Effects of exercise training on sleep apnoea in patients with coronary artery disease: a randomised trial.

Overnight fluid shift from the legs to the neck and lungs may contribute to the pathogenesis of obstructive sleep apnoea (OSA) and central sleep apnoea (CSA). We hypothesised that exercise training will decrease the severity of OSA and CSA in patients with coronary artery disease (CAD) by decreasing daytime leg fluid accumulation and overnight rostral fluid shift.Patients with CAD and OSA or CSA (apnoea-hypopnoea index >15 events per h) were randomised to 4 weeks of aerobic exercise training or to a control group. Polysomnography, with measurement of leg, thoracic and neck fluid volumes and up

Otorhinolaryngological problems in medical support of space flights.

At present the main trends among the most important problems of otorhinolaryngology in space medicine have become defined as vestibulology, audiology and clinical aspects (prophylaxis, diagnosis and treatment of ENT diseases in flight). The principal result of recent vestibular studies has probably been the establishment of an apparent relationship between the resistance of the vestibular system to adequate ground-based stimulation and tolerance to space flight. The findings of the studies formed the basis for the development of a new system of vestibular selection, as well as demonstrated the

Neonatal Hypernatremic Dehydration.

Newborn infants experience significant fluid shifts following delivery as a result of the contraction of extracellular water. Without prompt identification and treatment, significant weight loss can result in neonatal hypernatremic dehydration. This case study presents an infant with significant weight loss, dehydration, and electrolyte imbalances. The differential diagnosis for hypernatremia is presented along with clinical presentation, treatment, and discharge education recommendations.

Postural changes in intracranial pressure in chronically shunted patients.

A subset of hydrocephalic patients with indwelling shunts become symptomatic when they are upright and active. Intracranial pressure (ICP) measurements in these patients have shown a significant drop in pressure when the patient is upright with return to normal levels when the patient is supine. In 20 chronically shunted hydrocephalic patients who previously had no siphon protection devices, ICP changes in supine and upright position were studied at the time when the patient had external ventriculostomy for treatment of shunt infection. Our hypothesis was that these patients might display rapi

Effect of below-the-knee compression stockings on severity of obstructive sleep apnea.

BACKGROUND: Overnight fluid shift from the legs to the neck may narrow the upper airway and contribute to obstructive sleep apnea (OSA) pathogenesis. We hypothesized that below-the-knee compression stockings will decrease OSA severity in a general OSA population by decreasing daytime leg fluid accumulation and overnight fluid shift and increasing upper-airway size. METHODS: Patients with OSA (apnea-hypopnea index ≥ 10) were randomized to wear compression stockings during the daytime or to a control group for 2 weeks. Overnight polysomnography with measurement of leg and neck fluid volumes and

Physiopathology and Diagnosis of Congestive Heart Failure: Consolidated Certainties and New Perspectives.

Volume overload and fluid congestion are a fundamental issue in the assessment and management of patients with heart failure (HF). Recent studies have found that in acute decompensated heart failure (ADHF), right and left-sided pressures generally start to increase before any notable weight changes take place preceding an admission. ADHF may be a problem of volume redistribution among different vascular compartments instead of, or in addition to, fluid shift from the interstitial compartment. Thus, identifying heterogeneity of volume overload would allow guidance of tailored therapy. A compreh

Non-exudative fluid in age-related macular degeneration: Imaging, pathophysiology, and clinical implications.

Non-exudative retinal fluid in age-related macular degeneration (AMD) challenges the long-standing paradigm that any fluid on structural optical coherence tomography necessarily reflects neovascular exudation. Advances in multimodal imaging have revealed that both intraretinal fluid and subretinal fluid (SRF) may arise from non-neovascular mechanisms across intermediate AMD and geographic atrophy (GA). Non-exudative IRF encompasses 3 distinct processes: (1) degenerative mechanisms occurring in progressive atrophy, (2) "burnt-out" contraction with cavitation overlying chronic fibrotic scars, an

[Parenteral fluid therapy in the pediatric surgical patient].

