Policy pathways to address American youth firearm injury and death.

Context: The burden of firearm violence and death are uniquely American problems. Over 90% of firearm deaths among children and adolescents in high income countries occur in the United States. Despite similar overall crime rates, the gun homicide rate is about 25 times higher in the U.S. than other Western democracies, and 49 times higher for those aged 15-24 years. Firearm-related injuries are a leading cause of child and adolescent deaths, second only to motor vehicle crashes. Approximately 4.6 million children are living in U.S. homes with at least one loaded, unlocked firearm. Reducing gun

Firearm trauma: Race and insurance influence mortality and discharge disposition.

BACKGROUND: Health insurance and race impact mortality and discharge outcomes in the general trauma population. It remains unclear if disparities exist by race and/or insurance in outcomes following firearm injuries. The purpose of this study was to assess differences in mortality and discharge based on race and insurance status following firearm injuries. METHODS: The National Trauma Data Bank (2007-2016) was queried for firearm injuries by International Classification of Diseases, Ninth/Tenth Revision, Ecodes. Patients with known discharge disposition, age (18-64 years), race, and insurance

Patient Characteristics and Perspectives of Firearm Safety Discussions in the Emergency Department.

INTRODUCTION: Firearm injury prevention discussions with emergency department (ED) patients provide a unique opportunity to prevent death and injury in high-risk patient groups. Building mutual understanding of safe firearm practices between patients and providers will aid the development of effective interventions. Examining ED patient baseline characteristics, perspectives on healthcare-based safety discussions, and experience with and access to firearms, will allow practitioners to craft more effective messaging and interventions. METHODS: Using an institutional review board-approved cross

Patterns of Pediatric Firearm-Related Ocular Trauma in the United States.

IMPORTANCE: Gun violence represents a substantial public health issue, and firearm-related injuries rank second among the causes of injury-related deaths in children aged 0 to 17 years in the United States. Ocular trauma from firearm-related injuries can lead to devastating vision loss, but little is known to date about the specific demographics and characteristics of such injuries in children. OBJECTIVE: To evaluate the epidemiologic pattern of pediatric firearm-related ocular injuries. DESIGN, SETTING, AND PARTICIPANTS: This retrospective analysis used deidentified data from the National T

A Cross-Sectional Study of Firearm Injuries in Emergency Department Patients.

UNLABELLED: This is a single-center cross-sectional study of consecutive patients who presented to an urban emergency department (ED) with firearm injuries between July 1, 2014 and June 30, 2016. The objective of this study was to describe the characteristics of patients in this cohort and identify those at risk of firearm re-injury to inform future interventions. Patients in this cohort with both initial and recidivistic firearm injures were found to be predominately young, African American men. OBJECTIVE: To describe the characteristics of patients who present to the emergency department (E

Differences in Firearm Suicides by Residential Location in Texas, 2006-2015.

This study used Texas state vital statistics records, 2006-2015, to examine firearm use rates among 28,010 suicide decedents by residential location (urbanized vs. all others). Firearms were responsible for 44% of all teenage suicides and 76-90% and 50-60% of suicides of men and women aged 60 + years, respectively, and firearm use rates remained steady for both genders during the study period. Logistic regression analysis showed a significantly higher firearm use rate (AOR = 1.35, 95% CI = 1.28-1.42) among decedents who resided in nonurbanized areas. Differences in firearm use rates by residen

Extremity firearm trauma: the impact of injury pattern on clinical outcomes.

As a group, the extremities are the most commonly injured anatomic region in nonfatal firearm trauma and are associated with high rates of vascular and bony injury. This study examines the epidemiology, incidence and distribution of firearm-related extremity trauma and the relationship between injury pattern and local or systemic complications. Review of the National Trauma Databank identified 6987 patients with isolated extremity firearm injury. Epidemiologic data, injury pattern incidence, and local and systemic complications were reviewed. Multivariate analysis identified the impact of extr

Is tracheostomy suitable for securing airway after facial firearm injuries?

