrillary Acidic Protein Astrocytopathy.

INTRODUCTION: Glial fibrillary acidic protein (GFAP) astrocytopathy is a rare autoimmune central nervous system disorder defined by GFAP-IgG antibodies in cerebrospinal fluid (CSF). Optic disk edema is reported in up to half of cases, but detailed ophthalmic descriptions remain limited. CASE PRESENTATION: A 68-year-old man presented with transient visual obscurations in his right eye, triggered by postural changes. His history included fatigue, weight loss, tremor, vertigo, and progressive cognitive decline, for which he had already been evaluated by multiple specialists. Ophthalmologic exami

Isolated intraorbital schwannoma arising from the abducens nerve.

CASE REPORT: A case of isolated schwannoma of the orbit, arising from the terminal branches of the abducens nerve to the lateral rectus muscle, is reported. The patient presented with a painless proptosis of the left eye. DISCUSSION: Preoperative diagnosis of benign intraorbital neoplasm was made by means of CT and MR scans; the mass was radically excised through a microsurgical lateral orbitotomy and the pathological examination revealed a schwannoma. Features of orbital schwannoma are described, together with some details concerning the surgical strategy and the history of the evolution of

Surface microscopy. A new approach to the diagnosis of cutaneous pigmented tumors.

Clinical guidelines for the diagnosis of pigmented lesions of the skin are not always reliable. Surface microscopy (SM) represents an interesting approach to this problem. For this in vivo investigation, a stereomicroscope, a glass slide, and immersion oil are used. In the present study, the various criteria of SM that cannot be discerned by the naked eye are correlated with the histopathological structures in step-sectioned specimens. Characteristic SM features such as the pigment network, "black dots," or "irregular extensions" were found to correspond to particular histological findings. SM

Epiphora and Hyperlacrimation as Paradoxical Manifestations of Facial Nerve Injury: Mechanistic Insights.

The incidence of facial nerve paralysis is approximately 30 per 100,000 persons annually. Although it is often idiopathic, as in Bell's palsy, it can also result from infections, trauma, or neoplasms. Facial nerve paralysis may present with partial or total facial paresis, lagophthalmos, denervation of the lacrimal gland, and other ocular abnormalities. While dry eye is a commonly expected outcome of facial nerve injury, some patients may paradoxically experience epiphora and hyperlacrimation. In this review, we examine this phenomenon and its mechanisms in facial nerve injury. Several mechani

The diagnosis and management of ocular lymphoma.

PURPOSE: Lymphoma is the most common malignancy of the ocular adnexa. Most of the ocular adnexa lymphomas are non-Hodgkin B-cell lymphomas. The most common type of ocular adnexa lymphoma is primary extranodal marginal zone B-cell lymphoma of the MALT (mucosa-associated lymphoid tissue). Most of these neoplasms are primary extranodal lymphomas, although 10% to 32% are secondary tumors from disseminated disease. CASE REPORT: A 58-year-old woman presented for a comprehensive examination, with the chief complaint of ocular discomfort in both eyes. Anterior segment examination revealed bilateral s

Optic nerve glioma manifesting as intratumoral hemorrhage in a pregnant woman--case report.

A 33-year-old pregnant woman presented with intratumoral hemorrhage in an astrocytoma of the left optic nerve and chiasm manifesting as poor vision in her left eye. Visual examination found no light perception in the left eye and deteriorated acuity with temporal hemianopsia in the right eye. Computed tomography demonstrated a round high-density mass in the suprasellar region. T2-weighted magnetic resonance (MR) imaging showed the center of the mass as mildly hypointense and T2-weighted MR imaging as hypointense. Both T1- and T2-weighted MR imaging showed the rim of the mass as hyperintense. S

Role of Imaging in a Case of Toxoplasmosis Presenting as Generalized Lymphadenopathy.

Toxoplasmosis is caused by Toxoplasma gondii an obligate protozoan intracellular parasite. The disease has variable prevalence globally and is usually asymptomatic. Pregnant and immunocompromised people are at risk of getting infected. Enlarged lymph nodes are the most frequently observed clinical form of Toxoplasma in humans, mostly affecting posterior cervical nodes. Other organs usually affected are the brain and eyes. We present a case of toxoplasmosis with generalized lymphadenopathy mimicking metastasis in a lady with a previous history of operated pancreatic neoplasm.

Visual loss following treatment of sphenoid sinus carcinoma.

A 71-year-old woman developed complete third nerve palsy and total blindness of the right eye one month after completing a course of radiotherapy for sphenoid sinus carcinoma over a 13-month period. Differential diagnosis included recurrence of the tumor, radiation-induced second neoplasm, empty sella with chiasmal prolapse and secondary chiasmal arachnoid adhesions, and radionecrosis. Magnetic resonance imaging demonstrated gadolinium contrast enhancement of the right intracranial optic nerve and chiasm, suggesting a radionecrosis process.

Metastatic ciliary body carcinoid tumor.

A 50-year-old man with a history of systemic carcinoid had decreased right eye vision and a darkly pigmented cilio-choroidal mass. Results of ultrasonographic and fluorescein angiographic examinations were consistent with a malignant melanoma. Fine-needle aspiration biopsy of the lesion identified it as a carcinoid metastasis. The patient refused both radiation and enucleation; complete excision with a cyclochoroidectomy was performed. Histopathologic examination showed a carcinoid neoplasm covered by marked hyperplasia of the retinal pigment epithelium that produced the pigmented appearance o

Gamma Knife surgery for abducent nerve schwannoma. Report of 4 cases.

Abducent nerve schwannomas are extremely rare. The authors recently performed Gamma Knife surgery (GKS) in 4 patients with such tumors and describe their experiences with these cases. The patients consisted of 3 women and 1 man whose ages varied from 31 to 60 years (mean 46 years). Two patients had no symptoms, 1 complained of slight visual disturbances, and the other 1 had abducent nerve palsy. Neurofibromatosis was not diagnosed in any case. All 4 tumors were located in the cavernous sinus: 2 of these tumors within the borders of the sinus, 1 tumor extending into the orbit, and 1 tumor exten

A rare case of precursor B-cell lymphoblastic lymphoma of the orbit in a child.

A 5-year-old boy presented with gradually progressive painful proptosis of the left eye for 5 months. He had an incidental testicular swelling for which he underwent high inguinal orchidectomy and histopathology showed atypical lymphoid cells. His vision was 6/6, N6 in the right eye, and no perception of light in the left eye. Left eye examination revealed an inferior dystopia and total ophthalmoplegia with corneal exposure changes. A computerized tomography scan showed an isodense mass molding around the globe. An incision biopsy from the subconjunctival mass showed atypical large lymphocytes

Non-CMV infectious chorioretinopathies in AIDS.

Cytomegalovirus (CMV) retinitis is the most common posterior segment opportunistic infection and the leading cause of blindness in acquired immunodeficiency syndrome (AIDS) patients. CMV is not the sole agent that can infect the fundus of an immunocompromised patient. Disseminated herpes zoster (HZ), herpes simplex, toxoplasmosis, and Candidiasis are possible. Syphilis, Pneumocystis carinii, cryptococcosis, tuberculosis, and a host of other viruses, protozoa, bacteria, or neoplasms may invade the retina or choroid. The eye care practitioner must not only differentiate noninfectious retinopathy
