Esophageal Atresia: Migration of the gastrostomy tube into the bronchus.

A 2-day-old baby boy, 38 weeks gestation, weight 2000 g was brought due to hypersalivation and imperforate anus with gasless abdomen on plain X-ray. He underwent a gastrostomy tube insertion and colostomy. In contrast study of the stomach, on the 5th postoperative day, the dye spilled into the tracheo bronchial tree and the catheter was seen, entering the right main bronchus. The patient underwent right thoracotomy and the presence of fistula and catheter were confirmed. The fistula and distal esophagus were closed and fixed to the prevertebral fascia because of a long gap. He is under follow-

Early dislodgement of percutaneous and endoscopic gastrostomy tube.

One of the most serious complications of PEG tube placement is the inadvertent removal of the gastrostomy tube. This case report describes an elderly patient with a severe mental disorder who inadvertently pulled her PEG tube only hours after insertion and the subsequent therapeutic maneuvers to save it. If this occurs within 14 days of insertion, a mature tract has not adequately developed and blind reinsertion should not be attemped. In review of the literature, endoscopic replacement is preferred over celiotomy in the absence of peritonitis and this can be attempted early after dislodgement

Medical, surgical, and health outcomes of gastrostomy feeding.

A prospective controlled study with repeated measures before and after surgery examined the medical, surgical, and health outcomes of gastrostomy for children with disabilities at a tertiary paediatric referral centre in the North Thames area, UK. Anthropometric measures included weight, mid-upper-arm and head circumference. Five-day prospective food diaries were completed and data on physical health and surgical outcomes recorded. Seventy-six children participated and underwent gastrostomy (44 males, 32 females; median age 3 y 4 mo, range 4 mo-17 y 5 mo), and 35/76 required an anti-reflux pro

[Clinical value of percutaneous endoscopic gastrostomy and jejunostomy].

OBJECTIVE: To investigate the feasibility and safety of percutaneous endoscopic gastrostomy (PEG) and percutaneous endoscopic jejunostomy(PEJ). METHODS: From June 1996 to April 2005, clinical data of 121 patients treated with PEG or PEJ were analyzed retrospectively. RESULTS: A total of 121 patients experienced 134 times of PEG or PEG plus PEJ, including 90 cases (103 times) treated with PEG and 31 patients treated with PEG plus PEJ. Thirteen patients treated with PEG had fistula replacement 6-10 months after PEG. All patients had quick nutrition recovery after fistula tube insert, therefore

Gastrostomy, esophagitis, and gastrointestinal bleeding in older adults.

INTRODUCTION: Percutaneous endoscopic gastrostomy tubes (PEG) are commonly used in older adults with dysphagia and poor nutrition. The association of PEG with upper gastrointestinal bleeding (UGIB) and role of gastroesophageal reflux disease (GERD) in relation to UGIB in patients with PEG are not well known. METHODS: We conducted a retrospective analysis of older patients with PEG, hospitalized for UGIB, during a 1-year period between 1997 and 1998. The study was performed in a university teaching hospital involving residents from long-term care facilities (LTCF) of the Bronx. RESULTS: A tot

[Clinical study of percutaneous endoscopic gastrostomy in neurosurgery].

We investigated the efficacy of percutaneous endoscopic gastrostomy (PEG) in neurosurgery. PEG was applied 94 feeding patients using One-Step Button. The mean age was 70.8 +/- 15.4 years old, and the mean duration between the onset of swallowing disturbance and PEG was 3 months. Minor complication associated with PEG was 8 cases (8.5%), which treated conservatively. Body weight, BMI, and biochemical data were increased one month after PEG, so nutrition was improved. Aspiration pneumonia was improved in 9/18 patients. Fourteen patients got oral intake, and PEG was extracted in 10 patients. PEG

[Percutaneous endoscopic gastrostomy in patients with ENT tumors].

Oral food intake in patients with obstructing pharyngeal and esophageal carcinomas is commonly insufficient because of tumor-induced dysphagia which gives rise to cachexia unless treated. While entailing an unfavorable prognosis, malnutrition is often a therapy-limiting factor. Tube feeding with liquid formula diets currently offers the most efficient and least-risky approach to long-term use and is best adopted even at a pretreatment stage irrespective of the tumor therapy intended. A feeding tube placed by a percutaneous endoscopically controlled gastrostomy (PEG) increasingly offers an alte

Evaluation of percutaneous endoscopic gastrostomy in a university hospital.

