Treatment of close-range, low-velocity gunshot fractures of tibia and femur diaphysis with consecutive compression-distraction technique: a report of 11 cases.

Lower extremity injuries secondary to close-range, low-velocity gunshot wounds are frequently seen in both civilian and military populations. A close-range, low-velocity injury produces high energy and often results in comminuted and complicated fractures with significant morbidity. In this study, four femoral, four tibial, and three combined tibia and fibular comminuted diaphyseal fractures secondary to close-range, low-velocity gunshot wounds in 11 military personnel were treated with debridement followed by compression-distraction lengthening using a circular external fixator frame. Fractur

[Fatal head injury caused by a crossbow arrow with unusually preserved posttraumatic volitional activity - case report].

The authors deal with a case of suicidal attempt resulting in a fatal head injury. A young man shot himself with a serially produced mechanical sports crossbow. The young man with a critical intracranial injury, a penetration, was nevertheless capable of basic locomotive activity, as well as of coherent communication with another people present at the scene. The critically injured patient was transported from the scene directly to medical centre where he subsequently underwent a neurologic surgery. On the eight day after the incident he died in the hospital as a result of sustained wounds. Dur

Incidental occult gunshot wound detected by postmortem computed tomography.

The body of a 59 year old woman underwent postmortem computed tomography (PMCT) examination prior to forensic autopsy, using a 256 slice multidetector row computed tomography scanner. A large left tension pneumothorax detected on the PMCT was considered to be a likely cause of death and this was confirmed at autopsy. In addition there was an unsuspected PMCT finding of a probable gunshot injury traversing the right orbit, facial bones and frontal sinus. The autopsy technique was adjusted accordingly and PMCT findings confirmed. PMCT in this case was not only diagnostic of cause of death, but a

[Forensic medical assessment of injuries to the human body and clothes caused by a crossbow arrow].

Analysis of the data published in the special literature revealed the lack of information for the objective comprehensive forensic medical evaluation of injuries to the human body and clothes caused by the arrows from different models of crossbows. Morphological characteristics of injuries were shown to strongly depend on the design features of the arrows. This fact can be used to differentiate between injuries inflicted by crossbow arrows and other types of missile weapons.

Fatalities due to gunshot injury by hunting firearm or during hunting, that had been investigated in the Institute of Legal Medicine of Hamburg University during the last 20 years, were retrospectively analyzed from the view points of epidemiology, morphology, criminology and ballistics. Thirteen accidents, 23 suicides and 13 homicides were included in this study. Female victims were observed in only 2 cases. In the accidents, the age of the most victims ranged from 30 to 60 years. Seven of 13 accidental victims killed themselves; remaining 6 were shot by other hunters. Nine accidents happened

A case in which a bone fragment caused by a bullet made a second channel in addition to the bullet channel.

A 64-year-old male was found dead in his house with his face covered with blood and a 38-caliber revolver between his legs. He had been suffering from type 2 diabetes mellitus and aftereffects of cerebral infarction. Autopsy revealed a normal round contact wound in the left lateral cervical region. A bullet from the firearm had entered through the left lateral cervical region and traveled to the outer right sternocleidomastoid muscle. This also triggered another wound from the fifth cervical vertebra to the muscle tissue near the right cartilage thyroid. At the end of this channel, there were

Less-lethal hybrid ammunition wounds: a forensic assessment introducing bullet-skin-bone entity.

Agencies all around the world now use less-lethal weapons with homogeneous missiles such as bean bag or rubber bullets. Contusions and sometimes significant morbidity have been reported. This study focuses on wounds caused by hybrid ammunition with the pathologists' flap-by-flap procedure. Twenty-four postmortem human subjects were used, and lesions caused on frontal, temporal, sternal, and left tibial regions by a 40-mm hybrid ammunition (33 g weight) were evaluated on various distance range. The 50% risk of fractures occurred at 79.2 m/sec on the forehead, 72.9 m/sec on the temporal, 72.5 m/

Visualization and material-based differentiation of lodged projectiles by extended CT scale and the dual-energy index.

Computed tomography (CT) scans of gunshot wounds and their high sensitivity in detecting osseous lesions has often been reported in the literature. However, studies concerning in situ examinations of lodged projectiles with CT to determine the ammunition used are lacking. Projectile visualizations are hampered in standard CT due to the presence of metal artifacts and the limited range of Hounsfield units (HU). The use of special reconstruction algorithms can overcome these limitations. For instance, using extended CT scale (ECTS) reconstruction supports detailed visualizations of metallic obje

Ballistic impact to the forehead, zygoma, and mandible: comparison of human and frangible dummy face biomechanics.

BACKGROUND: Currently, there is a greater use of nonlethal force in law enforcement and military operations. Because facial injuries have been observed, there is a need to understand the human response to ballistic impacts involving various regions of the face. This study aimed to establish blunt ballistic response corridors for high-speed, low-mass facial impacts to the forehead, zygoma, and mandible, and to determine how these responses compare with those of the frangible Hybrid III headform. Correlation of the human and dummy responses allows injury risk assessment for munitions used in the

Post-mortem CT in the investigation of homicides.

AIM: To investigate the reliability of post-mortem computed tomography (PMCT) in a case series of homicides involving blunt-force, sharp-force, and ballistic trauma. MATERIALS AND METHODS: The study investigates 16 homicide cases that underwent PMCT before autopsy. Two radiologists assessed the PMCT examinations and the data were compared to the forensic pathology findings. Data were organised in broad categories: foreign bodies, external injuries, soft-tissue and organ injuries, fractures, air in cavities, fluid collections, random pathology, and wound track. Findings were organised by syste

[Bullet embolism].

