Heparin and coumarin derivatives have long been used for the prophylaxis and treatment of venous thromboembolism (VTE). Although they have demonstrated efficacy and safety, they act at multiple targets within the coagulation cascade and their efficacy is influenced by many patient variables. Because of the need to improve the benefit-to-risk ratio of antithrombotic drugs, newer agents that target single coagulation factors have been developed. These include selective factor Xa inhibitors, direct thrombin inhibitors (DTIs), and inhibitors of factor IXa and the factor VII-tissue factor complex.

Venous thromboembolism (VTE) is frequently encountered in cancer patients, acts as an important cause of morbidity and mortality, and may be a predictor of worse prognosis. In cancer patient who have a poor life expectancy, preventing death from pulmonary embolism is the mainstay of treatment. Patients who present with severe hypotension or other clinical manifestations suggestive of critical pulmonary embolism and do not have contraindications to thrombolysis should promptly be administered thrombolytic drugs. Except for selected cases requiring aggressive therapy, treatment of VTE in patient

ABSTRACT: The use of oral fibrinolytic agent (DLBS1033) has been proven for adjuvant treatment in venous thromboembolism, however until now there is no published report about its uses and effectiveness as an addition to the standard therapy of severe COVID-19 cases and hypercoagulopathy. We present two cases of severe confirmed COVID-19 from PCR tests, seen at Ngimbang Hospital, Lamongan, East Java in October and November, 2020. The first patient was a 51-year-old male who presented to ER with fever, dyspnoea, cough, and oxygen desaturation (SpO2 room air 87%), with comorbids of pulmonary hype

Heparin-induced thrombocytopenia is a rare complication of treatment with both unfractionated heparin (UFH) and low molecular weight heparin (LMWH). Antibodies against the complex heparin-platelet factor 4 are the main cause of pathogenesis, resulting in the activation of thrombocytes, coagulation, endothelium, monocytes, neutrophils and subsequent highly prothrombotic state. The prothrombotic state can result not only in venous but also in arterial thrombosis at different locations (which is manifested apart from venous thromboembolic disease also by acute limb ischemia, acute myocardial infa

Immediate type hypersensitivity reactions due to heparins are rare, and the exact immunologic pathomechanism has not been identified so far. In our 2 case reports, we describe first a 50-year-old female who received dalteparin (Fragmin®) and developed signs of an immediate type hypersensitivity reaction. The personal history revealed a previous application of dalteparin (Fragmin®). Evaluation with a skin prick test showed positive results for dalteparin. The second case deals with a 73-year-old female with a suspected immediate type reaction after the administration of dalteparin (Fragmin®). A

Heparin-based anticoagulants have been widely used for the prevention and treatment of venous thrombotic diseases, as well as for anticoagulation during cardiopulmonary bypass and hemodialysis. However, excessive heparin usage brings serious bleeding risk, necessitating immediate reversal of their anticoagulant activity. Additionally, to prevent bleeding during surgery and restore hemostatic function post-cardiopulmonary bypass and hemodialysis, it is also crucial to reverse heparin's anticoagulant effects. Currently, protamine sulfate (PS) is the only clinically approved antidote for heparin.

Antiphospholipid syndrome (APS) is characterized by arterial and/or venous thrombosis with antiphospholipid antibodies. Dysregulation of the complement pathway has been implicated in APS pathophysiology. We report the successful use of eculizumab, an anti-C5 monoclonal antibody, in controlling and preventing recurrent thrombosis in a refractory case of APS. An 18-year-old female was diagnosed with APS after developing extensive, unprovoked deep vein thrombosis (DVT) of axillary, inferior vena cava, and brachiocephalic veins. Thrombophilia evaluation revealed triple-positive lupus anticoagulant

We present important laboratory testing and clinical management strategies used to safely discharge home a 69-year old woman with heparin-induced thrombocytopenia (HIT) from the hospital. She was admitted for a coronary artery bypass graft procedure for which she was anticoagulated with heparin. Shortly after the procedure she developed thrombocytopenia and was diagnosed with HIT using the 4Ts scoring system, a latex-enhanced immunoassay (LEI) screen and confirmatory serotonin release assay. Her anticoagulation was switched from heparin to argatroban, and response to treatment was monitored in

