Improving outcomes for babies and toddlers in child welfare: A model for infant mental health intervention and collaboration.

Children under three comprise a sizable and growing proportion of foster care placements. Very young children who enter the child welfare system experience disruptions of critical attachments that are essential to this formative stage of brain development, as well as other traumatic events, leaving them at great risk for lifelong impairments. To reverse these concerning outcomes, babies who have been removed from their homes require intensive, relationship-based interventions that promote secure attachment to a primary caregiver and holistic attention the child's developmental needs. Child wel

'My sister, that's family enough': Sibling constellations in the process of leaving care.

BACKGROUND: Various studies have shown that siblings can be an important social resource during out-of-home care, but there is limited knowledge regarding the role of sibling relationships after leaving care. OBJECTIVE: Based on a study titled 'Relevance of family in the transition from care to adulthood', this article investigates the significance of siblings in the process of leaving care. PARTICIPANTS AND SETTING: The target group was 41 care leavers from various regions of Austria between the ages of 18 and 27. METHODS: A qualitative research design was used combining problem-centred in

A mixed-methods study of the development and evaluation of an Internal Family Systems (IFS)-based group intervention for post-traumatic cognitions and fear of compassion among trauma-exposed adolescents in Out-of-Home Care (OOHC).

BACKGROUND: Adolescents in Out-of-Home Care (OOHC) often suffer multiple Adverse Childhood Experiences, leading to high rates of distorted Post-Traumatic Cognitions (PTC) and Fear of Compassion (FC). Tailored, evidence-based psychological interventions are essential for this group. OBJECTIVE: This mixed-methods study aimed to design, validate, and empirically evaluate a Internal Family Systems (IFS)-based group intervention for trauma-exposed adolescents in Out-of-Home Care (OOHC). PARTICIPANTS AND SETTING: Fifty trauma-exposed adolescents (27 female, 23 male) were purposively selected from

Motivated by identity: A social identity analysis of why people become foster carers.

Understanding why individuals are motivated to take on new identities, particularly why people choose to become foster carers, is critical to addressing persistent shortages in foster care systems. Despite policy consensus that foster care is the preferred alternative when children cannot remain with birth families, shortages persist across many countries. Research has traditionally emphasised individualistic motivations, such as altruism, neglecting broader social and identity-based factors. This study applies a Social Identity Approach, drawing on the Social Identity Model of Identity Change

Pediatric Self-Harm Injuries and Child Protection Histories.

We assessed the child protective services (CPS) histories of children with medical encounters for self-harm (SH) using linked medical and CPS data. Most children with SH had prior CPS reports (61.0%), but few had foster care placements (5.9%). CPS histories may be important to understand, treat, and prevent pediatric SH.

Foster care policy and homelessness among youth transitioning to adulthood from foster care.

BACKGROUND: Youth aging out of foster care experience troubling rates of homelessness. OBJECTIVE: This study examines whether policies directed towards youth transitioning to adulthood from foster care, Extended Foster Care (EFC) and Supervised Independent Living Placements (SILPs), reduce the risk of homelessness. PARTICIPANTS AND SETTING: Participants were 595 youths aging out of foster care in California. METHODS: Data from interviews conducted with the youths, and state child welfare administrative data on their involvement with the child welfare system, were used to examine predictors

The trajectory of PTSD among youth in foster care: A survival analysis examining maltreatment experiences prior to entry into care.

BACKGROUND: Youth in foster care are more likely than non-foster care youth to experience posttraumatic stress disorder (PTSD). While research has identified maltreatment as a risk factor for PTSD, this research remains limited because it tends to only (a) examine a single type or dimension of maltreatment and ignore the polyvictimization and heterogeneity in exposure, and (b) study this relation across a short period time or retrospectively at the end of care. OBJECTIVE: The current study used survival analysis to simultaneously examine the influence of maltreatment characteristics on the ri

Systematic review of organisation-wide, trauma-informed care models in out-of-home care (OoHC) settings.

Trauma in early childhood has been shown to adversely affect children's social, emotional, and physical development. Children living in out-of-home care (OoHC) have better outcomes when care providers are present for children, physically, psychologically, and emotionally. Unfortunately, the high turnover of out-of-home carers, due to vicarious trauma (frequently resulting in burnout and exhaustion) can result in a child's trauma being re-enacted during their placement in OoHC. Organisation-wide therapeutic care models (encompassing the whole organisation, from the CEO to all workers including

Antipsychotic treatment among youth in foster care.

OBJECTIVE: Despite national concerns over high rates of antipsychotic medication use among youth in foster care, concomitant antipsychotic use has not been examined. In this study, concomitant antipsychotic use among Medicaid-enrolled youth in foster care was compared with disabled or low-income Medicaid-enrolled youth. PATIENTS AND METHODS: The sample included 16 969 youths younger than 20 years who were continuously enrolled in a Mid-Atlantic state Medicaid program and had ≥1 claim with a psychiatric diagnosis and ≥1 antipsychotic claim in 2003. Antipsychotic treatment was characterized by

Psychosocial and cognitive functioning of children with specific profiles of maltreatment.

