From Fear to Adaptation: The Journey of Patients with Liver Cancer Living with the Fear of Cancer Recurrence.

The study aimed to understand how patients with liver cancer experience and adapt to the fear of cancer recurrence, providing insights into psychological processes and strategies that can inform psycho-oncology research and interventions. In-depth interviews were conducted with 13 patients with liver cancer from December 2019 to February 2020 and analyzed using Colaizzi's phenomenological method. Four theme clusters emerged: (1) "Inevitable reality of recurrence," which highlighted the acceptance of recurrence; (2) "Amplified fears," which reflected heightened emotional distress; (3) "Changes

Disrupting fear memory reconsolidation in individuals with fear of spiders with cTBS: A Proof-of-Concept Study.

Anxiety disorders can be effectively treated with both cognitive behavioural therapy and psychopharmacological interventions. However, only 50 % of patients demonstrate significant benefits from these approaches. Therefore, investigating strategies to improve treatment effectiveness or develop novel therapeutic approaches remains an important research objective. Current therapeutic modalities may leave the original fear memory intact, potentially leading to symptom recurrence over time. In contrast, the disruption of reconsolidation processes can facilitate permanent modifications to fear memo

Hallucinogenic drugs and their potential for treating fear-related disorders: Through the lens of fear extinction.

Fear-related disorders, mainly phobias and post-traumatic stress disorder, are highly prevalent, debilitating disorders that pose a significant public health problem. They are characterized by aberrant processing of aversive experiences and dysregulated fear extinction, leading to excessive expression of fear and diminished quality of life. The gold standard for treating fear-related disorders is extinction-based exposure therapy (ET), shown to be ineffective for up to 35% of subjects. Moreover, ET combined with traditional pharmacological treatments for fear-related disorders, such as selecti

Effects of confronting the feared outcome during exposure therapy on the return of fear: An analogue study.

BACKGROUND AND OBJECTIVES: Although exposure therapy is an efficacious treatment for anxiety disorders, fear often returns after treatment. From an inhibitory learning perspective, long-term improvement depends not only on learning that feared stimuli are safe, but also that it is safe to experience the emotional response triggered by these stimuli. Accordingly, the current study examined the effects of occasional threat reinforcement during repeated exposure to multiple cues on the return of fear in snake phobia by incorporating reminders of the feared outcome. METHODS: Snake fearful communi

The Fear of Tinnitus Questionnaire: Toward a Reliable and Valid Means of Assessing Fear in Adults with Tinnitus.

OBJECTIVES: The purpose of this study was to assess the factor structure, reliability, and validity of the Fear of Tinnitus Questionnaire (FTQ); a brief self-report measure of people's fears about potential cognitive, emotional, behavioral, and social consequences of living with tinnitus. DESIGN: Five hundred eighty-eight Dutch-speaking adults with tinnitus completed an online battery of questionnaires measuring tinnitus-related distress, fear, catastrophizing, and quality of life. The sample was randomly split into two to perform exploratory and Bayesian confirmatory factor analyses. A subsa

Fear load: The psychophysiological over-expression of fear as an intermediate phenotype associated with trauma reactions.

Psychophysiological measures of fear expression provide observable intermediate phenotypes of fear-related symptoms. Research Domain Criteria (RDoC) advocate using neurobiological intermediate phenotypes that provide dimensional correlates of psychopathology. Negative Valence Systems in the RDoC matrix include the construct of acute threat, which can be measured on a physiological level using potentiation of the acoustic startle reflex assessed via electromyography recordings of the orbicularis oculi muscle. Impairments in extinction of fear-potentiated startle due to high levels of fear (term

Anxiety sensitivity: another reason to separate dental fears from blood-injury fears?

Although dental phobia is classified under the heading of blood-injury phobia, studies show differences between the two conditions in terms of frequency of fainting and gender distribution. Anxiety sensitivity (AS), which refers to discomfort and negative attributions to bodily anxiety sensations, was useful in differentiating panic anxiety from other phobic anxieties. No study has compared dental phobia with blood-injury phobia directly. We examined 61 subjects, working at a military aircraft factory, using measures on demographics, dental fears (MDAS - Modified Dental Anxiety Scale) and bloo

The legacy of fear: is fear impacting fatal and non-fatal drowning of African American children?

African American children’s rates for fatal and non-fatal drowning events are alarmingly elevated, with some age groups having three times the rate as compared to White peers. Adequate swimming skills are considered a protective agent toward the prevention of drowning, but marginalized youth report limited swimming ability. This research examined minority children’s and parents/caregivers’ fear of drowning as a possible variable associated with limited swimming ability. Results confirmed that there were significant racial differences concerning the fear of drowning, and adolescent African Amer

Fear-related pictures deteriorate the performance of university students with high fear of snakes or spiders.

Despite research regarding emotional processing, it is still unclear whether fear-evoking stimuli are processed when they are irrelevant and when attention is oriented elsewhere. In this study, 63 healthy university students with high fear from snakes or spiders participated in two different experiments. In an emotional modification of the spatial cueing task, 31 subjects (5 males) were asked to detect a target letter while ignoring a neutral or fear-related distracting picture. The distribution of attention was independently manipulated by a spatial cue that preceded the appearance of the pic

Testosterone reduces unconscious fear but not consciously experienced anxiety: implications for the disorders of fear and anxiety.

