Facial surface changes after cleft alveolar bone grafting.

PURPOSE: The aim of this study was to assess the 3-dimensional facial surface changes after cleft alveolar bone grafting with digital surface photogrammetry. PATIENTS AND METHODS: In a prospective study, 22 patients with cleft lip and palate underwent alveolar bone grafting. Before the procedure and 6 weeks postoperatively and before the continuation of orthodontic treatment, 3-dimensional images were taken with digital surface photogrammetry. Seven standard craniofacial landmarks on the nose and the upper lip were identified. Their spatial change because of bone grafting was assessed. Statis

[Progressive bone elongation of the maxillo-facial area: mandibular distraction].

Thanks to the distraction osteogenesis technique, it is nowadays possible to create new bone in the facial area. Between january 1997 and march 1999 we have performed 20 such procedures, from which 15 were mandibular. We present our experience in 10 patients with this new technique, 5 unilateral and 5 bilateral. Those were 7 boys and 3 girls, aged 2 to 14 years, affected with hemifacial microsomia, Goldenhar syndrome: 3; retrognatism with severe malocclusion: 4; facial assimetry due temporomandibular joint abnormalities: 2; and facial assimetry: 1. The proposed elongation was achieved in all c

[Sex hormones in fibrous dysplasia of the facial bone--an immunohistological study].

Fibrous dysplasia of the facial bones frequently occurs in the maxilla around the time of puberty and becomes inactive when skeletal growth is completed. A case of fibrous dysplasia of the maxilla and sphenoidal-temporal bone in a 19 year-old male was studied, in terms of the presence of estradiol, testosterone and dihydrotestosterone, using the peroxidase-antiperoxidase method. This case underwent partial surgical resection, for cosmetic deformity, of the maxillary tumor which, however, relapsed to preoperative size within three weeks. Estradiol and testosterone were strongly positive and dih

Transgenic mouse model for neurocristopathy: Schwannomas and facial bone tumors.

We have characterized a strain of double transgenic mice with simian virus 40 large tumor antigen and prokaryotic lacZ under the control of the myelin basic protein promoter that develops spindle-cell sarcomas and osteogenic sarcomas at 5-7 months of age. Although poorly differentiated, the spindle-cell sarcomas were characterized as malignant Schwannomas based on their neural association, the presence of basal lamina, and expression of Schwann cell-specific genes. The osteogenic sarcomas were often multiple and appeared predominantly in the facial bones, less frequently in the ribs and verteb

[Surgical treatment of facial nerve paralysis in fractures of the temporal bone].

Of 36 patients with facial nerve palsy caused by the fracture of the temporal bone, 28 were treated surgically and 8 conservatively. The way of treatment was determined after the detailed radiologic, neurootologic, vestibular and functional hearing examinations as well as of the damaged facial nerve. Numerous neurootologic disorders have the decisive clinical value in evaluation of the site and degree of destruction and selection of adequate way of treatment. The authors have shown that early surgical intervention and combined transmastoid and translabyrinthine approach provide the most suitab

Histopathology of the facial nerve after longitudinal temporal bone fracture.

The temporal bone histopathology in a patient who suffered repeated head trauma and a longitudinal temporal bone fracture shows unusual features associated with the facial nerve. In the distal meatal segment, a type of traumatic neuroma was found with disorganized nerve bundles and distinct areas of Schwann's cell proliferation but lacking fibrosis. Periosteal new bone formation in the labyrinthine segment narrows the fallopian canal and protrudes into the nerve, which completely fills the canal. The tympanic and mastoid segments of the nerve show severe degeneration of nerve fibers and an inc

Electrophysiologic monitoring of facial nerve during temporal bone surgery.

Monitoring of peripheral nerves during surgical procedures decreases trauma to the nerve, which may preserve function. During 1984, electrophysiologic monitoring of the facial nerve was used during 32 operative procedures in and around the temporal bone. The largest group were for cerebellopontine angle tumors. Clinically, monitoring appeared to be helpful by providing objective evidence of trauma to the facial nerve and a method of demonstrating integrity of the nerve. However, review of this series did not reveal objective evidence of improved facial nerve preservation compared to results in

Bone-Anchored Implant Survival in Oncologic Facial Prosthetic Rehabilitation.

BACKGROUND: Published survival rates for bone-anchored craniofacial implants exceed 85%, but derive largely from mixed populations. Whether these apply to head and neck oncologic surgery remains uncertain. METHODS: Retrospective survival analysis of implants placed for prosthetic rehabilitation following extirpative surgery at a UK center (2005-2024). RESULTS: Survival among 184 implants in 51 patients was 94%, 60% and 46% at 1, 3 and 5 years (median 4.1 years, 95% CI 3.0-6.3). 77% of patients received radiation therapy. Median dose to implant region was 57 Gy. On univariate analysis, radiat

Soft-tissue complications after facial feminization bone surgery.

The soft tissues have a considerable impact on whether the results of facial feminization surgery are favorable. Complications or suboptimal results related to the soft tissue may be due to poor choice of surgical approach, improper execution of the surgical technique, a lack of assistance when resuspending the soft tissues during closure, or deficient readaptation of the overlying soft tissue to the new bone contour. This article identifies the possible poor soft-tissue outcomes that may occur after facial feminization bone surgery, describing strategies to prevent them and treatment alternat

Intermittent hypoxia in neonatal rodents affects facial bone growth.

Preterm human infants often show periodic breathing (PB) or apnea of prematurity (AOP), breathing patterns which are accompanied by intermittent hypoxia (IH). We examined cause-effect relationships between transient IH and reduced facial bone growth using a rat model. Neonatal pups from 14 timed pregnant Sprague-Dawley rats were randomly assigned to an IH condition, with oxygen altering between 10% and 21% every 4 min for 1 h immediately after birth, or to a litter-matched control group. The IH pups were compared with their age- and sex-matched control groups in body weight (WT), size of facia

[The mechanical properties of the bone tissue from the middle facial region].

The objective of the present work was to study age-related features of the reaction of the maxillary sinus cavity walls to a variety of impacts associated with the surgical interventions. To this effect, a variety of stress-strain conditions were simulated that are known to arise in the maxillary sinus cavity walls under the action of different force vectors taking into consideration the age-specific peculiarities of the mechanical structure of the bone tissue. The 3D model of the maxillary sinus cavity was built up based on the results of spiral computed tomography with the use of the Solid W

Facial paralysis caused by metastasis of breast carcinoma to the temporal bone.

Metastatic tumors to the temporal bone are very rare. The most common sites of origin of temporal bone metastases are breast, lung, kidney, gastrointestinal tract, larynx, prostate gland, and thyroid gland. The pathogenesis of spread to the temporal bone is most commonly by the hematogenous route. The common otologic symptoms that manifest with facial nerve paralysis are often thought to be due to a mastoid infection. Here is a report on a case of breast carcinoma presenting with otalgia, otorrhea, and facial paralysis for 2 months. The patient was initially diagnosed as mastoiditis, and later
