Approach to the Ankle in Adult Acquired Flatfoot Deformity.

Adult acquired flatfoot is a progressive deformity of the foot and ankle, which frequently becomes increasingly symptomatic. The posterior tibial tendon is most commonly associated with the deformity. A targeted physical examination with plain film radiographs is the recommended initial assessment, which will further guide a physician toward procuring more advanced imaging or toward surgical intervention. In this chapter the authors review the current literature of their approach to the treatment of the ankle in end stage of adult acquired flatfoot deformity.

Subtalar Arthroereisis for Surgical Treatment of Flexible Flatfoot.

Various surgical techniques are known for the treatment of flexible flatfoot in children after failure of nonsurgical attempts. Data collected in a review of the last 10-year period (2010-2020) show that among the 691 feet undergoing subtalar arthroereisis with endorthesis, average age at surgery was 11.40 years and in the 1856 feet that underwent subtalar arthroereisis with calcaneo-stop 11.69 years, while the complications rate was 9.00% and 6.38%, respectively. These data confirm that subtalar arthroereisis with calcaneo-stop may have an advantage over subtalar arthroereisis with endorthesi

Biomechanics and Orthotic Treatment of the Adult Acquired Flatfoot.

The adult acquired flatfoot deformity resulting from posterior tibial tendon dysfunction is the result of rupture of the posterior tibial tendon as well as key ligaments of the ankle and hindfoot. Kinematic studies have verified certain levels of deformity causing hindfoot eversion, lowering of the medial longitudinal arch and forefoot abduction. The condition is progressive and left untreated will cause significant disability. Bracing with ankle-foot orthoses has shown promising results in arresting progression of deformity and avoiding debilitating surgery. Various types of ankle-foot orthos

Naviculocuneiform Arthrodesis in Adult Flatfoot: A Case Series.

Medial column arthrodesis and calcaneal osteotomies are commonly used for adult-acquired flatfoot surgical reconstruction. In this case series, 10 patients (11 feet) with a mean age of 54 ± 13 years underwent a medial column arthrodesis, with or without calcaneal osteotomy, between 2010 and 2017. The indication for surgery was a painful flatfoot deformity with peritalar subluxation and a fault in the naviculocuneiform joint. At a mean of 9.9 (range 2.5 to 33.1) months after surgery, in patients who underwent a medial column arthrodesis, radiographs showed a mean decrease in the talonavicular c

Bioabsorbable implants for subtalar arthroereisis in pediatric flatfoot.

Flatfoot is a common condition in growing-age patients. Despite its common presentation, nowadays surgical indications and treatments are still debated. Arthroereisis is a widely used technique, and several implants designs have been proposed over time. Despite the good results shown in the literature, the main drawback of these techniques has always been the need for a second surgery for implant removal. Bioabsorbable devices have been introduced to overcome this necessity.Correct approach to the patient, indications and contraindications and available studies on bioabsorbable implants for su

Surgical treatment results for flexible flatfoot in adolescents.

OBJECTIVE: Idiopathic flexible pes planus (IFPP) is a common foot problem in adolescents and young adults. Hypothesis for the present study was that combination of procedures for IFPP can achieve results in adolescents and young adults that are as good as those seen in adult-acquired pes planovalgus (AAPP) treatment in adults. METHODS: A total of 21 feet of 18 patients (10 boys, 8 girls) with mean age of 15.6 years underwent surgical reconstruction for flatfoot deformity. Symptomatic patients who had been unresponsive to conservative treatment were included in study group. Mean follow-up time

Surgical treatment of the adult acquired flexible flatfoot.

In this review article, the authors give an overview of the currently available soft tissue and bony procedures in the treatment of the adult acquired flexible flatfoot. Instead of starting from the classification for posterior tibial tendon dysfunction, described by Johnson and Storm, the authors address the flatfoot from a more anatomical point of view. Based on this, they will try to define a treatment algorithm.

Approach and treatment of the adult acquired flatfoot deformity.

Adult acquired flatfoot deformity (AAFD), embraces a wide spectrum of deformities. AAFD is a complex pathology consisting both of posterior tibial tendon insufficiency and failure of the capsular and ligamentous structures of the foot. Each patient presents with characteristic deformities across the involved joints, requiring individualized treatment. Early stages may respond well to aggressive conservative management, yet more severe AAFD necessitates prompt surgical therapy to halt the progression of the disease to stages requiring more complex procedures. We present the most current diagnos

The management of complications following the treatment of flatfoot deformity.

Adult acquired flatfoot deformity encompasses a wide spectrum of clinical conditions. Current management approaches have emerged from a growing understanding of its manifestations, which have been learned from decades of clinical trial and error. Although surgical trends continue to evolve, many basic principles and practices have endured. Adult flatfoot deformity can arise from multiple causes, the most common of which remains posterior tibial tendon rupture with subsequent elongation of secondary supportive structures. Regardless of the cause, the fundamental goals of surgical management inc

Footprint analysis of flatfoot in preschool-aged children.

