Impact of Nissen fundoplication on laryngopharyngeal reflux symptoms.

The reliability of Nissen fundoplication for the successful treatment of laryngopharyngeal reflux (LPR) symptoms remains in question. The purpose of this study was to assess the effect that antireflux surgery has on a variety of LPR symptoms as well as the patient's perceived success of surgical intervention. A retrospective review of all antireflux surgeries between 1998 and 2008 provided a patient base for a survey in which patients ranked pre- and postoperative LPR symptoms in addition to patient satisfaction with the outcome. Of the 611 patients identified and sent the evaluation forms, 24

Oesophageal acid hypersensitivity is not a contraindication to Nissen fundoplication.

BACKGROUND: The Rome III criteria classify patients with a positive relationship between symptoms and reflux episodes but a physiological oesophageal acid exposure time as having gastro-oesophageal reflux disease (GORD) with an acid hypersensitive oesophagus. The long-term outcome of antireflux surgery in these patients was investigated. METHODS: Outcomes of Nissen fundoplication in 28 patients with GORD refractory to proton-pump inhibitors (PPIs) and oesophageal acid hypersensitivity (group 1) were compared with those of 126 patients with pathological acid exposure (group 2). RESULTS: Fundo

Clinical effectiveness of laparoscopic fundoplication in a U.S. community.

BACKGROUND: The aim of our study was to determine the outcome of laparoscopic fundoplication for reflux disease in a cohort of patients who underwent this procedure in routine clinical practice. METHODS: We identified 151 patients who had undergone laparoscopic fundoplication in a managed care organization in Milwaukee. Symptoms were evaluated using a validated questionnaire. Postoperative medication use and endoscopic and surgical procedures were recorded. RESULTS: Eighty-seven patients agreed to participate, of whom 80 (41 [51%] men) were eligible. Their mean (+/- SD) age was 45 +/- 12 yea

[Laparoscopic treatment of gastro-esophageal reflux by fundoplication].

The aim of this retrospective study was to evaluate the results of the laparoscopic fundoplication for gastroesophageal reflux disease. From January 1997 to February 2001, we performed 8 laparoscopic fundoplication. They are 5 men and 3 women with mean age of 45 [30-67 years]. The delay of apparition the symptoms varied from 6 months to 14 years. Seven patients had a heart burn. The preoperative evaluation included endoscopy, gastrointestinal contrast radiography, 24 hour pH monitoring and esophageal manometry. The surgical procedures were partial fundoplication (Toupet) in 7 cases and complet

Quality of life before and after laparoscopic fundoplication.

BACKGROUND: Laparoscopic fundoplication is a well-established surgical option for the treatment of gastroesophageal reflux disease. The aim of this study was to assess the surgical outcomes from the patient's point of view by using a validated quality of life instrument. METHODS: Fifty patients have been prospectively included. All patients underwent a standardized 270-degree posterior fundoplication. Quality of life was measured by the Gastrointestinal Quality of Life Index (GIQLI), a 36-item-questionnaire. The patients received the questionnaire before surgery, and 3 months and 1 year after

Laparoscopic fundoplication for dysphagia and peptic esophageal stricture.

Peptic esophageal stricture with dysphagia is a late manifestation of severe gastroesophageal reflux disease (GERD). Although laparoscopic fundoplication is an effective antireflux operation, its efficacy for persons with peptic esophageal stricture and dysphagia has not been well defined. The aim of this study was to evaluate outcomes after fundoplication in this subgroup of GERD patients. Forty GERD patients with moderate, severe, or incapacitating dysphagia and peptic esophageal stricture were compared to a control group of 121 GERD patients without significant dysphagia or stricture. Paraoesophageal hernia repair with and without concomitant fundoplication.

There is currently no consensus as to whether an antireflux procedure should accompany surgical repair of paraoesophageal hernia. Forty consecutive patients with paraoesophageal hernia were studied. Surgery routinely included transabdominal hernia reduction, excision of the sac and crural repair. The addition of fundoplication was based on the presence of preoperative endoscopic evidence of oesophagitis. Twenty-three patients without endoscopic oesophagitis had no antireflux procedure whereas 17 with oesophagitis underwent concomitant antireflux surgery. Thirty-six patients were followed for 1

Fundoplication in preterm infants with gastro-oesophageal reflux.

Gastro-oesophageal reflux in preterm infants may result in recurrent pulmonary insult due to aspiration of gastric contents, and intractable obstructive apnoea. Fundoplication is effective in controlling reflux when medical management has failed. Our experience between 1981 and 1990 was reviewed to assess the efficacy of early surgery in the management of 11 such infants. The infants had a median gestational age of 29 weeks and a median birthweight of 1032 g. Nine infants had fundoplication for recurrent aspiration pneumonitis and two for intractable apnoea. The median age at the time of surge

Is yield pressure at the cardia increased by effective fundoplication?

Yield pressure at the cardia was measured before and after fundoplication in 10 patients; five had a Nissen fundoplication and five a Belsey mark IV procedure. Surgery was considered successful if oesophagitis healed or if 24 hour pH monitoring returned to normal. There was a marked rise in yield pressure in all eight patients with a successful operation. Yield pressure remained low in two patients in whom oesophagitis persisted.