PURPOSE: To describe the management challenges of bilateral adrenal hemorrhage (BAH) in a patient on apixaban with recent heparin-induced thrombocytopenia (HIT) and active deep vein thrombosis (DVT).
SUMMARY: A 71-year-old woman with a history of orthopedic trauma developed HIT during hospitalization and was transitioned to fondaparinux. She was later discharged on apixaban for treatment of a provoked left common femoral DVT. Within 48 hours, she presented with shock initially attributed to sepsis. Computed tomography revealed BAH, raising concern for adrenal crisis. Reversal of apixaban was

BACKGROUND: Heparins are a widely used class of drugs known to cause delayed-type hypersensitivity (DTH) reactions. Recent publications indicate that the incidence of these may be higher than previously thought. To date, patient-related but no drug-related risk factors for the development of DTH reactions to heparins have been identified, although molecular weight is discussed as a potentially relevant parameter.
OBJECTIVES: To address this, a systematic review was conducted on the frequency of cross-reactions after DTH reactions to heparin preparations.
METHODS: We electronically searched M

The use of antithrombotic drugs for the prevention of venous thromboembolism (VTE) in patients undergoing surgery is presently based on solid principles and high-level scientific evidence. This article reviews current strategies of pharmacological thromboprophylaxis. The level of VTE risk following surgery depends on a variety of factors that the surgeon should take into account, including the type of surgery and the presence of additional risk factors, such as elderly age and cancer. In patients undergoing minor general surgery, early mobilization is sufficient as prophylaxis, whereas in thos

INTRODUCTION: The risk of venous thromboembolism varies according to the histological type of cancer. The failure of antithrombotic treatment is more frequent in cancer patients as compared to non-cancer ones.
AIM: We aimed to elucidate the mechanism of activation of blood coagulation induced by cancer, the impact of chemo-resistance phenotype on the capacity of cancer cells to trigger thrombin generation and the alterations of the efficiency of LMWHs and the specific inhibitors of factor Xa (fondaparinux and apixaban) in the presence of cancer cells.
MATERIALS AND METHODS: Thrombin generati

In the event of a deep vein thrombosis, the major concern is to prevent the subsequent development of an embolism or a recurrence. The therapeutic time gap to the development of the full effect of the vitamin K antagonists is bridged in the initial acute treatment phase with low molecular weight heparin (LMWH) or fondaparinux. In patients in whom vitamin K antagonists are contraindicated, or in pregnancy, secondary prophylaxis is alternatively continued with dose-adjusted LMWH. Compression therapy serves merely to prevent a post-thrombotic syndrome.

Selective inhibitors of specific coagulation factors represent a new class of antithrombotic agents, designed to overcome the limitations of traditional anticoagulants. Available clinical studies indicate that the most promising new anticoagulants are those selectively targeting factor Xa and thrombin. Typically, the standard steps for clinical evaluation of new anticoagulants are thromboprophylaxis in high risk orthopedic surgery, followed by treatment of established venous thromboembolism, nonvalvular atrial fibrillation and acute coronary syndromes. These agents - that have the potential to

Direct oral anticoagulants (DOAC), including dabigatran, apixaban, and rivaroxaban, are increasingly used in adults for painful venous malformations (VMs) instead of subcutaneous drugs such as low molecular weight heparins or fondaparinux. However, these medications are poorly studied in children. In seven children with painful VMs and chronic intravascular consumptive coagulopathy, DOACs allowed for successful pain control and improvement in coagulopathy, with no adverse effects. DOACs appear to be an effective alternative in childhood VM associated with intravascular coagulopathy.

BACKGROUND: The aim of this study was to perform a population-level analysis on the effect of different types of anticoagulation on postoperative stiffness after total knee replacement, requiring manipulation under anesthesia. We hypothesized that patients receiving warfarin would have a higher rate of manipulation under anesthesia compared with patients receiving low-molecular-weight heparin. We also hypothesized that aspirin, direct factor Xa inhibitors, and fondaparinux would have no effect on the rate of manipulation under anesthesia.
METHODS: Using the PearlDiver patient database, we ana

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is a transient, antibody-mediated thrombocytopenia syndrome that usually follows exposure to unfractioned heparin (UFH) or low-molecular-weight heparin (LMWH). In contrast to other pathological conditions which lead to thrombocytopenia and bleeding complications, HIT results in a paradoxical prothrombotic state. It is caused by antibodies directed to complexes containing UFH or LMWH and a self-platelet protein: the platelet factor 4 (PF4). The heparin-PF4 immune complex leads to activation of platelets, monocytes, and endothelial cells which r
