BACKGROUND CONTEXT: Nearly 70% of patients with multiple myeloma (MM) experience vertebral fracture. As a consequence, these patients suffer significantly poorer quality of life. However, no studies have characterized the natural progression of these fractures.

BACKGROUND CONTEXT: Nearly 80% of patients with newly diagnosed multiple myeloma (MM) have bony lesions on magnetic resonance imaging (MRI). These lesions may progress to debilitating vertebral fractures. No studies have quantitatively characterized these fractures or identified predictors of fracture burden and severity.

Objective of our study was to assess the outcome of cement augmentation in patients with multiple myeloma. We reviewed 12 patients with 48 vertebral fractures. Mean age was 62.5 years. Average length of follow-up was 27.5 months. Expected survival was less than 12 months in 2 patients and more than 12 months in the remaining patients. After surgery mean survival was 32.5 months. A patient on methotrexate therapy for scleroderma developed four stress fractures within a period of 13 months. She was not on steroid therapy and had no risk factors for osteoporosis. Ankylosing hyperostosis of the spine is a distinct clinical entity that merits diagnosis and recognition of its structural limitations. The case of multiple thoracic spine fractures presented here emphasizes the potentially increased susceptibility of involved spines to trauma and also suggests the need to caution diagnosed patients from placing their spines at undue risk.

A prospective series of 165 displaced subcapital fractures of the femoral neck treated with reduction and percutaneous multiple pinning is presented. Two cases of multiple fractures, one in a newborn with an unclassified bone dysplasia, another in an infant with recurrent fits of unknown etiology are reported.
