Modified Spitz procedure using a Collis gastroplasty for the repair of long-gap esophageal atresia.

OBJECTIVE: We report here on our experience with a modified Spitz procedure using a Collis gastroplasty for the repair of long-gap esophageal atresia (EA). PATIENTS AND METHODS: The authors reviewed 5 cases of children with long-gap (5-6 vertebral bodies) EA. The repair was carried out after 3 months of permanent monitoring, enteral nutrition through a gastrostomy tube and permanent aspiration of the proximal esophagus. The Collis gastroplasty was performed to create a gastric tube along the gastric lesser curvature using an Endo-GIA linear stapler. The gastric tube can be easily mobilized in

Laparoscopic Collis gastroplasty and Nissen fundoplication for reflux esophagitis with shortened esophagus in Japanese patients.

BACKGROUND: There is an extremely small number of surgical cases of laparoscopic Collis gastroplasty and Nissen fundoplication (LCN procedure) in Japan, and it is a fact that the surgical results are not thoroughly examined. PURPOSE: To investigate the results of LCN procedure for shortened esophagus. PATIENTS AND METHODS: The subjects consisted of 11 patients who underwent LCN procedure for shortened esophagus and followed for at least 2 years after surgery. The group of subjects consisted of 3 men and 8 women with an average age of 65.0+/-11.6 years, and an average follow-up period of 40.7

Stapled gastroplasty for antral stricture after repair of neonatal gastric rupture: report of a case.

The patient was the second of twin girls born at 32 weeks' gestation, weighing 1736 g. Abdominal distention was noted the day after birth, and abdominal x-ray showed free air in the abdomen. An emergent laparotomy with debridement and primary closure were performed for spontaneous gastric rupture. From the age of 8 months, the infant sometimes vomited after meals and was unable to gain weight. She was diagnosed, by upper gastrointestinal series and gastroscopy, with antral stricture with a normal prepyloric area and pylorus. The presence of an antral web, ulcers, or ectopic pancreatic tissue w

Effect of weight loss on bone metabolism: comparison of vertical banded gastroplasty and medical intervention.

BACKGROUND: We studied the effects of weight loss on bone metabolism. METHODS: 16 consecutive surgically-treated (14 female, 2 male) morbidly obese patients and 65 obese (53 male, 12 female) medically-treated patients were enrolled in an observational study. Surgical treatment for morbidly obese patients was vertical banded gastroplasty (VBG). Studies were performed prior to and 12 months after the start of treatment. Bone mineral density (BMD), bone turnover markers, sex steroids, calcium excretion and parathyroid hormone measurements were done at each visit. RESULTS: Weight loss was more p

Vertical banded gastroplasty in the treatment of morbid obesity: results of three year follow up.

Vertical banded gastroplasty has replaced jejunoileal bypass in the surgical treatment of morbid obesity. Although this procedure carries a low incidence of early postoperative complications there is little information on either longer term follow up or the impact on nutritional status. The present study describes the outcome in 42 patients with morbid obesity who underwent vertical banded gastroplasty and were followed up in a special nutrition clinic between nine and 36 months postoperatively. Body mass index (BMI) fell from a preoperative value of 47.4 (6.7) kg/m2 (mean (SD] to 37.5 (5.8),

Long-term outcomes after endoscopic sleeve gastroplasty: impact of clinician follow-up and lifestyle engagement.

BACKGROUND: Bariatric interventions rely on lifestyle changes that benefit from long-term follow-up and patient engagement. Mid- and long-term outcomes following endoscopic sleeve gastroplasty (ESG) are heterogeneous and likely multifactorial. While the literature has explored patient demographic and anthropometric influence, as well as procedural technique, on post-ESG weight loss, there are knowledge gaps about the influence of aftercare programs. We hypothesize that clinician follow-up intensity and exercise participation correlate with weight loss after ESG. METHODS: We conducted a retros

Improvement in obesity-related comorbidities 5 years after endoscopic sleeve gastroplasty: a prospective cohort study.

BACKGROUND AND AIMS: Endoscopic sleeve gastroplasty (ESG) is a minimally invasive bariatric procedure of the gastric cavity to facilitate weight loss. We aimed to evaluate the long-term effects of ESG as a monotherapy on obesity-related comorbidities over 5 years. METHODS: This prospective study analyzed data from 404 consecutive patients (aged 45 ± 11.9 years, 76% women) who underwent ESG from August 2013 through June 2024. All patients had body mass indices ≥30 or ≥27 kg/m2 with comorbidities. Patients receiving adjuvant therapy were excluded to assess ESG as a monotherapy. All procedures w

Successful revision surgery for very late-onset stomal obstruction following Gomez gastroplasty: a case report.

BACKGROUND: Gomez gastroplasty, which was developed in the 1970s as one of the gastric restrictive surgeries for severe obesity, partitions the stomach using a stapler from the lesser towards the greater curvature at the upper gastric body, leaving a small channel. This procedure is no longer performed due to poor outcomes, but surgeons can encounter late-onset complications even decades after the surgery. Here, we report a case of very late-onset stomal obstruction following Gomez gastroplasty which was successfully treated by revision surgery. CASE PRESENTATION: A 58-year-old man was referr

A Video Case Report of Gastric Perforation Following Endoscopic Sleeve Gastroplasty and its Surgical Treatment.