OBJECTIVE: Up to 90% of child welfare system cases involve multiple types of maltreatment; however, studies have rarely incorporated multiple dimensions of maltreatment. The present study employed a latent profile analysis to identify naturally occurring subgroups of children who had experienced maltreatment. METHODS: Reports of maltreatment incidents for 117 preschool-aged foster children were classified along two dimensions: type (e.g., physical abuse, sexual abuse, physical neglect, supervisory neglect, or emotional maltreatment) and severity within type. RESULTS: The analyses revealed fo

Biologic, foster, and adoptive parents: care givers of children exposed perinatally to human immunodeficiency virus in the United States. The Pediatric Spectrum of Disease Clinical Consortium.

Children born to human immunodeficiency virus (HIV)-infected mothers often do not live with a biologic parent because of drug use, illness, or death of the mother. Public health officials need to know the number and proportion of children who will require care by someone other than a biologic parent (alternative care giver). The Pediatric Spectrum of Disease project, conducted in six different geographic regions in the United States, assesses issues specific to HIV in children. Among the information being collected in this study are data regarding the primary care giver. Of 1683 children born

[Comparison of the results of clinico-epidemiological examination of retarded children-orphans and retarded children brought up in the family].

A group of mentally retarded children (n = 881) aged 8 to 14 years, identified as a result of a clinico-epidemiological screening, were examined. The results obtained were compared in two groups. Group 1 (n = 272 or 30.9%) included orphans and children who remained without their parents' care since infancy and group II (n = 609 or 69.1%) included children brought up in complete or incomplete families. The differences were established in the degree of mental retardation, in its complication by additional psychopathological symptomatology, in the share of some complicating syndromes and the time

Homicides of abused children prematurely returned home.

Five homicides of abused children are reviewed. These children had been removed from the parents because of physical abuse, then later returned to the home where they were subsequently killed. In each case, certain factors were present either during the interval of removal or after the return of the child, which could have alerted individuals, agencies or the court and, if heeded, could have prevented the deaths. Premature return occurred because of administrative inadequacies, including abbreviated investigations, violation of routine procedures because of heavy caseloads, and inadequate or a

An Expert-Derived Data-Driven Approach to Identifying Medicaid Children at Risk of Out-of-Home Placement: Development, Application, and Data-Sharing Considerations.

BACKGROUND: Out-of-home placement (OOHP) through child welfare, residential substance treatment, or extended inpatient care has life-altering impacts on child health and generates an outsized proportion of pediatric Medicaid costs. Early risk identification may prevent OOHP but is challenged by the difficulty of navigating state and federal data sharing regulations and multiple disconnected data systems. Identifying ways to streamline and simplify this process are critical to OOHP prevention. OBJECTIVES: To describe the development and implementation of an expert-derived data-driven algorithm

Adoption or Placement in Foster Care and Catch-up in Linear Growth and Development: A Meta-Analysis of Individual Participant Data.

The ability of children to recover from linear growth retardation, often referred to as catch-up growth, has intrigued researchers for many decades. Whether adoption from a low-income to a high-income setting, which provides a comprehensive improvement in the conditions that cause children to not grow well, leads to catch-up growth is unknown. We estimated the association of adoption (or placement in foster care) with catch-up in linear growth and child development before 5 y of age. We conducted a 2-stage meta-analysis using individual participant data for linear growth. We obtained study-spe

Patterns of substance use among adolescents in and out of foster care: An analysis of linked health and child welfare administrative data.

BACKGROUND: Young adults with a history of foster care have higher risk for substance use disorders. Social systems can deliver substance use prevention to youth; however, the timing of intervention delivery and how needs differ for youth in foster care are unclear. OBJECTIVE: To compare initiation and rates of substance use among adolescents in foster care to demographically similar adolescents never in foster care as identified by the healthcare system, and identify factors associated with increased substance use. PARTICIPANTS AND SETTING: Youth in foster care (n = 2787, ages 10-20, inclus

Do Foster Parents and Care Workers Recognize the Needs of Youth in Family Foster Care with a History of Sexual Abuse?

Children in family foster care have the right to participate in decisions regarding their life, however, adults often advocate on behalf of children. This Q methodological study explored whether shared perspectives among foster parents and care workers resemble shared perspectives of youth regarding the psychosocial needs experienced by youth with a history of sexual abuse. Participants sorted a set of statement cards according to what they thought was most important for youth. By-person factor analyses examined how the Q sorts of foster parents and care workers related to those of youth. The

Development and Examination of the Reactive Attachment Disorder and Disinhibited Social Engagement Disorder Assessment Interview.

The fifth edition of the Diagnostic and Statistical Manual (DSM) categorizes reactive attachment disorder (RAD) and disinhibited social engagement disorder (DSED) as two separate disorders, and their criteria are revised. For DSED, the core symptoms focus on abnormal social disinhibition, and symptoms regarding lack of selective attachment have been removed. The core symptoms of RAD are the absence of attachment behaviors and emotional dysregulation. In this study, an international team of researchers modified the Child and Adolescent Psychiatric Assessment for RAD to update it from DSM-IV to

Reactive Attachment Disorder and Disinhibited Social Engagement Disorder in School-Aged Foster Children--A Confirmatory Approach to Dimensional Measures.

This study aimed to investigate the factor structure and external correlates of the constructs Reactive Attachment Disorder (RAD) and Disinhibited Social Engagement Disorder (DSED) from the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). The following were addressed: First, do our data support the DSM-5 conceptualization of RAD/DSED as two separate constructs? Second, are RAD and DSED distinct from other well-established dimensions of child psychopathology? Third, what are the external correlates of RAD/DSED in this sample? The study sample included 122 foster children aged 6-10
