Stress fractures of the proximal fibula are uncommon and usually result from axial loading, which is mostly described in runners. We report an unusual mechanism of such a fracture in a circus performer resulting from repetitive direct horizontal loading from a trapeze bar. In addition, the bony injury resulted in a secondary injury to the common peroneal nerve with corresponding weakness. Both injuries responded well to nonoperative treatment and the athlete had an excellent recovery with no residual symptoms. He was able to resume his training with the use of protective padding applied to the

Simple lateral malleolar fractures type B according to Weber can be fixed with a hemicerclage alone. Technique and results at sixteen own cases are shown. At this indication the hemicerclage is regarded as a good alternative to common methods of osteosynthesis. Advantages are simplicity, minimal damage of soft tissue and bone, high interfragmentary compression and allowed full weight bearing.

BACKGROUND: Ankle fractures are becoming more common in the elderly and their management is frequently challenging. There is increasing evidence of good outcomes following fibula nail fixation in the management of ankle fractures. The use of a fibula nail in open fractures comes with potential advantages such as minimal soft tissue dissection, early rehabilitation, and decreased wound complications. We aimed to assess their use in the management of open ankle fractures in the elderly.
METHODS: A review of patients aged 60 years or older with an open ankle fracture treated with a fibular nail

PURPOSE: Despite the fact that open reduction and internal fixation with a plate, either non-locked or locked, is the standard of care for managing lateral malleolus fractures, intramedullary (IM) fixation of the fibula has been recently introduced as an alternative, mainly for some potential complicated situations. We hypothesized that almost all patterns of distal fibula fracture can be safely fixed with an IM device, with the potential benefit of providing biomechanical efficiency, but using a soft-tissue friendly implant. Here, we present a multicenter case series based on a proposed algor

BACKGROUND: The mechanical role of supplemental fibula fixation in both bone lower leg fractures is not well defined. The benefit of fibula plate fixation in this context is controversial. The purpose of this study was to ascertain the mechanical contributions of the fibula under three conditions (intact, fractured, or plated fibula) following standard tibia locked plating.
METHODS: A laboratory fracture model was created with 10 cadaveric legs (5 matched pairs) with no known history of lower extremity trauma or other musculoskeletal conditions. A both bone lower leg fracture was simulated by

The tibia is the most commonly fractured long bone. Tibia fractures are simple, and most are amenable to immediate closed reduction. Reported cases of irreducibility resulting from entrapment of soft-tissue structures are rare. We report the case of a 23-year-old man who, in a high-speed motor vehicle crash, sustained a closed lower leg fracture that was later found to have a unique pattern in which the fibula was entrapped in the medullary cavity of the tibia. Limited open reduction was performed, and the entrapped distal fibula was removed with use of a bone hook. The tibia was reduced, and

We present for discussion a case of a right displaced proximal metaphyseal tibial fracture with intraarticular extension and a same-level proximal fibula fracture with severe degenerative arthritis in a medically complex 89 year old community dwelling woman with a history of paroxysmal atrial fibrillation, right lower extremity deep venous thrombosis 1 month prior to this admission on warfarin, obesity, peripheral vascular disease, and previous stroke. Her course is complicated by demand-mediated myocardial infarction. This case demonstrates that comanagement can lead to minimal delays and a s

The operative treatment of distal fibula fractures is debated. This study compared intramedullary nailing (IMN) versus open reduction and internal fixation (ORIF) in patients with distal fibula fractures. The outcomes of interest were patient-reported outcome measures (PROMs) and the rate of anatomic reduction, bony union, complication, and nerve injury. This study was conducted according to the 2020 PRISMA statement. In October 2024, the following databases were accessed: PubMed, Web of Science, Google Scholar, and Embase, with no time constraint. All the clinical studies comparing IMN versus

INTRODUCTION: Open (compound) fractures of the tibia and fibula diaphysis remain a major challenge in orthopedic trauma due to their high risk of infection, non-union, and chronic osteomyelitis. Effective management requires timely debridement, stable fixation, and meticulous soft-tissue handling . This study aimed to analyze the complication rates and identify the major determinants influencing outcomes in patients with open tibial fractures treated at a tertiary care center.
MATERIALS AND METHODS: A retrospective analysis was conducted on 138 consecutive patients with Gustilo-Anderson Grade

