Single-Stage Corrective Osteotomies for Multiple Angular Deformities Around the Knee Joint with Patellar Instability in a Patient with Multiple Epiphyseal Dysplasia: A Case Report.

CASE: It is rare to find a case report on combined recurvatum and valgus deformity of the knee with associated patellar instability due to a depressed lateral tibial plateau in a young adult with multiple epiphyseal dysplasia. We treated such a patient with single-stage osseous and soft-tissue procedures and achieved a stable, well-aligned, pain-free, and mobile joint. CONCLUSION: Patellar instability with genu recurvatum and genu valgum leading to reduced range of motion of the knee in multiple epiphyseal dysplasia shows mechanical and functional improvement following single-stage surgical i

Distal Femoral Valgus and Recurrent Traumatic Patellar Instability: Is an Isolated Varus Producing Distal Femoral Osteotomy a Treatment Option?

BACKGROUND: Genu valgum, a risk factor for recurrent patellofemoral instability, can be addressed with a varus producing distal femoral osteotomy (DFO). The purpose of this study is to report 3-year clinical and radiographic outcomes on a series of skeletally mature adolescents with traumatic patellofemoral instability and genu valgum who underwent a varus producing DFO. METHODS: Consecutive patients (n=11) who underwent an isolated DFO for recurrent traumatic patellar instability over a 4-year study period (2009 to 2012) were reviewed. All patients were below 19 years of age, skeletally matu

Poor Prognostic Factors in Surgically Treated Habitual Patellar Dislocation in Children and Adolescents.

Background/Objectives: Habitual patellar dislocation is a rare but debilitating form of patellofemoral instability in children and adolescents, frequently associated with underlying anatomical abnormalities and ligamentous laxity. Despite multiple surgical techniques, recurrence and suboptimal functional recovery remain concerns. This study aimed to identify the demographic, clinical, and imaging factors associated with postoperative recurrence and poorer functional outcomes in pediatric patients surgically treated for habitual patellar dislocation. Methods: A retrospective cohort study was co

Bilateral Unfused Medial Process of Calcaneal Apophysis associated with Lower Extremity Malalignment: A Case Report.

INTRODUCTION: The calcaneal apophysis develops through a complex ossification process during childhood growth, with multiple secondary ossification centers emerging in distinct temporal and spatial patterns. Its ossification patterns, fusion process, and associated pediatric injuries and osteochondral conditions have been well documented in the literature. This report presents a previously unreported case of bilateral unfused medial process of calcaneal apophysis incidentally discovered in an adolescent patient during evaluation for genu valgum. We aim to describe this unique presentation and

Case report: rare skeletal manifestations in a child with primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) is uncommon among children with an incidence of 1:300,000. This diagnosis is often missed in children in contrast to adults where it is detected at a pre symptomatic stage due to routine blood investigations. Etiology of PHPT can be due to adenoma, hyperplasia or rarely carcinoma. CASE PRESENTATION: A 12 year old Sri Lankan girl presented with progressive difficulty in walking since 1 year. On examination she had bilateral genu valgum. Skeletal survey revealed valgus deformity of knee joints, bilateral subluxation of upper femoral epiphysis(SUFE)

Systematic Review of the Effectiveness of Transphyseal Screws for Correction of Coronal Plane Knee Deformities and Meta-analysis of Comparative Studies With Tension Band Plates.

BACKGROUND: Coronal-plane knee deformities, both genu varum and genu valgum, are common in pediatric orthopaedics and can be effectively managed in skeletally immature patients with guided growth techniques. Among these, percutaneous epiphysiodesis using transphyseal screws (PETS) and tension band plates (TBP) are widely used, yet their comparative effectiveness remains debated. METHODS: A comprehensive search of PubMed, Embase, and Web of Science (January 2004 to January 2025) identified 603 studies. After applying the inclusion criteria, 13 studies (8 noncomparative and 5 comparative) were

Ellis-van Creveld syndrome.

Ellis-van Creveld syndrome is a rare autosomal recessive disorder caused by mutations in the EVC and EVC2 gene (4p16), characterized by chondrodystrophy, postaxial polydactyly, ectodermal dysplasia and cardiac anomalies. We present the case of a 24 years old female patient with unaffected parents and an affected sister, with a personal history of surgically corrected postaxial polydactyly of both hands and atriventricular canal. Clinical features were: a marked acromesomelic short stature (135 cm height), narrow thorax, genu valgum, club feet, brachydactyly, malposed toes, hypoplastic nails an

Proximal Tibial Recurvatum-Valgus: Analysis and Treatment.

