Electrophysiological characteristics of the pediatric femoral nerve and their use in clinical diagnosis.

OBJECTIVE: To explore the electrophysiological characteristics of the pediatric femoral nerve at different ages. METHODS: Surface electrodes were used to detect femoral nerve conduction in 163 healthy children aged 0-14 years recruited to this study and divided into six age groups. Based on the range of normal values obtained, the diagnosis of 22 patients with suspected femoral nerve injury was confirmed. RESULTS: We obtained normal values for pediatric femoral nerve motor and sensory conduction in all age groups, including proximal and distal compound muscle action potential latencies, prox

Nerve blocks for initial pain management of femoral fractures in children.

BACKGROUND: Children and adolescents with femoral fractures are almost always admitted to hospital. They invariably start their hospital experience in the Emergency Department, often requiring transfer to a specialist children's hospital. They require analgesia or anaesthesia so that radiographs can be obtained and for management of their fractures. The initial care process involves from two to six transfers from stretcher to stretcher/imaging/operating-suite table or hospital bed within the first few hours, so prompt pain relief is essential. Systemic analgesia can be provided orally or paren

What is the ED95 of prilocaine for femoral nerve block using ultrasound?

BACKGROUND: Our aim was to estimate the ED95 of prilocaine 1% w/v for femoral nerve block. METHODS: This two-stage dose-finding sequential clinical trial followed an adaptive design based on the continual reassessment method (CRM). Adult patients undergoing Vastus medialis muscle biopsy under ultrasound-guided femoral nerve block were recruited. Data from previously published studies and our own previous experience were used to set the dose levels and their guesstimate probabilities of response. RESULTS: Forty patients were recruited in the trial (n=26 in the first stage and n=14 in the seco

Ultrasound in diagnosis of retroperitoneal femoral nerve injury: a case report.

We present a case of retroperitoneal femoral nerve injury after cut wounds at the lower abdomen. Electrodiagnostic tests revealed impaired function of nerve and muscles. A mass was observed at the retroperitoneal part of the femoral nerve by ultrasound, indicating that a traumatic neuroma composed of disordered fascicles was configurated after the injury. Postoperative pathology confirmed hypertrophic and hyperplastic nerve bundles of the mass. Great improvements were shown in the patient's symptoms and electrodiagnostic tests in the subsequent 3 months. This case presented a rare position of

Anatomical variations of the lateral femoral cutaneous nerve and the consequences for surgery.

PURPOSE: This two-part study was undertaken to (a) determine the course and variations of the lateral femoral cutaneous nerve in a cadaver study and (b) develop prospectively the preoperative protocol to diminish the possibility of a postoperative meralgia paresthetica; the latter was achieved by reviewing the patient series retrospectively for complaints of a lateral femoral cutaneous nerve (LFCN) lesion and by studying the relation of the course of the LFCN and meralgia paresthetica. METHODS: Anatomical studies were performed on 200 recently deceased patients. After exploring the LFCN from

[War injuries of the femoral nerve. Apropos of a series of 27 cases].

The authors report a series of 27 war injuries to the femoral nerve which represent approximately 1% of all of the war injuries to peripheral nerves operated in our department since the Islamic revolution in Iran in 1978 and during the 8 years of the war between Iran and Iraq. All of the victims presented with associated lesions: abdominal (24 laparotomies, including 10 colostomies) and/or vascular (3 iliac artery by-pass grafts). We divided the cases into 3 groups. In Group 1 (18 cases), the femoral nerve had a mean defect of 8 centimetres which was grafted. In Group 2 (4 cases), the nerve wa

Subclinical entrapment of the lateral femoral cutaneous nerve: an autopsy study.

Five out of 12 lateral femoral cutaneous nerves, removed at routine autopsies, showed pathologic changes in myelinated nerve fibers in the vicinity of the inguinal ligament. These changes included both local demyelination and Wallerian degeneration, particularly affecting the fibers with the largest diameters. The presence of polarized internodal swellings on single nerve fibers from two specimens suggested that mechanical factors were involved in pathogenesis. Endoneurial vascular thickening confined to the region of the inguinal ligament was also seen and may be implicated in the production

Femoral nerve palsy: compression by lymph glands in the inguinal region.

Femoral neuropathy caused by compression of the femoral nerve is not a common syndrome when compared with other compression neuropathies. In most cases reported, the compression was intraabdominal, either in the retroperitoneal region or in the course of the nerve through the psoas major muscle. In this case, the lesion was located extraabdominally. Enlarged and inflamed nodes in the inguinal region caused compression of the nerve resulting in femoral neuropathy. Electromyography (emg) and nerve conduction studies revealed that the lesion was neurapraxia. As expected, the condition was reversi

Preoperative Ultrasound-Guided Wire Localization of the Lateral Femoral Cutaneous Nerve.

BACKGROUND: Difficulty and sometimes inability to find the lateral femoral cutaneous nerve (LFCN) intraoperatively is well known. Variabilities in the course of the nerve are well documented in the literature. In a previous paper, we defined a tight fascial canal that completely surrounds the LFCN in the proximal thigh. These 2 factors sometimes render finding the nerve intraoperatively, to treat meralgia paresthetica, very challenging. OBJECTIVE: To explore the use of preoperative ultrasound to minimize operative time and eliminate situations in which the nerve is not found. METHODS: Since

Laparoscopic Neurolysis of Deep Endometriosis Infiltrating Left Femoral Nerve: Case Report.