INTRODUCTION: Endoscopic sleeve gastroplasty (ESG) is a novel weight loss procedure that reduces the size of the stomach using an endoscopic suturing device. There are severe adverse events that have been reported following ESG (Brethauer et al. Surg Obes Relat Dis. 6:689-94, 2010; Abu Dayyeh et al. Gastrointest Endosc. 78:530-5, 2013; Nava et al. Endoscopy. 47:449-52, 2015; Nava et al. Endosc Int Open. 4(2):E222-7, 2016). However, complications like gastric perforation following ESG have not been reported. This video presents a case with gastric perforation following ESG and its surgical trea

Multimodal Antiemetic Prophylaxis in Patients Undergoing Endoscopic Sleeve Gastroplasty: A Single-Center Retrospective Case Series.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is associated with a high incidence of postoperative nausea and vomiting (PONV) due to gastric volume reduction, preservation of vagal innervation, and inherent patient-related risk factors. Standard antiemetic prophylactic regimens are frequently insufficient in this population. This case series describes the clinical experience and postoperative outcomes associated with a multimodal antiemetic prophylaxis regimen. METHODS: In this single-center retrospective case series, data from 20 adult patients who underwent ESG under general anesthesia w

Six-month comparative outcomes of endoscopic sleeve gastroplasty versus laparoscopic sleeve gastrectomy: an MBSAQIP analysis.

BACKGROUND: North American data regarding long-term efficacy and durability of endoscopic sleeve gastroplasty (ESG) are remarkably scarce outside of clinical trial settings hampering our understanding of its optimal role in the obesity care continuum. The aims of this study are to compare safety and efficacy of elective ESG versus laparoscopic sleeve gastrectomy (LSG) at 6 months using accredited MBSAQIP centers. METHODS: Data were obtained from the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) database for 2023 and merged with the 6‑month follow‑up d

Personality Traits Predict Weight Loss Following Endoscopic Sleeve Gastroplasty: A Cohort Study with External Validation.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is a safe and effective treatment for obesity, but weight loss varies widely among patients. Personality traits may influence behavioral adherence after bariatric interventions. We evaluated whether Big Five personality traits predict weight loss after ESG and validated findings across two demographically distinct cohorts. METHODS: Two retrospective cohorts were analyzed. The primary cohort included self-pay patients who underwent ESG at a tertiary center in New York (2013–2019). The external cohort included predominantly uninsured or underinsur

A Machine-Learning Assisted Genetic Risk Score Identifies Improved Weight Loss After Endoscopic Sleeve Gastroplasty.

BACKGROUND AND AIMS: Obesity remains an epidemic associated with significant health consequences. Endoscopic sleeve gastroplasty (ESG) can lead to significant and sustained weight loss, with heterogeneous response. We previously reported a machine-learning (ML) assisted genetic risk score (GRS) for calories to satiation that predicts response to anti-obesity medications. Here, we evaluated the performance of novel GRSs for emotional hunger (EH), and calories to satiation (CTS, also high or low CTSGRS) to predict weight loss after ESG. METHODS: Individuals treated with ESG at two separate endo

Applicability of individualized metabolic surgery score for prediction of diabetes remission after endoscopic sleeve gastroplasty.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is a safe and effective obesity treatment. The individualized metabolic score (IMS) is a validated score that uses preoperative variables predicting T2D remission (DR) in bariatric surgery. OBJECTIVES: We evaluated the applicability of using the IMS score to predict DR in patients after ESG. DESIGN/METHODS: We performed a retrospective review of patients with obesity and T2D who underwent ESG. We calculated DR, IMS score, and severity, and divided patients based on IMS category. RESULTS: The cohort comprised 20 patients: 25% (5) mild, 55% (11

Impenetrable Abdomen: A Case Report of Endoscopic Sleeve Gastroplasty Performed in Patient with Huge Paramedian Laparocele.

INTRODUCTION: Bariatric surgical techniques have demonstrated scientific efficacy and interventional safety; however, these approaches are not universally appropriate for all patients. The term "impenetrable abdomen" refers to the inability to achieve surgical access to the abdominal cavity, a condition that may arise from several underlying factors. Endoscopic sleeve gastroplasty (ESG) is proposed as a safe and effective technique in this subgroup of patients with obesity. MATERIALS AND METHODS: A 56-year-old Caucasian woman with an impenetrable abdomen due to the presence of a large left pa

Conversion of vertical banded gastroplasty to stand-alone sleeve gastrectomy or biliopancreatic diversion with duodenal switch.

INTRODUCTION: Vertical banded gastroplasty (VBG) originated as a simplified bariatric operation to avoid malabsorption and provide lasting results due to a fixed stoma. Short-term results were excellent (50-70 % excess weight loss); however, patients often displayed maladaptive eating behaviors, and many failed to either achieve or sustain adequate long-term weight loss. Complications were also common including severe reflux and regurgitation, gastric outlet stenosis or stricture, gastrogastric fistula, and breakdown of the staple line. METHODS: VBG conversions to Roux-en-Y gastric bypass or
