The present study was undertaken to examine a possible effect of aprotinin, a 6.5-kDa polypeptide with an inhibitory effect on proteolysis, on aminoglycoside nephrotoxicity. Experimental animals (female Sprague-Dawley rats, 175-200 g body wt) were treated for 4 days with 40 mg/kg gentamicin given ip at 12-hr intervals. Gentamicin (GM), an aminoglycoside antibiotic, can cause acute renal failure in humans and experimental animals. It has been proposed that lipid peroxidation may play a role in GM nephrotoxicity. Lipid peroxidation may play a role in the acute nephrotoxicity of gentamicin (GM). Copper (Cu) depletion is associated with reduced activity of certain antioxidant enzymes in tissues, and with kidney lesions and impaired renal function. Many risk factors associated with aminoglycoside nephrotoxicity have been identified in humans and experimental animals. The authors present five cases of severe retinal ischemia associated with gentamicin injection. Gentamicin application to the middle ear may relieve Meniere's disease, presumably by reducing endolymph secretion by the dark cells. To explore this possibility, the ears of adult cats were treated with daily intratympanic infusions of gentamicin until ataxia occurred. Six cases are described of Pseudomonas aeruginosa ulcerative keratitis in which antibiotic sensitivity studies demonstrate organism resistance to gentamicin sulfate but sensitivity to other aminoglycosides such as tobramycin and amikacin. In four cases, community-acquired infections represent the source of these ulcers. This paper documents the emergence of aminoglycoside resistance among Pseudomonas aeruginosa keratitis within the general community.

Thirty-eight patients with severe mixed bacterial infections were treated with clindamycin and gentamicin. In vitro, all staphylococci and streptococci (other than enterococci) and 96% of anaerobes were susceptible to clindamycin; all staphylococci and 92% of Enterobacteriaceae and Pseudomonas aeruginosa were susceptible to gentamicin.