Pelvic endometriosis may infiltrate somatic nerves causing severe neuropathic symptoms with a high impact on quality of life. It is a medical condition poorly known, and few published data about involvement of femoral nerve are available. We report an isolated unilateral endometriosis lesion of the left lumbar region infiltrating the femoral nerve in a 38-year-old woman. She described severe dysmenorrhea, dyspareunia, dischezia, and chronic pelvic pain with irradiation to the anterior part of the left thigh. After investigation, it was identified as a 5-centimeter endometriotic nodule involvin

Unusual entrapment of deep peroneal nerve after femoral distal extension osteotomy.

The lateral exposure of the supracondylar femur includes the risk of damaging the neurovascular structures or tightening of the neurological structures within the popliteal fossa may occur as a complication of the osteotomy. Although different pathways of common peroneal nerve (CPN) have been reported throughout the literature, division of deep and superficial branches above the supracondylar femur level has not been reported. A 15-year-old boy with bilateral knee flexion contracture and spastic diplegic cerebral palsy underwent bilateral femoral distal extension osteotomy. The authors found a

Femoral nerve palsy. An unusual complication of anterior lumbar interbody fusion.

SUMMARY OF BACKGROUND DATA: Compression neuropathy of the femoral nerve has been reported as an uncommon complication of bleeding into the iliopsoas muscle. OBJECTIVE: The authors detected anatomic reasons of direct injury to the femoral nerve at the lower lumbar level. METHODS: Keeping the hip in extension during the course of carrying out anterior fusion on a previously failed posterior fusion was considered another causative factor of femoral nerve injury. Anatomical dissection confirmed the likelihood of this injury being produced in this situation. RESULTS: Femoral nerve traction and c

Femoral nerve dysfunction after retroperitoneal hemorrhage: pathophysiology revealed by computed tomography.

In three patients receiving anticoagulation therapy who developed retroperitoneal hemorrhage computed tomography (CT) clearly localized the resulting hematoma in each case. Three distinct syndromes are described. A hemorrhage within the iliacus muscle resulted in femoral nerve dysfunction. A large hemorrhage within the iliacus muscle which extended into the psoas muscle produced both femoral and obturator nerve dysfunction. A retroperitoneal hemorrhage extrinsic to both the iliacus and psoas muscles did not produce peripheral nerve dysfunction. The pathophysiology of peripheral nerve dysfuncti

Lateral Femoral Cutaneous Nerve Cryoneurolysis for Meralgia Paresthetica: A Case Report.

Meralgia paresthetica (MP) is a disorder of lateral femoral cutaneous nerve mononeuropathy caused by entrapment or compression of the nerve. It is characterized by numbing, tingling, and burning pain in the lateral aspect of the thigh. The current treatments for MP include conventional medical management, peripheral nerve blocks, and surgical interventions. Some patients who suffer from MP can experience intractable pain and medical management of MP is often inadequate to provide satisfactory pain control. Although regional anesthesia provides excellent pain relief, the analgesic effects of pe

Deep Decompression of the Lateral Femoral Cutaneous Nerve Under Local Anesthesia.

OBJECTIVE: Meralgia paresthetica is a mononeuropathy of the lateral femoral cutaneous nerve (LFCN) caused by compression around the inguinal ligament. We report a surgical alternative for the treatment of meralgia paresthetica under local anesthesia and its outcomes. METHODS: We operated on 12 patients with unilateral meralgia paresthetica whose age at surgery ranged from 62 to 75 years. The mean postoperative follow-up period was 19 months. Microsurgical deep decompression of LFCN was performed with the patient under local anesthesia. Clinical outcomes of surgical treatment were assessed bas

Nerve Tissue Prefabrication Inside the Rat Femoral Bone: Does It Work?

AIM: To investigate whether nerve regeneration can be induced in the tubular bone between distal and proximal cut nerve ends. MATERIAL AND METHODS: Twenty adult Wistar rats were used for the study. Rats were divided into three groups; femoral bone conduit group, nerve transection group, sham group. The sciatic nerve was surgically cut and from both ends inserted into the adjacent femoral bone tunnel in the femoral bone conduit group. The sciatic nerve was cut transversely in the nerve transection group. In the Sham group, only sciatic nerve exploration was performed, without a nerve cut. The

Single-injection or continuous femoral nerve block for total knee arthroplasty?

BACKGROUND: The ideal local anesthetic regime for femoral nerve block that balances analgesia with mobility after total knee arthroplasty (TKA) remains undefined. QUESTIONS/PURPOSES: We compared two volumes and concentrations of a fixed dose of ropivacaine for continuous femoral nerve block after TKA to a single injection femoral nerve block with ropivacaine to determine (1) time to discharge readiness; (2) early pain scores and analgesic consumption; and (3) functional outcomes, including range of motion and WOMAC scores at the time of recovery. METHODS: Ninety-nine patients were allocated