Our aim in this study was to analyze the footprint measurements of flatfoot in a population of preschool-aged children. Three footprint measurements, the Clarke's angle (CA), Chippaux-Smirak index (CSI), and Staheli arch index (AI), were used for comparison with clinical diagnosis. A total of 2,638 static footprints of children aged from 3 to 6 years were recorded. The clinical diagnosis as a gold standard compared with the results of the CA, CSI, and AI and displayed in a receiver operating characteristic (ROC) curve. In order to illustrate the diagnostic accuracy in clinical settings, their

Medial column procedures in the acquired flatfoot deformity.

This article focuses on the medial column procedures for the treatment of the acquired flatfoot. When performing a medial column procedure alone, in combination with soft tissue reconstruction, or with concomitant lateral bony procedure, the deformity being addressed should include significant abduction or collapse through the talonavicular (TN), naviculocuneiform (NC), or tarsometatarsal (TMT) joints. The procedures covered in this article include arthrodesis of the TN, NC, and metatarsocuneiform (MTC) joints, together or in isloation. Osteotomies of the cuneiforms will also be addressed.

Biomechanics and clinical analysis of the adult acquired flatfoot.

The adult acquired flatfoot is a deformity that results from the loss of dynamic and static supportive structures of the medial longitudinal arch. The severity of the deformity is dependent upon the role of ligamentous disruption on the hindfoot that can be determined by careful clinical examination. Treatment of the adult flatfoot requires an understanding of the biomechanical effects of deforming forces, tendon dysfunction, ligament disruption, and joint sublaxation.

The surgical management of equinus in the adult acquired flatfoot.

Ankle joint equinus plays a significant role in the pathogenesis of adult and pediatric flatfoot. The surgical management of ankle equinus is a widely debated topic, and procedure selection is often based on surgeon preference because there is no consensus regarding the superiority of a single procedure. Gastrocnemius recession offers acceptable cosmesis and minimizes perceived weakness, yet requires increased operating time and is indicated only in mild to moderate contractures. Whereas tendo-achilles lengthening is efficient and technically undemanding, and one may achieve large amounts of r

Triple arthrodesis for the adult-acquired flatfoot deformity.

Triple arthrodesis is often the procedure of choice for end-stage adult-acquired flatfoot. The benefits of triple arthrodesis include resolution of symptoms, hindfoot realignment, and stability. This article reviews the operative technique, realignment considerations, and postoperative management when performing triple arthrodesis. When taken into account, these recommendations provide a favorable outcome for both the patient and the surgeon.

Current concepts for treatment of the painful flatfoot in the elderly.

Flatfoot deformities are predominantly asymptomatic variants of normal, which do not adversely affect function. However, the acquired flatfoot deformity secondary to attrition of the posterior tibial tendon is a progressively painful condition, ultimately affecting the hindfoot, midfoot, and forefoot. It is most common during the sixth and seventh decades of life. Chronic micro-trauma to the tendon causes eventual rupture and the resulting deformity. Prior to tendon rupture, early symptoms are those of a unilateral localized inflammatory condition with tenderness and swelling behind the medial

Experimental flatfoot model: the contribution of dynamic loading.

The goal of this study was to determine if the application of muscle forces (simulating the dynamic phase of the midstance part of gait) had an effect on flatfoot deformity. We created a flatfoot model in each of seven cadaver foot specimens by grasping the Achilles, peroneus longus, peroneus brevis, flexor digitorum longus, and flexor hallucis longus tendons with soft-tissue vice clamps connected via wire cables to pneumatic cylinders. The experiment included four stages: 1) initial static axial loading; 2) axial loading after 3,000 load cycles (average, 735 N; range, 70 to 1400 N); 3) axial

Biomechanics of procedures used in adult flatfoot deformity.

A flatfoot deformity can occur secondary to fairly obvious causes, or more subtle and less definable entities. Complicating the situation further, it is likely that the cause of an acquired flatfoot deformity in an adult is multifactorial. This likelihood makes the definition, diagnosis, and appropriate treatment of this condition a daunting task. More research is needed to define further the biomechanics of the foot and to understand the significance of the forces that combine to create flatfoot deformity.

Complications of surgical treatments for adult flatfoot deformities.

Numerous surgical procedures have been proposed for reconstructing and correcting adult flatfoot deformity. Most procedures include lengthening of the tight Achilles tendon and transferring the FDL tendon to substitute for a dysfunctional PTT. These procedures commonly are combined with a bone procedure to stabilize the correction and correct the pathologic anatomy. All of these procedures have the potential for a range of complications, many of them serious. In general, the clinician should perform the least invasive procedure that decreases pain and improves function. The effects of each pro
