Fever of unknown origin: clinical overview of classic and current concepts.

Fever of unknown origin (FUO) refers to disorders that present with prolonged and perplexing fevers that are difficult to diagnose. This article presents a clinical overview of classic and current causes of FUOs, which may be due to infectious, rheumatic/inflammatory, neoplastic, or miscellaneous disorders. Comprehensive but nonfocused diagnostic testing is ineffective and should be avoided. The FUO workup should be directed by the key history, physical, and laboratory findings in clinical presentation. The clinical syndromic approach in the differential diagnosis of FUOs is emphasized, and th

[Clinical significant of splenectomy for fever of unknown origin with splenomegaly].

OBJECTIVE: To investigate the feasibility, safety, success rate of diagnosis and possible risk factors of splenectomy for fever of unknown origin (FUO) with splenomegaly. METHODS: The clinical data of 54 patients of FUO with splenomegaly who underwent splenectomy in Peking Union Medical College Hospital in the past 20 years were reviewed retrospectively. The diagnosis was not clear even after an extensive clinical evaluation before surgery. The pathologic findings, morbidity, mortality and possible risk factors were analyzed. RESULTS: Pathological diagnosis was made in 39 of the 54 patients

Reverse Shapiro's syndrome--an unusual cause of fever of unknown origin.

Reverse Shapiro's syndrome was first described by Hirayama et al. in a girl with periodic hyperthermia associated with complete agenesis of the corpus callosum. Here we report another such case in a 9-month-old girl presenting with fever of unknown origin since the age of 7 months. On examination, she had mild hypotonia with delayed developmental milestones. No other neurological or physical abnormalities were noted. The cause of her prolonged fever of unknown origin was investigated and all results were negative. Her brain magnetic resonance images showed agenesis of the corpus callosum. On t

Linear IgA dermatosis: a new cause of fever of unknown origin.

We describe a 60-year-old patient who presented with prolonged fever, weight loss, adenopathies and malaise. Two months later, an aphthous stomatitis and pharyngitis developed, together with ulcerations bilaterally in the groin. Two separate skin biopsies revealed the diagnosis of linear IgA dermatosis. This entity, which is well known to dermatologists but not to internists, should be added to the extensive list of disorders than can provoke the syndrome of fever of unknown origin.

Fever of unknown origin: a follow-up investigation of 34 patients.

34 patients hospitalized for fever of unknown origin (FUO) were subjected to a follow-up investigation on the average 5 yr after the hospital stay. Two patients had died of a disease related to FUO: one had adenocarcinoma with an unknown primary site and the other a supposed autoimmune disease. In one case the cause of FUO was a periappendicular abscess. A woman continued to have periods of fever with high ESR. In the remaining 30 cases the cause of FUO had apparently been a self-limited, benign disease. In contrast to an earlier series from our clinic, no cases of tuberculosis were revealed.

Investigation of the Blood Microbiome in Horses With Fever of Unknown Origin.

BACKGROUND: Fever of unknown origin (FUO) without a respiratory component is a frequent clinical presentation in horses. Multiple pathogens, both tick-borne and enteric, can be involved as etiologic agents. An additional potential mechanism is intestinal barrier dysfunction. OBJECTIVES: This case-control study aimed to detect and associate microbial taxa in blood with disease state. STUDY DESIGN: Areas known for a high prevalence of tick-borne diseases in humans were chosen to survey horses with FUO, which was defined as fever of 101.5°F or higher with no signs of respiratory illness or othe

Helicobacter canis bacteremia in a patient with fever of unknown origin.

A 57-year-old woman with common variable immune deficiency and liver failure of unknown etiology presented with recurrent fevers over a 5-month period. She was found to have Helicobacter canis bacteremia. Immunocompromised hosts with exposure to cats or dogs may be at risk for infection with this organism, which may be challenging to diagnose.

Fever of unknown origin in pregnancy: the need for a full history.

A 26-year-old woman with 29 weeks gestation presented with headache, photophobia and fever. She had deranged liver function tests and low platelets on admission. Blood film, performed to look for haemolysis, revealed she had Plasmodium vivax malaria, despite not having travelled to an endemic malaria area for over 1 year. The diagnosis was confirmed on PCR test performed in the HPA malaria reference laboratory in London and she was treated with chloroquine. She delivered a healthy baby at 33+3 weeks gestation, and once the patient and the baby had both tested negative for glucose-6-phospate de

[Severe periodontal disease as the cause of a fever of unknown origin].