The radiographic appearance of complications following Nissen fundoplication.

Seventy patients had an upper gastrointestinal examination following Nissen fundoplication for reflux esophagitis associated with hiatal hernia. Thirty-nine were asymptomatic and the fundoplication appeared normal. Of the 31 patients with symptoms (dysphagia, pain, or vomiting), 15 had spontaneous relief and demonstrated a normal postsurgical radiographic appearance of the stomach. The other 16 had both persistent symptoms and radiographic abnormalities, including 5 stenoses, 3 recurrent hernias, and 8 pouch deformities of the fundus. The roentgenographic features and etiology of these surgica

[The telescope phenomenon. A complication possibility following fundoplication].

On the basis of four cases from our own clinic and three outside cases, we describe a so-called, "telescope phenomenon". This involves a specific complication that may arise after fundoplication. The characteristic of this postoperative complication is the slipping down of the fundus cuff, accompanied by a telescopic pushing upwards of the cardia. Recurrent reflux and occasionally new manifestations of dysphagia are the clinical results of this. This paper describes the diagnostic procedure and therapy of the telescope phenomenon.

Symmetrical anterior-posterior partial fundoplication: technique and outcomes.

BACKGROUND: Partial fundoplication is the most common surgical treatment for gastroesophageal reflux disease (GERD). Our preferred technique is a symmetrical anterior-posterior partial (300°) fundoplication created along the natural axis of the stomach, leaving the bare area on the esophagus along the lesser curvature. We assessed perioperative and long-term outcomes of this technique. METHODS: We retrospectively reviewed records of all patients who underwent primary elective antireflux surgery by a single foregut surgeon from September 2016 to December 2024. Non-primary and emergency procedu

Gastric Volvulus After Nissen Fundoplication: A Case Report.

Gastric volvulus is a rare condition in which there is an abnormal rotation of the stomach, which can be life-threatening. Most cases occur in patients with a hiatal hernia or other diaphragmatic defects. Gastric volvulus after Nissen fundoplication has been rarely reported, with several proposed mechanisms. We present the case of a 53-year-old woman with a history of previous Nissen fundoplication who presented with nausea and vomiting due to a mesenteroaxial gastric volvulus caused by an adhesion. Adhesiolysis and volvulus reduction were performed, along with gastropexy. The postoperative co

Superior Mesenteric Arterial Occlusion Following Laparoscopic Partial Fundoplication.

A 62-year-old male with history and endoscopic findings consistent with gastroesophageal reflux underwent elective laparoscopic fundoplication. He developed severe abdominal pain four days postoperatively, and computed tomography (CT) angiogram of the abdomen demonstrated occlusion of the superior mesenteric artery due to dissection. The patient was administered intravenous heparin following vascular surgical advice, resulting in resolution of the pain within an hour and no subsequent complications. Laparoscopy-associated mesenteric vascular events are rare but associated with very high morbid

Laparoscopic Posterior Partial Fundoplication for Gastroesophageal Reflux Disease.

Gastroesophageal reflux disease (GERD) is a common condition that greatly impacts quality of life. Management options include medical and surgical therapies. Nonoperative management typically relies on longitudinal use of acid-suppressive medications such as proton pump inhibitors, which is associated with a significant financial burden and an increasing number of recognized side effects. The surgical management of GERD is focused on correction of the lower esophageal sphincter dysfunction by means of a fundoplication, thus limiting acid and nonacid gastroesophageal reflux. Multiple techniques

Roux-En-Y gastric bypass following failed fundoplication.

INTRODUCTION: Roux-En-Y gastric bypass (RYGB) is an alternative to reoperative fundoplication. The aim of this study was to expand long-term outcomes of patients undergoing RYGB after failed fundoplication and assess symptom resolution. METHODS: A single institution prospective study was performed of patients undergoing fundoplication takedown and RYGB between March 2007 and September 2016. Demographics, body mass index (BMI), preoperative symptoms, operative duration and findings, and postoperative outcomes were recorded. Data were assessed using standard statistical methods. Concomitant fundoplication increases morbidity of gastrostomy tube placement.

BACKGROUND: Fundoplication is often performed in conjunction with gastrostomy tube (GT) placement in children, but there is a great deal of variation in rates of and indications for this procedure. Little is known about the impact of fundoplication on peri-operative outcomes. This study examines a national cohort of pediatric patients to compare risk-adjusted surgical outcomes in patients undergoing GT placement with or without concomitant fundoplication. METHODS: We identified all patients undergoing GT placement in the 2012 National Surgical Quality Improvement Program - Pediatric. We evalu

Preoperative factors predicting clinical outcome following laparoscopic fundoplication.

BACKGROUND: Antireflux surgery is effective for the treatment of gastroesophageal reflux, but not all patients benefit equally from it. The challenge is to identify the patients who will ultimately benefit from antireflux surgery. The aim of this study was to identify preoperative factors that predict clinical outcome after antireflux surgery, with special interest in the influence of socioeconomic factors. METHODS: Preoperative clinical and socioeconomic data from 1,650 patients who were to undergo laparoscopic fundoplication were collected prospectively. Clinical outcome measures (persisten
