Comparative analysis of hydrogen breath test devices for the diagnosis of carbohydrate malabsorption: fructose tests lack reliability.

INTRODUCTION: Hydrogen breath tests are commonly used to diagnose lactose and fructose malabsorption. While lactose breath tests are reliable, the utility of fructose tests is debated due to inconsistent results. This study aimed to compare the performance of two hydrogen breath test devices - handheld and stationary - in diagnosing lactose and fructose malabsorption in paediatric patients. METHODS: Seventy-eight lactose and 53 fructose breath tests were performed in children (mean age: 11.4 ± 3.1 years) using both a handheld electrochemical detector and a stationary gas chromatograph. Malabs

Extragastrointestinal Symptoms and Sensory Responses During Breath Tests Distinguish Patients With Functional Gastrointestinal Disorders.

INTRODUCTION: Patients with functional gastrointestinal disorders (FGIDs) are classified based on their gastrointestinal (GI) symptoms, without considering their frequent extra-GI symptoms. This study defined subgroups of patients using both GI and extra-GI symptoms and examined underlying mechanisms with fructose and lactose breath tests. METHODS: Latent class analysis defined distinct clusters of patients with FGID based on their long-term GI and extra-GI symptoms. Sensory and breath gas responses after fructose and lactose ingestion were compared across symptom clusters to investigate diff

Circulating levels of glucoselysine, a fructose-specific glycation product, are modulated by dietary fructose; post-hoc analysis of a case-control study and a randomized controlled trial.

BACKGROUND: Fructose consumption has been associated with cardiometabolic disease, including type 2 diabetes and cardiovascular disease. The formation of the fructose-specific glycation product glucoselysine (GL) may underlie the development of these complications. A previous study showed that endogenously produced fructose contributes to GL formation. OBJECTIVE: Here, we aimed to investigate the effects of exogenous fructose on the formation of GL. Methods We first developed and validated an ultra-performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS) stable isotope dilution

Fructose-induced aberration of metabolism in familial gout identified by 31P magnetic resonance spectroscopy.

The hyperuricemia responsible for the development of gouty arthritis results from a wide range of environmental factors and underlying genetically determined aberrations of metabolism. 31P magnetic resonance spectroscopy studies of children with hereditary fructose intolerance revealed a readily detectable rise in phosphomonoesters with a marked fall in inorganic phosphate in their liver in vivo and a rise in serum urate in response to very low doses of oral fructose. Parents and some family members heterozygous for this enzyme deficiency showed a similar pattern when given a substantially lar

Pathogenesis of Hypertension in Metabolic Syndrome: The Role of Fructose and Salt.

Metabolic syndrome is manifested by visceral obesity, hypertension, glucose intolerance, hyperinsulinism, and dyslipidemia. According to the CDC, metabolic syndrome in the US has increased drastically since the 1960s leading to chronic diseases and rising healthcare costs. Hypertension is a key component of metabolic syndrome and is associated with an increase in morbidity and mortality due to stroke, cardiovascular ailments, and kidney disease. The pathogenesis of hypertension in metabolic syndrome, however, remains poorly understood. Metabolic syndrome results primarily from increased calori

Inborn errors of carbohydrate metabolism.

Glycogen storage diseases (GSD) and inborn errors of galactose and fructose metabolism are the most common representatives of inborn errors of carbohydrate metabolism. In this review the focus is set on the current knowledge about clinical symptoms, diagnosis and treatment. Hepatomegaly and hypoglycaemia are the main findings in liver-affecting GSD like type I, III and IX. Diagnosis is usually made by non invasive investigations, e.g. mutation analysis. In GSD I, a carbohydrate balanced diet with frequent meals and nocturnal continuous tube feeding or addition of uncooked corn starch are the m

Synergistic Effects of Naringenin and Metformin on Dyslipidemia and Glucose Regulation in High Fructose-Fed Rats.

BACKGROUND: Excessive intake of fructose can lead to insulin resistance (IR). Metformin is commonly used as a first-line treatment for type 2 diabetes, and naringenin, a flavonoid known for its anti-inflammatory properties, can potentially improve insulin resistance. However, the effects of combining these two treatments have not been evaluated. Therefore, the present study aims to investigate the combined effects of naringenin and metformin on fructose-induced insulin resistance in male rats. MATERIALS AND METHODS: Male Wistar rats were divided into five groups: control (without receiving fr

Changes of liver metabolite concentrations in adults with disorders of fructose metabolism after intravenous fructose by 31P magnetic resonance spectroscopy.

