Nervus intermedius (NI) arises from the superior salivary nucleus, solitary nucleus, and trigeminal tract. It leaves the pons as 1 to 5 roots and travels between the facial and vestibulocochlear nerves before merging with the facial nerve within the internal auditory canal. The mastoid segment of the facial nerve then gives rise to a sensory branch that supplies the posteroinferior wall of the external auditory meatus and inferior pina. This complex pathway renders the nerve susceptible to various pathologies, leading to NI neuralgia. Here, the authors present an unusual intraoperative finding

OBJECTIVE: To determine the outcomes of 354 radiofrequency rhizotomies and 21 neurovascular decompressions performed as treatment for 367 facial pain patients (290 idiopathic trigeminal neuralgia, 52 symptomatic trigeminal neuralgia, 16 atypical facial pain, 9 post-herpetic neuralgia).
METHOD: Clinical findings and surgery success rate were considered for evaluation. A scale of success rate was determined to classify patients, which considered pain relief and functional/sensorial deficits.
RESULTS: Radiofrequency rhizotomy was performed in 273 patients with idiopathic trigeminal neuralgia an

Neurovascular compression (NC) seems to have been confirmed as the major cause of classical trigeminal neuralgia (TN). In spite of the large number of surgically positive cases, however, there are still cases where no vascular compression of the trigeminal nerve can be found. To evaluate whether NC could be demonstrated preoperatively, high-resolution magnetic resonance angiography (MRA) was performed in 287 consecutive patients with TN and persistent idiopathic facial pain (PIFP) on a 0.5-T and a 1-T MR unit. Depending on the clinical symptoms, the TN cases were divided into typical TN and tr

BACKGROUND: Based on a classification scheme for facial pain syndromes and a binomial (yes/no) facial pain questionnaire, we previously reported on the ability of an artificial neural network (ANN) to recognize and correctly diagnose patients with different facial pain syndromes.
OBJECTIVES: We now report on an updated questionnaire, the development of a secure web-based neural network application and details of ANNs trained to diagnose patients with different facial pain syndromes.
METHODS: Online facial pain questionnaire responses collected from 607 facial pain patients (395 female, 65%,

PURPOSE: A patient-oriented classification scheme for facial pains commonly encountered in neurosurgical practice is proposed.
CONCEPT: This classification is driven principally by the patient's history.
RATIONALE: The scheme incorporates descriptions for so-called "atypical" trigeminal neuralgias and facial pains but minimizes the pejorative, accepting that the physiology of neuropathic pains could reasonably encompass a variety of pain sensations, both episodic and constant. Seven diagnostic labels result: trigeminal neuralgia Types 1 and 2 refer to patients with the spontaneous onset of f

Sixty patients with trigeminal neuralgia or atypical facial pain were followed for an average of 1 year after percutaneous retrogasserian glycerol rhizolysis. The procedure was initially effective in relieving pain in 80% of the patients with typical trigeminal neuralgia and symptomatic trigeminal neuralgia secondary to multiple sclerosis. However, life-table analysis indicated that 50% of this group had persistence or recurrence of pain within 18 months after the operation. Percutaneous retrogasserian glycerol rhizolysis was ineffective in relieving atypical trigeminal neuralgia or atypical f

OBJECTIVE: Trigeminal neuralgia is usually considered a separate entity from atypical trigeminal neuralgia. The exact relationship among these two and several other syndromes of facial pain remains unknown. There is no long-term prospective study of the natural history of trigeminal neuralgia nor any explanation for the existence of different, albeit somewhat similar, facial pain syndromes.
DESCRIPTION OF CONCEPT: On the basis of our clinical experience, we propose a theory that may explain different facial pain syndromes as sequential stages of the same disease process. Typical trigeminal ne

Orofacial pain syndromes have traditionally been divided into two groups: the typical neuralgias and atypical facial pain. While typical neuralgias are well characterised, the term atypical facial pain (now persistent idiopathic facial pain) covers a variety of poorly defined head and face pains whose aetiological mechanisms are poorly understood. This paper examines the characteristics and nosography of these conditions, and suggests they should not be considered as neuralgias (section 13 of the 2004 IHS classification) but as other primary headaches (section 4).

1052 patients with facial pain have been examined and followed up by the author for an 18-year period. The patients are classified according to type of attack into: Typical Trigeminal Neuralgia (brief pain paroxysms with pain-free intervals). Atypical Trigeminal Neuralgia (pain paroxysms with intervals of pain or paroxysms lasting for minutes). Non-neuralgiform Facial Pain (pain lasting or occurring for long periods). The material was equally distributed between patients with Neuralgia and Non-neuralgiform Facial Pain. In the majority of cases Trigeminal Neuralgia occurred after the age of 50.