Genu recurvatum-valgus arises from the proximal tibia and poses challenges in its treatment. The etiology of the combined deformities can include physeal trauma (often unrecognized), iatrogenic injury, infection, tumor, Osgood-Schlatter syndrome, skeletal dysplasia, and ligamentous laxity. Both osseous and ligamentous contributions must be recognized for successful treatment. A graphical planning method identifies the true (oblique) plane of deformity. Surgical treatment options include epiphysiodesis to prevent progressive deformity, guided growth, opening-wedge proximal tibial osteotomy, and

Copping Coping With the Braced Limbs.

Harold Copping illustrated an edition of Charles Dickens' classic, "A Christmas Carol." One of the paintings of Bob Cratchit and Tiny Tim in the book shows Tiny Tim wearing braces on his lower limbs. Several diagnoses of Tiny Tim's ailment have been suggested in the past but based on the details of Copping's painting we suggest that he had rickets. The above-knee braces with uprights only on the lateral side of the limbs may have been given to prevent genu valgum, a common deformity in childhood rickets.

[Total knee arthroplasty in severe valgus deformity in a patient with achondroplasia].

The authors present the results of total knee replacement in a 66-year-old woman with achondroplasia. The condition was diagnosed on the basis of clinical and radiographic findings; molecular genetic examination confirmed that the patient was heterozygous for the G1138A mutation responsible for substitution of an arginine for a glycine residue at position 380 (G380R). The patient presented with an unusual finding of severe fixed 20-degree valgus deformity, so far not published, which did not conform with the clinical features of achondroplasia. The authors presumed that the deformity had devel

Autocorrection of inadvertently oblique tension band plates in guided growth: a torsional safety analysis.

OBJECTIVES: Oblique placement of tension band plates is a common surgical variance in guided growth, raising concerns about its clinical safety, particularly the potential for inducing iatrogenic deformities. This study aimed to evaluate the comprehensive impact of this plate autocorrection on limb torsion, as well as on coronal and sagittal plane alignment. METHODS: This retrospective study analyzed 56 children who underwent guided growth for genu varum or valgum. At treatment initiation and termination, we measured the plate-diaphysis angle (α), femoral/tibial torsion, and coronal/sagittal

Mobility in Hurler syndrome.

Hurler syndrome is an autosomal recessive metabolic storage disease. Hematopoietic stem cell transplant increases life expectancy, but the effects on associated musculoskeletal abnormalities remains unclear, and long-term data are limited. We detail the follow-up of 23 patients at a mean of 8.5 years after successful hematopoietic stem cell transplant. All patients underwent clinical examination at an annual multidisciplinary clinic. Serial radiological studies were reviewed to assess development and management of hip dysplasia and genu valgum. All patients demonstrated characteristic acetabul

Osteotomy of the knee: is there a seat at the table?

Osteotomy is an alternative treatment for unicompartmental osteoarthritis of the knee. In genu varum, a high valgus closing wedge osteotomy is preferred, with overcorrection by 5 degrees. Complications and poor outcomes can be overcome by incomplete osteotomy with careful preoperative planning, precise use of cutting jigs, compression fixation, and early mobilization and weight bearing. Patella baja is related to postoperative immobilization, and unicompartmental arthroplasties are more difficult to revise because of bone loss. Supracondylar femoral osteotomy is preferred for genu valgum using

Navigation as a predictor of soft tissue release during 90 cases of computer-assisted total knee arthroplasty.

Soft tissue balance during total knee arthroplasty (TKA) procedures may be challenging to achieve in knees with severe valgus or varus deformities. The purpose of this study was to evaluate the role of navigation in predicting soft tissue release during 90 computer-assisted TKA procedures performed within a 2-year period. Fourteen soft tissue releases were performed, 13 of which were for genu varum, including 9 deep medial collateral ligaments and 4 deep and superficial medial collateral ligaments, and 1 for genu valgum deformity. Forty-one cases were intraoperatively overcorrectable (45.5%),