A 41-year old man was admitted to a university medical centre with a belly ache, bloody diarrhoea and fever with peaks rising up to 39 degrees C. The differential diagnosis included among others infectious bowel disease, tuberculosis and amoeba-infection. After the exclusion of other pathologies the diagnosis morbus Crohn was given. A few days later the patient complained about toothache and severe periodontal disease was diagnosed. Instantly after extraction of the entire dentition the fever disappeared.

Tuberculosis of the spleen as a cause of Fever of unknown origin and splenomegaly.

BACKGROUND/AIMS: Splenic involvement of tuberculosis, which is rare, warrants better definition in the current era of resurgence of tuberculosis. METHODS: Out of 339 splenectomies performed between January 1989 and December 2008 for indications other than trauma, histopathologic analysis of the spleen revealed tuberculosis in 8 patients. RESULTS: All eight patients were referred for splenectomy due to fever of unknown origin (FUO). No patient was infected with HIV, and all had at least moderate splenomegaly and hepatomegaly. Three patients had hypersplenism with bleeding manifestations. Radi

Fever of unknown origin in a hemodialysis patient with a failed allograft.

Fever of unknown origin (FUO) in hemodialysis (HD) patients represents a diagnostic challenge because differential diagnosis includes diverse etiologies. Causes of FUO in the general population can be classified into 3 diagnostic categories: infections, tumors, and noninfectious inflammatory diseases. Also, chronic HD patients may have additional problems such as infections, the risk for which may be increased by the immunosuppression associated with uremia, vascular access-related infections, and nosocomial infections. Moreover, patients with chronically failed kidney transplants can have low

[Fever of unknown origin in a young child: diagnosis and treatment].

In children with fever without focus, evaluation should be aimed at recognition of children with an increased risk of bacteraemia or of major bacterial infections, while young children in particular will show few typical symptoms of bacterial infections. A child younger than 1 month with fever (> or = 38.0 degrees C) or hypothermia (< 36.0 degrees C) needs clinical evaluation and additional diagnostic investigations. In children aged from 1 to 36 months referral to hospital should be based on presence of age specific clinical characteristics, i.e. toxic appearance, high fever, diarrhoea, decre

[The Schnitzler syndrome as a cause of recurrent fever of unknown origin].

HISTORY AND CLINICAL FINDINGS: For 4 years a 56-year-old woman had been suffering from chronic urticaria. In the past two years she had developed recurrent fever, each 1-3 days in duration. In the last 6 to 8 months she also had severe aching in the legs. There were no significant findings on physical examination other than non-itching urticarial rash over trunk and limbs. INVESTIGATIONS: Blood sedimentation rate was increased (maximum 88/110 mm), as were WBC count (16,200/microliter, 83% neutrophils) and the activity of alkaline leucocyte phosphatase (225 U/l). Monoclonal IgM gammopathy type

Fever of unknown origin in the elderly. A sequential approach to diagnosis.

Elderly patients with persistent unexplained fever require a diagnostic evaluation that focuses on specific infections (eg, occult abdominal abscess, bacterial endocarditis, miliary tuberculosis), rheumatic disorders (eg, temporal arteritis, polyarteritis nodosa), and neoplasms (eg, lymphoma, nephroma). Assessment is directed by the subtle clues elicited from meticulous, repeated history taking and physical examination. Therapeutic trials or exploratory laparotomy may be appropriate but should not be attempted out of a sense of frustration.

Bartonella henselae tricuspid valve endocarditis presenting as fever of unknown origin.

Cat scratch disease (CSD), caused by Bartonella henselae, is an uncommon cause of fever of unknown origin (FUO) and rarely presents with visceral organ involvement. This case report describes a middle-aged woman with 3 months of night sweats and 1 month of fevers. Initial workup showed elevated inflammatory markers, multiple splenic lesions and a pelvic fluid collection. A detailed history noted that she recently adopted a stray kitten, prompting Bartonella serological testing. Given the atypical presentation, a broad infectious evaluation was also pursued, including testing for mycobacterial,
