Utility of food allergy thresholds.

Food allergy is a prevalent disease worldwide that is a significant quality-of-life burden, and accidental exposures to food allergens may elicit severe, life-threatening reactions such as anaphylaxis. The threshold level, or the dose that triggers an allergic reaction determined by oral food challenges, varies considerably among individuals suffering from food allergies. Moreover, IgE concentration, diversity, or function can only partially explain this variation in threshold; pathogenic effector TH2 cells have also been found to contribute to the eliciting dose. Though very sensitive to cofa

Cow's milk allergic patients should be informed of the sources of caseinate.

Cow's milk allergy is one of the most common food allergies prevalent during infancy and it may cause anaphylactic reactions in more sensitive patients. Cow's milk is found in a large variety of processed foods but its presence may be hidden as it is occasionally listed on the product label in an manner unrecognizable to the consumer. The parents of more sensitive patients need to be instructed in the detection of these potential sources of hidden cow's milk by judiciously reading food labels and in recognition of the sources, thus avoiding possible untoward reactions. We report a patient with

Pathological IgE Production in Inborn Errors of Immunity and Beyond.

In this review, we examine different mechanisms for the generation of pathological IgE through the study of inborn errors of immunity (IEI). In particular, we focus on IEIs that are susceptible to atopic disease, such as severe eczema, eosinophilia, and/or food anaphylaxis; this group of approximately 50 genes has been coined primary atopic disorders (PADs). PADs link specific genes and pathways to atopic disease and the production of IgE and can provide insights into more common allergies experienced by the general population.

[Differential diagnosis in food allergy].

It is important to establish the differential diagnosis of food allergy with other disorders, for example: toxic reactions that occur in any person exposed to a sufficient amount of some allergen, and non-toxic reactions that depend on individual susceptibility (food allergy or intolerance). The differential diagnosis is decisive to establish the appropriate treatment. Food intolerance involves adverse reactions to foods without any immunological response involved, and commonly manifests with gastrointestinal symptoms (malaise, abdominal pain or diarrhea). Food allergy is an exaggerated reacti

Lupine allergens: Clinical relevance, molecular characterization, cross-reactivity, and detection strategies.

Lupine is commonly utilized as a technological food and ingredient in a great variety of processed products (snacks, bakery, meat, and dairy products) principally owing to its nutritional value and technological properties. However, its ingestion, even at trace amounts (in the range of mg protein per kg of food), can lead to severe adverse reactions in allergic individuals. Lupine belongs to the Leguminosae family, having the conglutins (α-, β-, δ-, and γ-) as allergens, among other proteins. Cross-sensitization of lupine-sensitized individuals with other legume species, mainly peanut, can occ

[Detection of IgE specific for egg yolk by enzyme immunoassay. Description of a case].

Food allergy is a common disease in our country, especially affecting atopic children. Egg-white hypersensitivity is frequently found in these patients. However, egg-yolk hypersensitivity is not usually reported in patients with egg allergy. This article describes a young patient with egg-yolk hypersensitivity, a 12 year old female patient with a medical history of contact urticaria, angioedema and severe acute bronchospasm shortly after the intake of small amounts of egg-yolk. All these episodes required treatment in emergency care units because of the severity of the symptoms. The patient di

The Expanding Role of Omalizumab: From Food Allergy to Drug Desensitization.

Although omalizumab is currently approved for a limited number of indications-such as asthma, chronic rhinosinusitis with nasal polyps, and chronic spontaneous urticaria-its potential applications are expanding each year. Owing to its diverse and not yet fully elucidated mechanism of action, including effects on both IgE- and non-IgE-mediated hypersensitivity reactions with delayed onset, this monoclonal antibody may be beneficial in a wide range of allergic and non-allergic conditions. To date, numerous clinical trials and case reports have documented the successful off-label use of omalizuma

[The initiating role of several respiratory and skin contaminants in food allergies].

The authors present data which suggest that mustard hypersensitivity can result from Isocyanates induced occupational asthma. Isothiocyanates present in mustard spice is probably responsible for this cross-sensitivity. Frequently, food allergies result from skin or respiratory primary contact and sensitization. Some examples are proposed. Distinctive structures and functions of immunological system in cutaneous, respiratory and digestive apparatus, give an explanation for the predominance of skin and respiratory immunological systems to promote hypersensitivity. For the prevention of food alle

Examination of patients suspected as having hypersensitivity to iodinated contrast media with leukocyte migration test.

