Fecal incontinence in patients with spina bifida: The target is the rectum.

AIMS: The prevalence rate of severe fecal incontinence (FI) in adults with spina bifida (SB) is high. The physiological basis of FI in SB has not been clearly established, which contributes to inadequate care. The aim was to better characterize a large cohort of adults with special consideration of anorectal physiology. METHODS: A multidisciplinary team from a French referral centre for SB prospectively collected data on patients who had an anorectal manometry. Factors associated with severe FI (Cleveland clinical incontinence score ≥ 9) were assessed in a multivariate analysis model. RESULT

[Anorectal manometry and ultrasound in the study of patients with fecal incontinence].

INTRODUCTION: Fecal incontinence is a complex disease that affects the quality of life of patients suffering from it. Anorectal manometry and endoanal ultrasound 360 grades are included among the studies used. There are no clear agreement about the relevance and usefulness of this studies and their role as apart of diagnostic procedures. OBJECTIVE: To describe the relationship between the clinical severity score in patients with fecal incontinence and the findings of anorectal manometry and endoanal ultrasound. Another objective of this study is to describe the degree of agreement of both met

Efficacy of sacral nerve stimulation for the treatment of fecal incontinence.

BACKGROUND: Sacral nerve stimulation has been shown to be an effective treatment for fecal incontinence and early studies reported success rates of 67% to 100%. However, "success" has been arbitrarily set at a 50% reduction in symptoms, and data are rarely reported with "intention to treat." OBJECTIVE: This study aimed to assess the true efficacy of sacral nerve stimulation for fecal incontinence compared with the published literature. DESIGN: This prospective 5-year study was conducted to assess 50 patients with fecal incontinence treated with sacral nerve stimulation. All analyses were per

Treatment of fecal incontinence with a comprehensive bowel management program.

PURPOSE: Many articles describe the antegrade continence enemas (ACEs), but few refer to a bowel management program. A successful ACE may not help a patient without such management. Valuable lessons were learned by implementation of bowel management in 495 fecally incontinent patients. METHODS: We previously reported 201 patients. Thereafter, another 294 patients participated in our program. On the basis of a contrast enema and symptoms, they were divided as follows: (a) 220 constipated patients and (b) 74 patients with tendency toward diarrhea. Colonic stool was monitored with abdominal radi

Surgery and sacral nerve stimulation for constipation and fecal incontinence.

Fecal continence is a complex bodily function, which requires the interplay of sensation, rectal capacity, and anal neuromuscular function. Fecal incontinence affects approximately 2% of the population and has a prevalence of 15% in elderly patients. Constipation is one of the most common gastrointestinal disorders. The variety of symptoms and risk factors suggest a multifactorial origin. Before any invasive intervention, the surgeon should have a thorough understanding of the etiology of these conditions. Appropriate medical management can improve symptoms in the majority of patients. Surgery

Dynamic graciloplasty in children with fecal incontinence: a preliminary report.

BACKGROUND: Various therapeutic methods have been developed for anal incontinence. A reconstruction of the sphincter system with functional adaptation can be achieved by stimulation of the transposed gracilis muscle with an implanted stimulator. METHODS: The authors performed a dynamic graciloplasty in 4 boys, aged between 6 and 10 years. Causes for the incontinence were polytrauma with pelvic rupture, VACTERL syndrome with rectal urethral fistula, anal atresia with primary reconstruction and 6 consecutive operations, and coccygeal teratoma in a premature infant with surgical treatment of the

Delorme's operation and sphincteroplasty for rectal prolapse and fecal incontinence.

Clinical and manometric results of Delorme's operation and sphincteroplasty were assessed retrospectively in patients undergoing this procedure for fecal incontinence and rectal prolapse. A series of 33 patients (11 males, 22 females; aged 18-83 years, mean 59) with external rectal prolapse were treated by Delorme's operation between 1989 and 1996. Mean follow-up was 39 months (range 7-84). Sphincteroplasty was associated in 12 cases with severe fecal incontinence due to striated muscle defects. Good results were achieved in 27 patients (79%); prolapse recurrence was observed in 6 (21%), the m

[The possibilities for the surgical treatment of neurogenic fecal incontinence in children].

Treatment of anal incontinence in children with a pathology of the distal portion of the spine is a serious problem. We observed 36 children aged from 3 to 6 years with the 3rd degree of fecal incontinence. Twenty patients had spina bifida, 12 had meningocele, 4 children had the following symptoms: disturbed formation of the arches, sacrum agenesia, deformity of the distal portion of the spine. The conservative treatment was effective but in children without concomitant neurological disorders (motor disorders of the lower extremities, disturbance of the perineum sensitivity). Great increase of

[Postanal repair in the treatment of idiopathic neurogenic fecal incontinence].

Faecal incontinence is an important disabling symptom in the affected patients. Classically, we divide faecal incontinence in two main types: traumatic faecal incontinence and neurogenic faecal incontinence. Neurogenic faecal incontinence is characterized by a diffuse weakness of anal sphincter and of the elevator muscles. The aim of the present study was to evaluate the outcome of postanal repair procedure in the management of neurogenic faecal incontinence. To this end we studied 20 patients with neurogenic faecal incontinence who underwent a postanal repair procedure according to the method

Effectiveness of Medical-Behavioral Therapy in Patients with Fecal Incontinence.

BACKGROUND AND AIMS: Guidelines recommend individualized medical and behavioral therapy (MBT) for fecal incontinence (FI), yet its effectiveness remains poorly defined. We assessed the response to MBT in FI. METHODS: In this 5-center U.S. trial, adults with FI received individualized MBT (dietary modifications, medications, and pelvic floor exercises) for up to 3 months. The primary outcome was a ≥75% reduction in FI frequency after 4 weeks of MBT. Secondary outcomes included 3-month FI, bowel habits, FI-related quality of life (QoL) and changes exceeding minimal clinically important differen

Hypotheses on physiopathogenic mechanisms of fecal incontinence in Wolfram syndrome.

Type 1 Wolfram syndrome is a rare, neurodegenerative pathology characterized by the association of insulin-dependent diabetes and optic atrophy. Other symptoms include diabetes insipidus, hearing loss and neurological damage. Fecal incontinence was recently recognized as a new symptom. Its pathophysiological mechanism remains unknown. We plan to collect data on the severity of the syndrome at the onset of fecal incontinence, the frequency of signs suggestive of pons volume reduction, and the frequency of urinary disturbances. Based on this information, we will investigate the possible location