Perioperative fluid therapy aims to provide water, electrolytes and calorie to maintain metabolic homeostasis. The landmark article in which Holliday and Segar proposed the rate and composition of parenteral maintenance fluids for hospitalized children is used to the fluid management for the pediatric surgical patient. Critically ill patients require meticulous intraoperative fluid management to avoid acidosis, hypoglycemia, hyponatremia, and hypocalcemia as well as to maintain adequate intravascular volume. The glucose, electrolyte, and intravascular volume requirements of the pediatric surgi

Experience with a low volume ultrafiltration cell in small children*.

A low volume (16 ml) ultrafiltration cell was used ten times in two small, fluid overloaded children to remove plasma water. The device was simple to use and, at slow blood flow rates (25-50 ml/minutes) and low transmembrane pressures (10-30 mm Hg), provided controlled removal of excess fluid. Although no major complications were encountered, hypothermia and hypotension (at ultrafiltrate flux rates exceeding 0.5 ml/kg/minute) were observed. The ultrafiltrate solute concentration was similar to plasma and no significant shifts in serum electrolytes were induced. The ultrafiltrate protein concen

Radiographic signs in post pneumonectomy bronchopleural fistula.

Four instances of bronchopleural fistula (BPF) occurred in three patients following right pneumonectomy. In all, a striking increase in air content and fall in the fluid level of the pleural space provided to be diagnostic. The radiologic diagnosis of BPF requires monitoring of the air-fluid level in the pneumonectomy space, using a horizontal beam to demonstrate the changing volume of air and fluid regardless of the patient's position. One patient presented with a tension pyopneumothorax without good clinical evidence of BPF and required prompt chest tube drainage for the relief of severe dys

The escape of retrobulbar cerebrospinal fluid in the astronaut's eye: mission impossible?

Ophthalmic abnormalities including unilateral and bilateral optic disc edema, optic nerve sheath distention, globe flattening, choroidal folds, and hyperopic shifts have been observed in astronauts during and after long-duration spaceflight. An increased understanding of factors contributing to this syndrome, termed spaceflight-associated neuro-ocular syndrome, is currently a top priority for the ESA and NASA, especially since this medical obstacle could impact the visual health of astronauts as well as the success of future missions, including continued trips to the International Space Statio

Effect of hemodialysis on uterine and umbilical artery Doppler flow velocity waveforms.

A case of a pregnant woman with membranous glomerulonephropathy requiring hemodialysis is described. During hemodialysis the patient underwent Doppler flow studies of the uterine and umbilical arteries. In spite of avoidance of hypotension and major fluid shifts, hemodialysis was accompanied by a significant increase in the systolic/diastolic ratio of the proximal uterine artery. There was no significant change in the umbilical artery systolic/diastolic ratio.

A Pilot Study Beyond Ovarian Reserve: How AMH Levels Shape the Proteomic and Metabolic Landscape of the Follicle.

PURPOSE: The follicular fluid (FF) microenvironment critically influences oocyte development, with Anti-Müllerian Hormone (AMH) serving as a primary marker of ovarian reserve. This exploratory pilot study investigates the biochemical and proteomic shifts across the AMH spectrum to provide initial insights into potential targeted intrafollicular strategies. METHODS: FF samples from 49 patients undergoing assisted reproduction were grouped into Low-, Normal-, and High-AMH groups. Oxidative stress, antioxidant capacity, and metabolic indicators (lactate, adiponectin) were analyzed. Proteomic pro

Metabolic shifts, a consequence of hyperosmolarity, are a hallmark of mental disorders.

Mental and neurodevelopmental disorders are heterogeneous, complex, and overlapping entities. Despite progress, their neurobiological underpinnings are not well understood, and current treatments have limited efficacy. However, a growing number of studies have shown impaired brain and systemic energy metabolism evidenced by low-grade inflammation, metabolic syndrome, mitochondrial dysfunction, and abnormal glucose utilization, although their underlying mechanisms remain poorly understood. This paper reviews metabolic shifts in mental disorders, examines the underlying mechanisms driving these

Permissive Hypotension in a Patient with Severe Hypernatremia: A Case Report.

