Root resorption and ankylosis associated with guided tissue regeneration.

BACKGROUND: Root resorption and ankylosis have been reported rarely as sequelae to guided tissue regeneration (GTR). The authors describe a clinical case of root resorption following GTR that involved the use of a bioabsorbable membrane. CASE DESCRIPTION: Two years after GTR was performed on a Class II furcation defect, the clinical examination revealed root resorption reaching the pulp chamber. The furcation defect was filled with epithelium and connective tissue, which contained inflammatory infiltrate and fragments of the membrane. The authors also observed areas of the tooth that exhibite

[Root resection in the era of dental implants].

The treatment of furcation defects is one of the most challenging aspects of periodontal therapy. The periodontal therapist can utilize various treatment modalities, including: non surgical root debridement, local drug delivery, open flap surgery, tunneling, root resection, guided tissue regeneration and extraction. Each treatment method has its advantages and disadvantages. The dilemmas concerning tooth prognosis and prosthetic considerations in such cases are especially demanding, especially in comparison to implant therapy. The clinician's decision in these situations must comply with the o

Subgingival restorations with resin ionomer: a periodontal alternative.

The successful use and placement of subgingival resin-ionomer restorations in both anterior root and molar furcation defects are demonstrated in this article. Sustained tissue health and minimal probing depths at the surgical site demonstrate clinical success. These case reports illustrate the continued success of alternative treatment procedures for restoring subgingival mechanical root or periodontal lesions.

Influence of root anatomy on periodontal disease.

One of the essential elements of periodontal therapy is long-term maintenance. Anatomic factors that favor localized plaque accumulation may contribute to the progression of disease. The purpose of this investigation was to evaluate the influence of root concavities on the severity of furcation involvement. Measurements were recorded clinically at the time of surgical treatment. Statistical analysis revealed no significant association between the degree of root concavity and the severity of furcation involvement.

Clinical significance of furcation anatomy of the maxillary first premolar: a biometric study on extracted teeth.

Multirooted teeth in which chronic periodontitis has progressed to involve the furcation present special problems in treatment. The apical placement of furcation, narrow furcation entrance, concavities on the root surface, and the undesirable root configuration make instrumentation and plaque control difficult. Furcation involvement, when present in maxillary first premolars in particular, presents a poor prognosis as the furcation opens mesiodistally. This study on 100 extracted maxillary first premolars is an attempt to determine the frequency of bifurcation, the proximity of bifurcation to

Diagnostic accuracy of artificial intelligence-based deep learning models in detecting furcation involvement: A systematic review and meta-analysis.

BACKGROUND: Furcation involvement complicates the management of periodontitis and increases the risk of tooth loss. Conventional methods of detection, such as probing and two-dimensional radiographs, are limited by operator variability and anatomical complexity. Deep learning has shown a potential to detect furcation involvement on radiographic images. The aim of this review was to systematically evaluate the diagnostic potentials of deep learning models in detecting furcation involvement on radiographic images. METHODS: Systematic search was conducted in PubMed, EMBASE, CENTRAL, ClinicalTria

Periodontal treatment of furcation involvement at the mandibular first molar with a follow-up of 27 years.

Due to the complicated anatomical structures in the furcation area of multirooted mandibular first molars, dental hygiene is greatly compromised once the furcation is involved in the periodontitis, leading to the unfavorable prognosis of teeth with furcation involvement. A patient came to a dental office with the chief complaint of "mobile mandibular posterior tooth" 27 years ago. The periapical film showed alveolar bone resorption at the root furcation of the right mandibular first molar. Flap surgery and fine supportive therapy were conducted. The patient was diagnosed with "furcation involv

Unsupervised phenotyping of the periodontal architecture through high-dimensional clustering of electronic health records: A multicenter study.

BACKGROUND: To identify novel periodontal phenotypes using unsupervised machine learning on a large-scale, multicenter cohort, specifically characterizing disease patterns based on the "periodontal architecture" of localized structural failures (tooth mobility and molar furcation defects) rather than global severity averages alone. METHODS: This cross-sectional study analyzed electronic health records from 15,723 adult patients with periodontitis. A high-dimensional feature vector (D = 72) was constructed for each patient, integrating tooth-specific ordinal grades for mobility and furcation,

Evolution of Barrier Membranes in Periodontal Regeneration-"Are the third Generation Membranes really here?".

In the last decades, Guide Tissue Regeneration (GTR) technique has been applied for the treatment of various periodontal defects such as intrabony defects, furcation involvements and localized gingival recession defects. From early days of using membranes with the simple aim of minimizing toxic response in the host, membranes have come a long way. Third generation membranes not only act as barriers but also as delivery devices to release specific agents. Many clinical trials have focused on using membranes as delivery devices for antibiotics and growth factors. In this article we take a brief