Neurogenic facial pain can be classified as either paroxysmal or persistent. Trigeminal neuralgia is the commonest example of the former, and postherpetic neuralgia, atypical facial pain, and tension head and facial pains are examples of the latter. The cause of many of these pains is poorly understood, the complex neuroanatomy of the head and neck being a contributory factor. Even when the aetiology is known, the mechanism whereby pain is produced is usually obscure. While treatment with drugs and surgical measures for trigeminal neuralgia are often satisfactory, and acupuncture for pain due

BACKGROUND: Recurrent or chronic facial pain may be a diagnostic challenge. Applying the diagnostic criteria of the second edition of the International Classification of Headache Disorders (ICHD-II) leaves a considerable number of patients unclassifiable.
OBJECTIVE: The aim of this study was to establish and evaluate revised criteria of trigeminal neuralgia and persistent idiopathic facial pain.
METHODS: Based on the diagnostic value of 12 clinical features of trigeminal neuralgia and 15 features of persistent idiopathic facial in 97 patients referred for facial pain to a neurological tertia

Facial pain has a considerable impact on quality of life. Accurate incidence estimates in the general population are scant. The aim was therefore to estimate the incidence rate (IR) of trigeminal neuralgia (TGN), postherpetic neuralgia (PHN), cluster headache (CH), occipital neuralgia (ON), local neuralgia (LoN), atypical facial pain (AFP), glossopharyngeal neuralgia (GPN) and paroxysmal hemicrania (PH) in the Netherlands. In the population-based Integrated Primary Care Information (IPCI) medical record database potential facial pain cases were identified from codes and narratives. Two medical

257 patients with facial neuralgia (idiopathic trigeminal neuralgia, symptomatic trigeminal neuralgia, atypical facial pain) have been treated by percutaneous selective thermorhizotomy. 115 patients have been followed up 1-13 years after operation. In 71% we found excellent and good results. Preoperative symptoms, postoperative results of pain arrest, complications and cutaneous sensibility will be analyzed. The results are compared with data from the literature.

A sample of 228 patients with facial pain is reported in the present paper. Among them, 112 cases are typical idiopathic trigeminal neuralgias, 111 are cases of secondary facial pain (including 7 cases symptomatic trigeminal neuralgias) and 5 patients were classified as vascular facial pain. Among the 119 cases of clinically diagnosed trigeminal neuralgias there were 112 (94, 11%) with no evidence of organic cause. Only 7 cases (5.89%) were identified as organic, including 3 cases of bulbar and pontic vascular lesions, 2 cases of iatrogenic injuries following neurosurgery, 1 case of meningioma

In three patients, trigeminal sensory symptoms remitted with treatment of associated hydrocephalus, two with facial pain resembling trigeminal neuralgia, and one with facial paraesthesiae. The pathological basis of these phenomena and their relationship to idiopathic trigeminal neuralgia are discussed.

With the advances in endoscopic technology, endoscopy is widely used in many neurosurgical procedures, such as microvascular decompression, which is an effective method to treat glossopharyngeal neuralgia, trigeminal neuralgia, and facial spasm. The purpose of this study was to determine the efficacy of fully endoscopic microvascular decompression in the treatment of glossopharyngeal neuralgia. We managed a patient with glossopharyngeal neuralgia in our department, whose main clinical manifestation was recurrent left ear and facial pain for 3 years. The patient underwent a fully endoscopic mic

Facial pain is a debilitating disorder if left untreated. It has been suggested that the most commonly undiagnosed facial pain conditions include neuropathic and myofascial pains because their pathophysiologies are not well understood. Facial neuralgias are otherwise rare in children. They are not acknowledged in most paediatric tests, there are few published reports on them, and glossopharyngeal neuralgia (GPN) has not been described in children. Some of the most common forms of facial pain will be considered and some considerations concerning the problem of atypical facial pain in young age

Painful post-traumatic trigeminal neuropathy (PTTN), or anesthesia dolorosa, is a condition on the same continuum as trigeminal neuralgia. Unlike trigeminal neuralgia, trigeminal neuropathy is a more significant state of neural pathology resulting in constant pain opposed to the episodic nature of trigeminal neuralgia. This typically occurs after facial trauma or as a complication from some treatments for trigeminal neuralgia. Useful diagnostic features that can help distinguish PTTN are an identified trauma to the nerve and other clinical features such as facial swelling. The presented case d

Trigeminal neuralgia (TN) is a neuropathic pain disorder characterized by paroxysmal pain in the maxillary and mandibular regions of the face. Morbihan syndrome is a disease that classically presents with dermatologic findings, including progressive facial edema and erythema. There are no previous reports of the onset of trigeminal neuralgia with Morbihan syndrome or previous reports describing improvement in symptoms of Morbihan syndrome with treatment of trigeminal neuralgia. We describe the case of a 62-year-old female who presented with trigeminal neuralgia and shortly thereafter developed

The differential diagnosis of chronic facial pain is facilitated by a knowledge of anatomy. Nasal and dental conditions are prevalent causes of facial pain. Orbital discomfort with ophthalmoplegia or Horner's syndrome generally has a vascular etiology. The lower-half headache or atypical facial neuralgia also is vascular in origin and should be referred to as facial migraine. Previously, chronic iatrogenic trigeminal neuralgia has been erroneously included in the category of lower-half headaches. This disabling condition is due to repeated trauma to the trigeminal nerve.