Bullet embolism is an uncommon consequence of penetrating firearm injuries, rarely observed in everyday forensic practice. The present report presents two cases of gunshot bullet embolism. The first examined case is a contact gunshot wound with the entry wound situated at the back of the victim. The wound channel terminated in the thoracic aorta, where from, in keeping with the blood flow, the bullet was transported into the left common iliac artery. The second case pertains to a gunshot in the head, with the bullet shot from a point blank range. The wound channel had a downward course and ter

Brain injuries caused by spherical bolts.

OBJECT: Metallic particles contained in antihuman bombs increase the number of fatalities. The ballistics of these particles depends on the explosive that is used, the distance from the explosion, the shape of the particle projected, and the biomechanics of the injured tissue. The authors present their experience with penetrating spherical bolt injuries to the brain. METHODS: The authors retrospectively reviewed clinical and radiological data obtained in eight patients with penetrating spherical bolt injuries to the cranium: four had Glasgow Coma Scale (GCS) scores less than 8 (three died, on

When One's Not Enough: Gunshot Suicide Characteristics and Demographics Associated With Having Multiple Gunshots.

Self-inflicted gunshot wounds are a common modality of suicide. Cases with multiple gunshot wounds are rare. Problems with determining manner may arise when there is a lack of understanding of how and why they occur. Demographic data has seldom been explored in these cases. Gunshot wound suicides from the years 2015 to 2023 were reviewed at the Hillsborough County Medical Examiner's Office. Nineteen cases with multiple gunshot wounds were found. The multi-GSW cases, compared to the single-GSW cases, had a significantly increased proportion of revolvers (57.9% vs. 29.8%, P =0.008), lower muzzle

Anatomical distribution and autopsy features of gunshot injuries to support the manner of death.

The autopsy features of gunshot wounds can be useful in understanding the manner of death. This research aims to provide concrete data to help to discriminate between homicide and suicide based on specific autopsy findings. A search of the database of the Cook County Medical Examiner's Office from August 2014 through April 2019 identified 3491 deaths due to gunshot wounds. Deaths due to complication from delayed fatal gunshot wounds, subjects who received hospital care prior to death, and decomposed bodies were identified and excluded. The following data were recorded: manner of death, demogra

A case of hunting death due to an overpenetrated bullet.

The authors report the case of a death which occurred during wild boar hunting. The scene details revealed that the victim and the beast were aligned in the visual shooting path and the victim had been struck by overpenetrating bullet shot by his friend on the neighbouring stand. The fatal entrance wound was on the victim's left chest. The authors emphasise the importance of the exhaustive forensic investigation, including autopsy of the beast, in cases like this, in order to distinguish accident from homicide.

The first case of C4-Blast related death in Malaysia: A multidisciplinary approach illustrated with emphasis on conjoint anthropologic and radiologic expertise in forensic identification of skeletal remains.

BACKGROUND: Blast related deaths are often shrouded by diagnostic and medicolegal complexities requiring multidisciplinary expertise in order to gauge accurate identification of the victims and document scientific investigations comprehensively. In the advent of more sophisticated technology, anthropologic methods can now be applied into post mortem imaging interpretation. The traditional imaging roles of characterizing osseous fragmentation, detecting and localizing foreign bodies can be expanded to simulate and support physical anthropologic examination to assist in documentation for court p

Effect of helmet liner systems and impact directions on severity of head injuries sustained in ballistic impacts: a finite element (FE) study.

The current study aims to investigate the effectiveness of two different designs of helmet interior cushion, (Helmet 1: strap-netting; Helmet 2: Oregon Aero foam-padding), and the effect of the impact directions on the helmeted head during ballistic impact. Series of ballistic impact simulations (frontal, lateral, rear, and top) of a full-metal-jacketed bullet were performed on a validated finite element head model equipped with the two helmets, to assess the severity of head injuries sustained in ballistic impacts using both head kinematics and biomechanical metrics. Benchmarking with experim

Characterisation of explosive fragments injuring the neck.

Penetrating explosive fragments are the most common cause of neck injuries sustained by UK service personnel deployed to Afghanistan. Analysis of these fragments will enable future ballistic protective materials to be tested with appropriate projectiles. However, only a small number of fragments excised from the neck have been available for analysis and they are potentially unrepresentative. We analysed computed tomograms (CTs) of 110 consecutive UK soldiers whose necks were wounded by explosive fragments. Fragments were classified according to shape, and their dimensions used to estimate volu

Blunt eye trauma: empirical histopathologic paintball impact thresholds in fresh mounted porcine eyes.

PURPOSE: Ballistic studies were conducted using gelatin-embedded abattoir-fresh porcine eyes suspended within clear acrylic orbits to discern the energy required to produce specific ocular injuries. Paintball impact provides a robust ballistic model for isolating and quantifying the role of direct blunt force in ocular trauma. METHODS: Fifty-nine porcine orbital preparations received direct blows from 0.68 caliber (16-18 mm diameter/3.8 g) paintballs fired at impact velocities ranging from 26 to 97 meters per second (2-13.5 J). Five additional eyes not subjected to ballistic impact were also

Retrobulbar lymphoma associated with a ballistic foreign body in a cat.

A seven-year-old domestic shorthair cat, adopted 5 years previously with a corneal perforation of the left eye, was presented for investigation of a left orbital mass. Computed tomography revealed a metallic foreign body within a contrast-enhancing, heterogeneous orbital mass. Large cell lymphoma was diagnosed from a fine needle aspirate. The cat staged negatively and was treated with L-asparaginase, prednisolone and three fractions of radiation therapy. A rapid clinical remission was obtained and the cat remained in remission for 3 years after therapy. This is the first report of large cell l