BACKGROUND: Head and neck region have many vital structures, and facial firearm injuries (FFIs) more likely lead to life-threatening situations than other body injuies. These injuries have high potential of airway compromise associated with significant morbidity and mortality. METHODS: We describe an 11-year-old boy who had received tracheostomy after a FFI complicated with pneumothorax and subcutaneous emphysema 8 hours after the procedure. The patient was treated at the Department of Emergency and Critical Care, Gazi University School of Medicine, Turkey. RESULTS: The patient was discharge

Non-firearm weapon use and injury severity: priorities for prevention.

OBJECTIVES: To test the hypothesis that weapon-related violence (excluding firearms) results in more severe injury relative to the use of body parts (fists, feet and other body parts), and to rank order of injury severity by assault mechanism. DESIGN: Retrospective cohort study. PARTICIPANTS: 24,660 patients who were treated in a UK emergency department for violence-related injury. MAIN OUTCOME MEASURE: Score on the Manchester Triage Scale. RESULTS: The use of a weapon resulted in significantly more serious injury (adjusted odds ratio (AOR) 1.13, 95% confidence interval (CI) 1.00 to 1.28).

Backspatter on the firearm and hand in experimental close-range gunshots to the head.

The deposition of backspatter on the firearm or person shooting can greatly assist the reconstruction of shooting incidents. Backspatter was investigated in experimental transverse gunshots (9-mm Luger) to the heads of calves (n = 9) from shooting distances of 0 to 10 cm. The firearms were examined with a magnifying glass; the surgical gloves and the right sleeve worn by the person shooting were examined with a stereomicroscope. On the firearms, backspatter of blood was found in five of the nine cases, and one or both gloves showed bloodspatter deposits in six and the right sleeves in four cas

Trusted voices and tailored messaging: public health approaches to firearm safety for older adult firearm owners in the Washington, Wyoming, Alaska, Montana and Idaho (WWAMI) region.

BACKGROUND: Despite high rates of firearm suicide among older adults, little is known about their needs and preferences for information on safer firearm storage. This study examines information needs and communication preferences of older adult firearm owners with depression or cognitive impairment to inform public health firearm injury prevention strategies. METHODS: We conducted 20 semi-structured interviews with older adult firearm owners with depression or mild cognitive impairment in the Washington, Wyoming, Alaska, Montana and Idaho (WWAMI) region of the USA. We used a hybrid inductive-

Complex suicides involving the use of firearms and hanging: A retrospective study and review of the literature.

INTRODUCTION: Suicides are the second most common cause of non-natural death in Germany after accidents. Most common is death by hanging, followed by the use of firearms. More frequent "simple" suicides in which one suicide method is used are distinguished from rare "complex" suicides, in which several methods are applied, whereby the combination of gunshot and strangulation is frequently chosen. Such cases require a thorough criminalistic and forensic medical examination to check the plausibility of assumed sequences of events and to detect covered-up homicides. MATERIAL AND METHODS: A retro

Firearms, storage practices, and child blood lead levels and household dust lead loadings: findings from the HOME study.

BACKGROUND: Lead is a persistent environmental toxicant associated with adverse neurodevelopmental and behavioral outcomes, with children particularly vulnerable due to heightened exposure and biological susceptibility. Although population blood lead levels in U.S. children have declined substantially, a large proportion remain above levels associated with harm, indicating ongoing exposure from additional sources. Firearms use lead-based ammunition and primers and discharging a firearm releases lead particles that can be inhaled or transported into residential environments via take-home contam

A longitudinal ecological study of household firearm ownership and firearm-related deaths in the United States from 1999 through 2014: A specific focus on gender, race, and geographic variables.

Firearms have a longstanding tradition in the United States (US) and are viewed by many with iconic stature with regards to safety and personal freedom. Unfortunately, from a public health point of view, firearm-related deaths (FRDs) in the US have reached a crisis point with an estimated > 31,000 deaths and 74,000 nonfatal injuries resulting from firearms each year. This longitudinal ecological study analyzed variations in FRDs following firearm assaults (FAs) and law enforcement incidents involving a firearm (LEIF) in comparison to variations in household firearm ownership (HFO) among differ