In vivo tests may be used for the diagnosis of allergy to iodinated contrast media (ICM); however, the tests do not provide definitive diagnosis and are associated with risks for patients. Diagnoses based on in vitro tests are limited, and there are almost no relevant studies. Herein, the authors examined involvement of allergic reaction from a multilateral standpoint in 39 patients suspected of having ICM allergies using leukocyte migration test (LMT). The positive rate of LMT was 44%. A comparison with the positive rate of LMT in drugs other than ICM (74%) indicated 30% difference, which was

The role of natural color additives in food allergy.

A critical evaluation of the available information demonstrates that reactions to natural color additives are rare. Studies of turmeric and carotenoid pigments administered in mixtures with other food colorings failed to definitely identify reactions to either color additive. For carotenoids, the one case report of an adverse reaction was not conclusive. An anaphylactic reaction to saffron does suggest an IgE-mediated reaction, but the high use of saffron as compared with this single report of an adverse reaction suggests that sensitivity to saffron is extremely rare. Numerous reports of react

Food-associated gastrointestinal disease.

The ingestion of food is frequently associated with specific adverse gastrointestinal symptoms, including emesis, diarrhea, abdominal cramping, and gastrointestinal bleeding. This brief review discusses a variety of food-induced gastrointestinal diseases and will highlight the recent information pertaining to the pathophysiology, clinical presentation, diagnostic approach, and treatment of these disorders. Reactions resulting from immediate hypersensitivity and cell-mediated processes will be reviewed in detail, as well as other proposed pathophysiologic mechanisms. The clinical presentation a

[Adverse reactions induced by food additives: sulfites].

Many chemicals are used to preserve, color and flavor foods and drugs. There have been numerous reports of adverse reactions, including urticaria, angioneurotic edema, asthma an anaphylaxis following the ingestion of food additives such as tartrazine, monosodium glutamate and benzoic acid. Recently the food and drug additives reaching medical awareness as a cause of sensitivity are the sulfiting agents. Sulfites are widely used in the food and beverage industry as preservatives and antioxidants. They are also used by the pharmaceutical industry. This work describes the common uses of sulfiting

Will Oral Food Challenges Still Be Part of Allergy Care in 10 Years' Time?

Oral food challenges (OFCs) are currently the definitive diagnostic procedure in food allergy. Their design has evolved over the decades to maximize safety, optimize convenience, and address several specific clinical questions. However, they are a resource-intensive investigation that carry a risk for severe allergic reaction in which fatal outcomes, although rare, have been reported. In this review, we explore the many roles that OFC fulfil in the clinical and research settings. We also discuss progress that has been made in developing alternative diagnostic tools and how far these have reach

Models and Tools for Investigating Eosinophilic Esophagitis at the Bench.

Eosinophilic esophagitis (EoE) is an increasingly common food allergy disease of the esophagus that received its medical designation code in 2008. Despite this recency, great strides have been made in the understanding of EoE pathophysiology and type 2 immunity through basic and translational scientific investigations conducted at the bench. These advances have been critical to our understanding of disease mechanisms and generating new hypotheses, however, there currently is only one very recently approved FDA-approved therapy for EoE, leaving a great deal to be uncovered for patients with thi

Adverse reactions to food additives.

There are thousands of additives used by the food industry for a variety of purposes in the foods we eat. However, only a small number have been implicated in causing adverse reactions in humans. Although there are reported cases of individuals who have reactions to single additives, most of the medical literature involves patients with asthma or chronic idiopathic urticaria/angioedema whose conditions are exacerbated after ingestion of food additives. Many of these reports are characterized by poorly controlled challenge procedures. Recent studies performed under properly controlled condition

Risk factors of adverse reactions to food in German adults.

BACKGROUND: Allergic diseases have been increasing during the last decades. Previous studies suggest an impact of a variety of risk factors on the prevalence of food hypersensitivity. OBJECTIVE: Data of a cross-sectional population-based survey were analysed to study the prevalence of food hypersensitivity in females and males adjusted for age and education. METHODS: A population aged 18-79 years from a representative, randomly sampled survey studying 13,300 inhabitants from Germany (Berlin) was analysed. The Berlin study data were weighted with regard to age, sex, education and allergic dis