INTRODUCTION: Severe hypernatremia is a critical situation, and when coupled with intravascular depletion and hypotension can create a treatment dilemma. CASE REPORT: We present the case of a medically complex patient who had gradually worsening alteration of mental status and mean arterial pressures in the 50s on presentation to the emergency department. CONCLUSION: Final diagnoses included severe hypernatremia and hypovolemic shock secondary to poor oral intake. We used judicious fluid repletion with gradual improvement in sodium levels and permissive hypotension to avoid rapid osmotic shi

Mediastinal position and air-fluid height after pneumonectomy: the effect of the respiratory cycle.

After pneumonectomy, a shift of the mediastinum toward the nonoperated side may be the initial indicator of a postoperative problem such as empyema, bronchopleural fistula, esophageal-pleural fistula, hemothorax, or recurrent tumor. Postoperatively, a drop in the fluid level on the operated side may indicate a bronchopleural fistula or other fistulous connection. To assess the effect of the respiratory cycle on these two signs, 16 patients were studied with inspiration-expiration films. In every case the mediastinum shifted toward the normal side in expiration, and in no case did a fluid level

Relationship of pharyngeal water content and jugular volume with severity of obstructive sleep apnea in renal failure.

BACKGROUND: In patients with end-stage renal disease (ESRD), fluid overload may contribute to their high prevalence of obstructive sleep apnea (OSA) by increasing the amount of fluid displaced from the legs into the neck overnight, and possibly compressing the upper airway (UA). Indeed, in ESRD patients, the amount of overnight rostral fluid displacement from the legs is related to the frequency of apneas and hypopneas per hour of sleep (apnea-hypopnea index, AHI). We, therefore, hypothesized that in ESRD patients, the greater the UA-mucosal water content (UA-MWC) and internal jugular vein vol

Reduced fluid gain during cardiopulmonary bypass in piglets using a continuous infusion of a hyperosmolar/hyperoncotic solution.

BACKGROUND: The aim of this study was to evaluate how a continuous infusion of a hyperosmolar/hyperoncotic solution influences fluid shifts and intracranial pressure during cardiopulmonary bypass in piglets. METHODS: Fourteen animals, randomized to the control (CT) group or the hypertonic saline/hydroxyethyl starch (HyperHaes) (HSH) group, received acetated Ringer's solution as prime and supplemental fluid. The HSH group received, in addition, HyperHaes 1 ml/kg/h. After 1 h of normothermic cardiopulmonary bypass, hypothermic cardiopulmonary bypass (28 degrees C) was initiated and continued fo

Microbial diurnal rhythmicity in the rumen fluid impacted by feeding regimes and exogenous microbiome providing novel mechanisms regulating dynamics of the rumen microbiome.

BACKGROUND: Diurnal oscillations have been reported on ruminal prokaryotes, but the daily rhythmicity of eukaryotes remains unknown. This study investigated diurnal oscillations of ruminal prokaryotes and eukaryotes under three different feeding managements and rumen fluid transplantation conditions, aiming to elucidate the regulatory mechanisms influencing the dynamic shifts of rumen microbiome through the daily feeding cycle. RESULTS: Quantification and profiling of the microbiota of 288 rumen samples collected from lactating dairy cows (n = 12) every 6-h over 48-h feeding cycles under ad l

Pre- and postoperative fluid management in infancy.

The fluid and electrolyte management of the infant either before or following surgery is not difficult if the several principles are carefully followed: (1) Fluid requirements include maintenance therapy, correction of ongoing losses, and replacement of deficit losses. (2) Calculation for fluid requirements in the postoperative period will include maintenance therapy, correction of ongoing losses, and provision of fluid lost by internal shifts. (3) Maintenance needs for fluid in infants equals 100 to 120 mL/kg per 24 hours, and Na+ at 3 mEq/kg per 24 hours and K+ at 2 to 3 mEq/kg per 24 hours