BACKGROUND: Assessment of mortise stability is paramount in determining appropriate management of ankle fractures. Although instability is readily apparent in bimalleolar or trimalleolar ankle fractures, determination of instability in the isolated Weber B fibula fracture often requires further investigation. Prior authors have demonstrated poor predictive value of physical examination findings such as tenderness, ecchymosis, and swelling with instability. The goal of this study is to test the validity of a new clinical examination maneuver, the lateral drawer test, against the gravity stress

There is no consensus on whether a fibular fracture should be fixed when a concurrent extra-articular distal tibia fracture is managed with intramedullary nails. We evaluated the use of fibular fixation in a meta-analysis of randomized trials and observational studies. Two researchers independently assessed the quality of eligible studies and extracted the data. We analyzed 4 trials with a pooled sample of 283 patients (mean age, 24 to 43 years; 141 men), 94 who had undergone fibular fixation and 189 who had not. Two randomized trials assessed on the Cochrane risk-assessment criteria were dete

AIM: Open reduction and internal plate fixation of the fibula is the gold standard treatment for ankle fractures. The aim of this study was to perform a prospective randomized study to compare bone union, complications and functional results of two types of internal fixation of the fibula (plating and the Epifisa FH intramedullary nail).
MATERIALS AND METHODS: Inclusion criteria were: closed fractures, isolated displaced fractures of the lateral malleolus, inter- and supra-tubercular bimalleolar fractures, and trimalleolar fractures. This study included 71 patients (mean age 53 ± 19): plate f

In a retrospective trial 235 patients with fractures of the tibia and/or fibula shafts were followed up. In 16 cases non-union of the fracture occurred after closed reduction and plaster cast treatment. In 16 of these patients closed nailing of the tibia was performed 8--14 weeks after the accident. The healing of the fracture was uncomplicated and occurred in 10--14 weeks p. op. The results in primary nailing cases 8--14 days after the accident were much better than in cases treated with plaster casts, with regard to joint mobility, venous insufficiency and duration of treatment; however two

BACKGROUND: Isolated distal fibula fractures (DFF) are usually treated with open reduction and internal fixation (ORIF) and non-weight-bearing protocols. The study assessed the outcomes of immediate weight-bearing on DFF healing and stability after lateral locking plating.
MATERIALS AND METHODS: For this study, 49 patients affected by isolated DFF were enrolled. They underwent ORIF with a lateral polyaxial locking plate and were allowed immediate weight-bearing with crutches postoperatively. Clinical and radiographic evaluations were performed at 2, 6, and 12 weeks using the AOFAS (American O

INTRODUCTION: Syndesmotic injury in trimalleolar fracture is necessary to address intraoperatively by stability test, as it affects post-operative functional outcomes. Most of syndesmotic injuries are stable after fixation of the posterior malleolus, but in rare cases, syndesmosis is not stable after adequate fixation. In those rare conditions, we have to fix syndesmosis by corticocancellous (CC) screw throw lateral fibula plate. We emphasize this complex problem and offer solutions to overcome it.

Salter-Harris type I (SH-I) fractures of the distal fibula are commonly encountered in pediatric orthopedics. We describe 2 unique cases of adolescents with completely displaced SH-I distal fibula fractures that were treated operatively. In the first case, a closed reduction attempt failed and the patient required open reduction and internal fixation of the distal fibula and syndesmosis. The syndesmotic ligaments were avulsed from the distal fibular metaphysis. In the second case, closed reduction of the distal fibula fracture was partially successful, but anatomic reduction could not be achie

Previous biomechanical studies have advocated the use of locking plates for isolated distal fibula fractures in osteoporotic bone. Complex rotational ankle injuries involve an increased number of fractures, which can result in instability, potentially requiring the same fixed angle properties afforded by locking plates. However, the mechanical indication for locking plate technology has not been tested in this fracture model. The purpose of the present study was to compare the biomechanical properties of locking and conventional plate fixation for distal fibula fractures in trimalleolar ankle

AIM: The aim of our study was to identify the structures which may be at risk of injury when using a minimally invasive technique for the osteosynthesis of the lateral malleolus and the influence of the size of the implant on the frequency of injury to these structures.
METHOD: Forty plates were percutaneously inserted in 20 cadaveric legs. The region around the plate was then dissected to examine the relation of nerves and soft tissues to the plate.
RESULTS: The superficial peroneal nerve was in direct contact with the plate in 11 of the 20 cases (55%) of the 10 hole plates. We encountered