BACKGROUND/AIMS: Percutaneous endoscopic gastrostomy (PEG) has become a commonly performed procedure to provide nutritional support for chronically ill patients. The aim of this study was to review the safety and results of PEG in a teaching hospital. METHODOLOGY: A retrospective review of 44 patients who underwent PEG procedure. The indication was long-term enteral feeding in patients who were unable to maintain adequate nutrition by mouth with an otherwise functioning gut. The most common primary diagnosis was cerebrovascular accident (17 patients). All patients were unable to swallow. RES

Percutaneous gastrostomy with gastropexy: experience in 125 patients.

We report our experience with radiologically guided percutaneous tube gastrostomy in 125 patients by using a gastropexy technique in which the anterior gastric wall is nonsurgically sutured to the anterior abdominal wall with percutaneously placed T-fasteners before catheter insertion. Short-term follow-up of up to 2 weeks was available in all patients. In 63 patients, long-term follow-up (greater than 4 weeks; average, 3.5 months; maximum, 1 year) was available. Catheter placement was successful in 124 (99%) of 125 patients, including three patients with anatomic changes after Bilroth II hemi

Gastrostomy and Nissen fundoplication in neurologically impaired children.

We report our experience with 90 neurologically impaired children treated with gastrostomy and Nissen fundoplication. Malnutrition was the main problem, followed by aspiration, recurrent pneumonia, and vomiting. The symptomatology was caused by swallowing incoordination and gastroesophageal reflux. The diagnosis of gastroesophageal reflux was confirmed by upper gastrointestinal series and pH probe. Nissen fundoplication was performed following a standard technique with preservation of the vagus nerves and its branches, repair of the diaphragmatic crura, reconstruction of the angle of His, and

Complications of endoscopic gastrostomy: pneumoperitoneum and volvulus of the colon.

Pneumoperitoneum caused by percutaneous gastrostomy usually follows a benign clinical course, but led to progressive volvulus of the ileocolic segment in the patient described here. This article explains relationships between pneumoperitoneum after surgical or endoscopic procedures and volvulus involving the intraperitoneally mobile intestinal loops.

Cerebral Air Embolism Occurring During Percutaneous Endoscopic Gastrostomy.

Air embolism is a rare but potentially fatal complication of gastrointestinal endoscopic procedures, primarily affecting the pulmonary circulation. Systemic air embolism, especially with cerebral involvement, is exceedingly uncommon. We report a case of cerebral air embolism that occurred during percutaneous endoscopic gastrostomy (PEG) using the pull method in a 65-year-old man with dysphagia due to progressive supranuclear palsy with frontotemporal dementia. Immediately after guidewire externalization, the patient experienced sudden respiratory arrest and hemodynamic collapse, necessitating

Case report: Dravet syndrome, feeding difficulties and gastrostomy.

Dravet syndrome (DS) is a developmental and epileptic encephalopathy associated with variants in the voltage-gated sodium channel alpha 1 subunit (SCN1A) gene in around 90% of individuals. The core phenotype is well-recognized, and is characterized by seizure onset in infancy, typically with prolonged febrile seizures, followed by the emergence of multiple seizure types that are frequently drug-resistant, developmental delay, and intellectual disability. Comorbidities are common and include autism spectrum disorder, gait impairment, scoliosis, and sleep disorder. Feeding difficulties and weigh

Ultrasound-guided gastrostomy tube placement: A case series.

PURPOSE: Gastrostomy tubes (G-tubes) can be placed utilizing a variety of techniques. Here we present a case series to demonstrate feasibility of a novel method, ultrasound-guided G-tube placement (USGTP). METHODS: All cases of USGTP at our institution from September 2015-August 2016 were reviewed. Data included demographics, operative time, complications, time to first feeding, and 30-day readmissions. All steps of the procedure were carried out using ultrasound guidance, resulting in placement of a low-profile G-tube. RESULTS: Twelve patients underwent USGTP. Median age at operation was 2.