BACKGROUND: The fear-reducing properties of testosterone have been firmly established in animals but not in humans. However, human data on the relation between testosterone, fear, and anxiety have predominantly involved questionnaires that index cortically executed conscious appraisal of anxious mood. Animal studies, on the other hand, indicate that the effects of testosterone on motivation and emotion are of subcortical origin and of unconscious nature. Presently, it was hypothesized that a single testosterone administration to humans would reduce unconscious fear but not consciously experien

Fear of blushing: effects of task concentration training versus exposure in vivo on fear and physiology.

Patients with fear of blushing as the predominant complaint (N = 31) were randomly assigned to (1) exposure in vivo (EXP), or (2) task concentration training (TCT), in order to test the effect of redirecting attention above exposure only. In addition, it was investigated whether treatment reduced actual blush behavior; therefore, physiological parameters of blushing were measured during two behavioral tests. Half of the patients served as waiting-list controls first. Assessments were held before and after treatment, at 6-weeks, and at 1-year follow-up. Both treatments appeared to be effective

Adversity-induced relapse of fear: neural mechanisms and implications for relapse prevention from a study on experimentally induced return-of-fear following fear conditioning and extinction.

The efficacy of current treatments for anxiety disorders is limited by high relapse rates. Relapse of anxiety disorders and addiction can be triggered by exposure to life adversity, but the underlying mechanisms remain unexplored. Seventy-six healthy adults were a priori selected for the presence or absence of adverse experiences during childhood (CA) and recent past (RA; that is, past 12 months). Participants underwent fear conditioning (day 1) and fear extinction and experimental return-of-fear (ROF) induction through reinstatement (a model for adversity-induced relapse; day 2). Inactivation of medial or lateral orbitofrontal cortex during fear extinction did not interfere with fear renewal.

Hyperactive orbitofrontal cortical activation is commonly seen in patients of obsessive-compulsive disorder (OCD). Previous studies from our laboratory showed that for rats with aberrant activation of the orbitofrontal cortex (OFC) during the extinction phase, they were unable to use contexts as the reference for proper retrieval of fear memory during renewal test. This result supported the phenomenon that many OCD patients show poor regulation of fear-related behavior. Since there are robust anatomical connections of the OFC with the fear-circuit, we aim to further examine whether the OFC is

Fear-Conditioning to Unpredictable Threats Reveals Sex and Strain Differences in Rat Fear-Potentiated Startle (FPS).

Fear-potentiated startle (FPS) has been widely used to study fear processing in humans and rodents. Human studies showed higher startle amplitudes and exaggerated fear reactivity to unpredictable vs. predictable threats in individuals suffering from post-traumatic stress disorder (PTSD). Although human FPS studies use both sexes, a surprisingly limited number of rodent FPS studies use females. Here we investigate the effects of signal-threat contingency, signal-threat order and threat predictability on FPS in both sexes. We use a classic fear-conditioning protocol (100% contingency of cue and

Alterations in fear extinction neural circuitry and fear-related behavior linked to trauma exposure in children.

Exposure to childhood trauma is extremely common (>60 %) and is a leading risk factor for fear-based disorders, including anxiety and posttraumatic stress disorder. These disorders are characterized by deficits in fear extinction and dysfunction in underlying neural circuitry. Given the strong and pervasive link between childhood trauma and the development of psychopathology, fear extinction may be a key mechanism. The present study tests the impact of childhood trauma exposure on fear extinction and underlying neural circuitry. Children (N = 44, 45 % trauma-exposed; 6-11 yrs) completed a nove

The next generation of fear generalization.

Generalizing from past experience is a fundamental feature of cognition. Although generalization has been of long-standing interest to diverse domains in psychological science, the past decade has seen a rapid acceleration of fear generalization research in particular. The field of human fear generalization is at a pivotal juncture where it is now possible to synthesize extensive behavioral work, competing theoretical models, animal and human neuroscience, and clinical and individual-differences research to address enduring gaps. In this review, we provide an updated account of where the field

Neuroscience in pictures: Neuroscience of fear.

Fear is an evolutionarily adaptive mechanism that enables organisms to detect and respond to potential threats. Over the past century, theories of fear have evolved from Pavlovian and behavioral frameworks to neuroscientific models that emphasize specific neural circuits, neurotransmission, and plasticity. This pictorial review synthesizes key concepts underlying the neuroscience of fear, starting with a historical narrative of fear as conceptualized by conditioning and behaviorism theories, to the roles of different neurobiology structures, networks, to concepts of memory consolidation, recon

Neuromodulation of Fear and Anxiety Circuits.

Anxiety is a complex and heterogeneous condition consisting of multiple component processes and neural underpinnings. Recent neural accounts for anxiety have expanded beyond the canonical anxiety and fear circuitry centered on the amygdala to a distributed network. A burgeoning form of clinical intervention with remarkable potential to directly engage the neural substrates of anxiety is non-invasive brain stimulation (NIBS), which is especially notable for the ability to modulate macro- and meso-scopic circuits and networks. NIBS is poised as a powerful research tool with the ability to extend

[Perioperative anxiety and fear of death].

An important field of anesthesiology but also of psychiatry and psychotherapy, is perioperative anxiety and especially the fear of death. In this review article the most important types of anxiety in the individual phases before, during and after surgery are presented and diagnostic aspects as well as risk factors are discussed. Benzodiazepines can classically be used therapeutically here, but in recent years the preoperative anxiety-reducing effects of e.g., supporting talks, acupuncture, aroma therapy, and relaxation methods have come more into focus, because benzodiazepines promote postoper
