Addressing malnutrition in acute care through an innovative model: PEAR (patients eating and recovering).

BackgroundMalnutrition affects nearly half of patients in Canadian hospitals and is associated with increased complications, longer hospital stays, and higher mortality. Audits at an urban acute care hospital identified gaps in nutrition screening, oral intake monitoring, weight surveillance, and interdisciplinary coordination, highlighting the need for a structured, patient-centred nutrition care model.AimTo evaluate the patients eating and recovering (PEAR) model by assessing improvements in nutrition screening, oral intake monitoring, and weight surveillance; evaluating the feasibility and

Eco-FinOps: a causal-agentic framework for energy-efficient and explainable cloud cost optimization.

Two converging crises-uncontrolled operational expenditure and significant environmental hazards-have been aggravated by the exponential scaling of cloud infrastructure. This has led to a greater focus on the efficiency of Financial Operations (FinOps). Current monitoring methods exhibit a major flaw: passive dashboards require constant human monitoring and lead to alert fatigue, whereas deep learning models require substantial Graphics Processing Unit (GPU) resources and operate as opaque "black boxes." This lack of interpretability contradicts the sustainability goals these tools promise to

Malnutrition in the institutionalized elderly: the effects on wound healing.

Under-nutrition and protein-energy malnutrition are seen at alarmingly high rates in institutionalized elderly and in patients admitted to hospitals. A combination of immobility and loss of lean body mass - which comprises muscle and skin - and immune system challenges increases the risk of pressure ulcers by 74%. The development of pressure ulcers in the hospital affects 10% of admissions, with the elderly at the highest risk. Common causes of malnutrition in the elderly involve: decreased appetite, dependency on help for eating, impaired cognition and/or communication, poor positioning, freq

Establishing a Microsurgical Practice in a Limited Resource African Setting: The Kapsowar Hospital Experience.

BACKGROUND: Microsurgical technique is still not readily available in many low- and middle-income countries. Few works in the scholarly literature describe the establishment of microsurgical practice on the African continent, and there are virtually no descriptions of the financial aspects of free flap performance by locally staffed teams in sub-Saharan Africa. The Kapsowar Hospital is a hospital in rural Kenya with 2 plastic and reconstructive surgeons certified by the American Board of Plastic Surgery and has recently expanded clinical practice to include microsurgical procedures. METHODS:

Barriers and facilitators to implementing evidence based bleeding management in Australian Cardiac Surgery Units: a qualitative interview study analysed with the theoretical domains framework and COM-B model.

BACKGROUND: Bleeding during cardiac surgery is a common complication that often requires the transfusion of blood products. The combination of bleeding and blood product transfusion incrementally increases adverse outcomes including infection and mortality. Following bleeding management guideline recommendations could assist with minimising risk but adherence is not high, and the cause for lack of adherence is not well understood. This study aimed to identify barriers and facilitators to practicing and implementing evidenced-based intra-operative, bleeding management in Australian cardiac surg

Complications in pelvic floor surgery.

Pelvic floor disorders affect the quality of life of millions of women worldwide. Many options exist for the treatment of pelvic organ prolapse and urinary incontinence, surgery being one of the main strategies in the management of these conditions. Even though uncommon, all surgery has complications which can cause morbidity and rarely mortality. These complications can impair quality of life in the long-term and be a financial burden on both the patient and the health care system. Pelvic floor reconstructive surgery includes perioperative complications such as injury to neighboring organs, h

The impact of new diagnostic methodologies in the management of meningitis in adults at a teaching hospital.

BACKGROUND: Suspected meningitis is a frequent reason for admission to hospital in the UK. While bacterial meningitis requires prompt antibiotic therapy to reduce mortality and morbidity, enteroviral meningitis, the most frequent viral cause, is almost invariably a benign disease. AIM: To determine the clinical presentation, laboratory findings and outcome of meningitis by microbiological aetiology and patient age, and to assess the clinical management of adults presenting with meningitis, with reference to national guidelines. DESIGN: Retrospective case-note review. METHODS: Adult (>14 yea

Carotid endarterectomy: the financial impact of practice changes.

PURPOSE: New techniques in the management of extracranial carotid occlusive disease have focused attention on the outcome and economics of carotid endarterectomy (CEA). Changing practice patterns for CEA must be assessed to allow accurate comparisons. The purpose of this study was to evaluate the effect of practice modifications related to CEA on patient outcome and cost data. METHODS: Data on patients undergoing CEAs at a single institution from fiscal year 1992 to 1998 were prospectively collected and entered into a computerized database. Vaccine Handling Management in Korea, Japan, and Taiwan: A Comparative Study of Storage Standards, Implementation Practices, and Regulatory Approaches.

