Fifty patients with duodenal ulcer disease were examined. In 17 patients (1 group) the constantly recurrent course of the disease was noted, in 33 patients (2 group) the complications (hemorrhage, perforation, stenosing) have occurred in anamnesis. In patients of the first group the acid production rising was concomitant with specific somatostatin-dependent cell reactions domination, in the second one--pentagastrin-dependent.

The circadian pattern of gastric acidity was assessed in 53 normal subjects and in 33 patients with prior cholecystectomy using continuous pH monitoring. The 24-hour and nocturnal median pH values were significantly higher in cholecystectomized patients than in controls (p less than 0.03 and p less than 0.001, respectively). To clarify the protein metabolism in peptic ulcer disease, the amino acid content was determined in gastric juice of gastric ulcer patients (n = 30), duodenal ulcer patients (n = 16), gastroduodenal ulcer patients (n = 8), and hospital controls (n = 8). Use of anticholinergic drugs in treatment of duodenal ulcers is limited by the side effects of widespread parasympathetic blockade evoked by usual therapeutic doses. A study was conducted into the effectiveness of transdermal delivery of hyoscine methobromide using a new system which releases the drug into the circulation at a controlled rate. In six patients whose duodenal ulcer had healed secretion of acid was measured over two nights, the first on placebo and the second on hyoscine methobromide. All patients responded to the active drug and showed a significant inhibition of acid secretion.

At present proximal gastric vagotomy is the best long-term therapy for duodenal ulcer. It is indicated in cases in which conservative treatment has failed. To study the effect of Esaprazole, a new antiulcer drug, on acid, peptic and alkaline secretion a modified gastric acid test was performed in 18 duodenal ulcer patients.
