Cancer Incidence and Mortality among Firefighters: An Overview of Epidemiologic Systematic Reviews.

Firefighters are exposed to carcinogens that may increase their risk of developing many types of occupational cancer. Many systematic reviews (SRs) have been produced with sometimes conflicting conclusions. In this overview of reviews, we aim to assess the conclusion consistency across the available systematic reviews on the cancer risk in firefighters. Literature searches were conducted in several indexed databases and grey literature to retrieve systematic reviews aiming to evaluate cancer incidence or cancer mortality in firefighters. Results from included SRs were analyzed according to the

Occupational Exposure to Polycyclic Aromatic Hydrocarbons and Elevated Cancer Incidence in Firefighters.

Cancer incidence appears to be higher amongst firefighters compared to the general population. Given that many cancers have an environmental component, their occupational exposure to products of carbon combustion such as polycyclic aromatic hydrocarbons (PAHs) is of concern. This is the first UK study identifying firefighters exposure to PAH carcinogens. Wipe samples were collected from skin (jaw, neck, hands), personal protective equipment of firefighters, and work environment (offices, fire stations and engines) in two UK Fire and Rescue Service Stations. Levels of 16 US Environmental Protec

Systemic exposure to PAHs and benzene in firefighters suppressing controlled structure fires.

Turnout gear provides protection against dermal exposure to contaminants during firefighting; however, the level of protection is unknown. We explored the dermal contribution to the systemic dose of polycyclic aromatic hydrocarbons (PAHs) and other aromatic hydrocarbons in firefighters during suppression and overhaul of controlled structure burns. The study was organized into two rounds, three controlled burns per round, and five firefighters per burn. The firefighters wore new or laundered turnout gear tested before each burn to ensure lack of PAH contamination. To ensure that any increase in

Line-of-duty deaths among U.S. firefighters: an analysis of fatality investigations.

More than 100 firefighters die in the line-of-duty in the U.S. each year and over 80,000 are injured. This study examined all firefighter fatality investigations (N = 189) completed by the National Institute for Occupational Safety and Health (NIOSH) for fatalities occurring between 2004 and 2009. These investigations produced a total of 1167 recommendations for corrective actions. Thirty-five high frequency recommendations were derived from the total set: six related to medical fatalities and 29 to injury-related fatalities. These high frequency recommendations were mapped onto the major oper

Firefighters' occupational exposure: Contribution from biomarkers of effect to assess health risks.

Firefighting is physically and physiologically exhausting besides encompassing exposure to toxic fire emissions. Biomonitoring studies from the past five years have been significantly contributing to characterize the occupational-related health effects in this group of professionals and to improve risk assessment. Therefore, this study gathers and critically discusses the most characterized biomarkers of effect (oxidative stress, DNA and protein damage, stress hormones, inflammation, and vascular, lung, and liver injury), including those potentially more promising to be explored in future stud

Tinnitus and Self-Perceived Hearing Handicap in Firefighters: A Cross-Sectional Study.

Firefighters are susceptible to auditory dysfunction due to long-term exposure to noise from sirens, air horns, equipment, and tools used in forcible entry, ventilation, and extrication. In addition, they are exposed to ototoxic chemicals, particularly, during overhaul operations. Studies indicate that 40% of firefighters have hearing loss in the noise-sensitive frequencies of 4 and 6 kHz. Noise-induced hearing loss (NIHL) is often accompanied by tinnitus, which is characterized by ringing noise in the ears. The presence of phantom sounds can adversely affect the performance of firefighters. H

Development of role-related minimum cardiorespiratory fitness standards for firefighters and commanders.

A minimum cardiorespiratory fitness standard was derived for firefighters following a metabolic demands analysis. Design and minimal acceptable performance of generic firefighting task simulations (i.e. hose running, casualty evacuation, stair climb, equipment carry, wild-land fire) were endorsed by a panel of operationally experienced experts. Sixty-two UK firefighters completed these tasks wearing a standard protective firefighting ensemble while being monitored for peak steady-state metabolic demand and cardiovascular strain. Four tasks, endorsed as valid operational simulations by ≥90% of

Psychophysiological responses in experienced firefighters undertaking repeated self-contained breathing apparatus tasks.

In order to safely and effectively extinguish fires and rescue life, firefighters are required to routinely wear self-contained breathing apparatus (SCBA), yet little is known about the specific physiological and psychological demands associated with repeated exposure to tasks that require SCBA. A total of 12 experienced firefighters took part in a series of commonly encountered SCBA activities: free search, guideline search and live firefighting tasks under room temperature (∼20°C) and extreme heat (∼180°C) conditions to assess changes in heart rate, blood pressure, mood, perceived workload a

Wood smoke exposure induces a pulmonary and systemic inflammatory response in firefighters.

Epidemiological studies report an association between exposure to biomass smoke and cardiopulmonary morbidity. The mechanisms for this association are unclear. The aim of the present study was to characterise the acute pulmonary and systemic inflammatory effects of exposure to forest fire smoke. Seasonal forest firefighters (n = 52) were recruited before and/or after a day of fire-fighting. Exposure was assessed by questionnaires and measurement of carbon monoxide levels (used to estimate respirable particulate matter exposure). The pulmonary response was assessed by questionnaires, spirometry