BACKGROUND: Robust vaccine cold-chain management safeguards potency and public trust. Korea, Japan, and Taiwan share core practices (2-8°C storage, continuous monitoring, first-expiry-first-out [FEFO] stock rotation) yet differ in implementation and governance. We compared standards and real-world practices across the three systems to identify actionable improvements for Korea. METHODS: From March-July 2025 we conducted structured interviews and onsite observations at public health centers, hospitals, and clinics in Korea, Taiwan (National Taiwan University Children's Hospital), and Japan (Ky

Exploring innovative models of doula services in maternity care: A qualitative study on advancing equity and addressing disparities.

BACKGROUND: Maternal and infant health disparities remain a persistent public health issue in the United States, disproportionately affecting systematically marginalized communities. While doulas have been shown to improve birth outcomes and reduce inequities, their integration into maternity care systems remains limited. OBJECTIVES: This study aimed to explore the roles and challenges of different doula models, identify systemic barriers to their integration, and propose strategies for sustainable implementation. DESIGN: A qualitative study using a phenomenological approach that adhered to

Daily Dispo Documentation Improves Patient Flow and Discharge Efficiency.

"Dispo," short for disposition, refers to the anticipated plan for a hospitalized patient's discharge, including timing and destination. Reducing hospital length of stay requires more than discharge orders and case management rounds. It demands daily, structured planning embedded in the clinical workflow. This review explores the underused yet high-impact strategy of "daily dispo documentation," in which hospitalists explicitly document five key discharge planning elements in each progress note. This framework has been adopted by some institutions to improve discharge efficiency. These element

Implementation of a quality improvement programme using the Active Patient Link call and recall system to improve timeliness and equity of childhood vaccinations: protocol for a mixed-methods evaluation.

INTRODUCTION: Call and recall systems provide actionable intelligence to improve equity and timeliness of childhood vaccinations, which have been disrupted during the COVID-19 pandemic. We will evaluate the effectiveness, fidelity and sustainability of a data-enabled quality improvement programme delivered in primary care using an Active Patient Link Immunisation (APL-Imms) call and recall system to improve timeliness and equity of uptake in a multiethnic disadvantaged urban population. We will use qualitative methods to evaluate programme delivery, focusing on uptake and use, implementation b

Hospitalization Costs for Diabetes Related Lower Extremity Amputation at a Referral Hospital in KwaZulu Natal, South Africa.

BACKGROUND: Diabetes mellitus is a significant risk factor for lower extremity amputations (LEA), both alone and in combination with peripheral vascular disease and infection. Currently, in Africa, more than half of the cases do not meet the recommended blood glucose control levels to prevent complications suggesting that the risk of complications is high. OBJECTIVE: The study aims to estimate hospitalization costs of diabetes-related lower extremities amputation for patients consulted at a referral hospital in 2015/16. METHODS: The study was a retrospective analysis using a mixed costing ap

Health care cost of crusted scabies in Aboriginal communities in the Northern Territory, Australia.

BACKGROUND: Crusted scabies is a debilitating dermatological condition. Although still relatively rare in the urban areas of Australia, rates of crusted scabies in remote Aboriginal communities in the Northern Territory (NT) are reported to be among the highest in the world. OBJECTIVE: To estimate the health system costs associated with diagnosing, treating and managing crusted scabies. METHODS: A disease pathway model was developed to identify the major phases of managing crusted scabies. In recognition of the higher resource use required to treat more severe cases, the pathway differentiat

APPROPRIATE DOSE ROUNDING OF CYTOKINE MODULATORS FOR PAEDIATRIC RHEUMATOLOGY INPATIENTS.

BACKGROUND: Cytokine modulators (adalimumab, infliximab, etanercept, anakinra, canakinumab, rituximab, tocilizumab and abatacept) are high-cost biologics used primarily in paediatrics to treat patients with juvenile idiopathic arthritis.1 Funding mechanisms are unreliable and inconsistent hence appropriate dose rounding is a key cost-saving measure.2 However, there is a lack of evidence-based guidance for dose rounding in paediatrics.3 AIMS AND OBJECTIVES: Determine if 100% of inpatient cytokine modulator prescriptions in rheumatology are dose rounded up or down to the nearest whole vial, pen

Evidence for Distinguishable Treatment Costs among Paranoid Schizophrenia and Schizoaffective Disorder.

BACKGROUND: Schizophrenia spectrum disorders result in enormous individual suffering and financial burden on patients and on society. In Germany, there are about 1,000,000 individuals suffering from schizophrenia (SZ) or schizoaffective disorder (SAD), a combination of psychotic and affective symptoms. Given the heterogeneous nature of these syndromes, one may assume that there is a difference in treatment costs among patients with paranoid SZ and SAD. However, the current the national system of cost accounting in psychiatry and psychosomatics in Germany assesses all schizophrenia spectrum dis