A novel 31P magnetic resonance spectroscopy procedure allows the estimation of absolute concentrations of certain phosphorus-containing compounds in liver. We have validated this approach by measuring ATP, phosphomonesters, and inorganic phosphate (Pi) during fasting and after an i.v. fructose bolus in healthy adults and in three adults with disorders of fructose metabolism and by comparing results with known metabolic concentrations measured chemically. During fasting, the ATP concentration averaged 2.7 +/- 0.3 (SD, n = 9) mmol/L, which, after due correction for other nucleoside triphosphates

Fructose increases corticosterone production in association with NADPH metabolism alterations in rat epididymal white adipose tissue.

Metabolic syndrome is an array of closely metabolic disorders that includes glucose intolerance/insulin resistance, central obesity, dyslipidemia, and hypertension. Fructose, a highly lipogenic sugar, has profound metabolic effects in adipose tissue, and has been associated with the etiopathology of many components of the metabolic syndrome. In adipocytes, the enzyme 11 β-HSD1 amplifies local glucocorticoid production, being a key player in the pathogenesis of central obesity and metabolic syndrome. 11 β-HSD1 reductase activity is dependent on NADPH, a cofactor generated by H6PD inside the end

Long-term hepatic damage in high-fructose-fed C57BL/6 mice: hepatic fibrogenesis, endoplasmic reticulum stress markers, and fibrosis.

The rising fructose intake in sugar-sweetened beverages and ultra-processed foods relates to the high incidence of nonalcoholic fatty liver disease. This study aimed to examine the effects of long-term high-fructose diet intake (for 16 or 20 weeks) on progressive hepatic damage, focusing on the endoplasmic reticulum stress markers and fibrogenesis as possible triggers of liver fibrosis. Forty 3-month-old male C57BL/6J mice were randomly divided into four nutritional groups: C16 (control diet for 16 weeks), C20 (control diet for 20 weeks), HFRU16 (high-fructose diet for 16 weeks), and HFRU20 (h

Four cases of long-standing diarrhoea and colic pains cured by fructose-free diet--a pathogenetic discussion.

Four patients with a long history of abdominal swelling, colic pains and diarrhoea were cured by a fructose-free diet. They were all given an oral load of (14C) fructose and their expiratory excretion of 14CO2 was found to be significantly lower than that of control patients with similar symptoms. It is concluded taht the patients cured by a fructose-free diet might have a partial fructose malabsorption.

Excess Maternal Fructose Consumption Increases Fetal Loss and Impairs Endometrial Decidualization in Mice.

The most significant increase in metabolic syndrome over the previous decade occurred in women of reproductive age, which is alarming given that metabolic syndrome is associated with reproductive problems including subfertility and early pregnancy loss. Individuals with metabolic syndrome often consume excess fructose, and several studies have concluded that excess fructose intake contributes to metabolic syndrome development. Here, we examined the effects of increased fructose consumption on pregnancy outcomes in mice. Female mice fed a high-fructose diet (HFrD) for 6 weeks developed glucose

Galactose consumption induces conditioned flavor avoidance in rats.

Recent findings revealed that intragastric infusions of galactose conditioned a flavor avoidance in adult rats. To determine whether the galactose-conditioned avoidance was due to the infusion procedure, we investigated the flavor conditioning effect of orally consumed galactose. Food-restricted rats drank a flavored galactose solution, a flavored fructose solution and a flavored saccharin solution in separate one-bottle training sessions; grape, cherry and orange flavors were used. Because fructose is sweeter than galactose, saccharin was added to the galactose solution to increase its palata

Ketohexokinase: A central mediator of fructose-associated pathogenesis and promising therapeutic target.

Accumulating preclinical and clinical evidence identifies ketohexokinase (KHK), the initial enzyme of fructose catabolism, as a core mediator of fructose-driven pathogenesis, positioning pharmacological KHK inhibition as a promising therapeutic strategy for fructose-associated disorders. Functionally divergent KHK isoforms arise from alternative splicing. The high-activity KHK-C, predominantly expressed in the liver, kidney, and small intestine, drives fructose-related metabolic pathologies, including metabolic dysfunction-associated steatotic liver disease, diabetic kidney disease, and obesit

Secondary disorders of glycosylation in inborn errors of fructose metabolism.

Adamowicz and colleagues raised the alert in 2007 about patients with atypical hereditary fructose intolerance (HFI) primarily misdiagnosed as CDG Ix. We describe a girl with neonatal hypertonia, facial trismus, absent swallowing and coughing reflexes, gastro-oesophageal reflux and sporadically elevated Krebs cycle metabolites and lactate. At 14 months microcephaly and hepatomegaly were noted, with hypertransaminasaemia but normal blood coagulation, glucose, phosphate, and absent urinary reducing substances. Neurological impairment persisted. Because of hepatic and neurological abnormalities w
