The cytopathology of reactions to ventricular shunts.

A common cause of malfunctioning ventricular shunts is the occlusion of either tip by a variety of normal or reactive tissues and foreign substances. A six-year-old girl with communicating hydrocephalus and a meningomyelocele, a 48-year-old man with an ependymoma and an 11-year-old boy with a pineal germinoma had multinucleated histiocytic giant cells and ependymal cells in cerebrospinal fluid obtained from their ventricular shunts. These cellular changes were interpreted as the cytologic counterpart of the foreign-body inflammatory reactions often seen histologically on occluded shunt tips. N

Giant cell arteritis in association with cerebral amyloid angiopathy: immunohistochemical and molecular studies.

Giant cell arteritis (GCA) usually manifests as a transmural vascular infiltrate of mononuclear and multinucleated giant cells (MNGC). We describe six patients with GCA associated with severe cerebral amyloid angiopathy (CAA), all with cerebral hemorrhage or varying degrees of cerebral infarct, and histological evidence of Alzheimer's disease (cortical CAA often predominating over senile plaques and neurofibrillary tangles). One case showed mostly cortical involvement (with old microhemorrhages), and the others were primarily leptomeningeal (with involvement of the underlying cortex and extens

Long-term effects of polyacrylamide hydrogel on human breast tissue.

Polyacrylamide hydrogel is an atoxic, stable, nonresorbable sterile watery gel consisting of approximately 2.5% cross-linked polyacrylamide and nonpyrogenic water. Polyacrylamide hydrogel is widely used in ophthalmic operations, drug treatment, food packaging products, and water purification. In the former Soviet Union, polyacrylamide hydrogel has been used in plastic and aesthetic surgery for more than 10 years, and Kiev City Hospital treats approximately 300 women a year for breast augmentation using the polyacrylamide hydrogel Interfall (Contura SA, Montreux, Switzerland). Capsule shrinkage

Evaluation of the inflammatory process, collagen production, and MMP-2 and MMP-9 expressions produced by Luffa aegyptiaca Mill using the subcutaneous rat implanted model.

The subcutaneous rat implanted model is a preclinical approach used in studies to characterize the histocompatibility of materials that could be used as biomaterials. Biomaterials are obtained synthetically or from the environment, and they can be used to treat or replace any tissues or organs that the body has lost. To execute their roles, the biomaterials must present any level of histocompatibility and a lower level of inflammatory reaction. This work aimed to evaluate some aspects of histocompatibility, such as the inflammatory process, collagen production, and MMP-2 and 9 expression as re

Silicone lymphadenopathy with concomitant malignant lymphoma.

Six cases of axillary lymphadenopathy induced by silicone elastomer following silastic finger joint arthroplasty have been reported; 2 cases had associated malignant lymphoma. This is the 3rd reported case of malignant lymphoma with concomitant silicone lymphadenopathy discovered 8 years after insertion of silastic finger joint arthroplasty for longstanding rheumatoid arthritis. Histology revealed a partially replaced reactive lymph node with immunoblastic lymphoma. Both sides contained single and clusters of multinucleated giant cells with silastic particles, as well as formed asteroid bodies

Larynx injected with polytef paste.

A patient with inoperable lung cancer developed left-sided laryngeal paralysis. Her dysphonia, difficulty with swallowing, aspiration of secretions, and diminished cough reflex were improved with intracordal polytef injection for the remainder of her life. The foreign body reaction to the implant showed giant cells, few lymphocytes, and no polymorphonuclear leukocytes. This reaction may be described as a bland, chronic type consistent with the age of the implant. No areas of florid, acute reaction were found. Extrusion of part of the polytef through the cricothyroid space was observed. There w

Histological Features of Delayed Foreign Body Granuloma With Epithelioid Histiocyte Aggregation and Eosinophilic Reaction due to Hyaluronic Acid Injection.

Background: Dermal fillers such as hyaluronic acid (HA) have been widely used in recent years as a less surgically invasive cosmetic treatment. Although delayed foreign body granuloma may occur as a rare adverse reaction after the procedure, detailed histological reports are still limited. When occurring on the buccal mucosa of the oral cavity, the histopathology may resemble some lesions of minor salivary gland origin due to the material properties of HA. Here we report a delayed foreign body granuloma associated with HA showing eosinophilic infiltration of the buccal mucosa, with characteris

Lip augmentation dermal filler reactions, histopathologic features.

Instances of perioral and labial foreign body reactions to a variety of injectable dermal fillers were selected from the oral and maxillofacial pathology and dermatopathology archives at Pacific Pathology Laboratory of San Diego with the objective being to engender a compilation of histopathologic characteristics that allow the pathologist to identify the inciting materials. All cases of foreign body reactions located in the lips and perioral regions were reviewed by four pathologists, retaining those cases with a history of injection lip augmentation as well as those with histologic features

Posterior chamber lens implantation. A clinicopathological report of a successful case.

This report describes the clinical and histopathological studies of an eye 13 months after successful extracapsular cataract surgery and J-loop-type posterior chamber lens implantation. The lens was of PMMA while the loops were made of polypropylene. The corrected visual acuity of the patient was 1.0 and clinically the eye showed no evidence of anterior uveitis. The lower loop was in the capsular bag while the upper loop was embedded in the ciliary body surrounded by chronic foreign body giant-cell inflammatory reaction.

Histopathology of necrobiotic xanthogranuloma with paraproteinemia.

Fifty-three cutaneous biopsies from 22 patients with necrobiotic xanthogranuloma (NXG) were reviewed. One or more biopsies from each patient displayed a typical pattern of palisading histiocytic xanthogranuloma with bands of hyaline necrobiosis. Multiple foam cells and Touton giant cells were present, and atypical, bizarre-appearing foreign body giant cells were characteristic. Cholesterol clefts were found in 18 specimens, and lymphoid nodules were found in 24, 8 of which had germinal center formation. Twenty-one specimens contained foci of plasma cells, and these were located perivascularly,

Multinucleation resets human macrophages for specialized functions at the expense of their identity.

Macrophages undergo plasma membrane fusion and cell multinucleation to form multinucleated giant cells (MGCs) such as osteoclasts in bone, Langhans giant cells (LGCs) as part of granulomas or foreign-body giant cells (FBGCs) in reaction to exogenous material. How multinucleation per se contributes to functional specialization of mature mononuclear macrophages remains poorly understood in humans. Here, we integrate comparative transcriptomics with functional assays in purified mature mononuclear and multinucleated human osteoclasts, LGCs and FBGCs. Strikingly, in all three types of MGCs, multin

Osteopontin is strongly expressed by histiocytes in granulomas of diverse etiology.

Granulomatous inflammation is associated with a variety of important pathologic conditions. Osteopontin (OPN), a ligand for the alpha v beta 3 integrin, is a secreted glycoprotein with a glycine-arginine-glycine-aspartate-serine cell-binding domain. In this study, we examined expression of OPN in 22 cases of granulomatous inflammation including cases of sarcoidosis, granulomatous temporal arteritis, histoplasmosis, rheumatoid nodule, granuloma annulare, erythema nodosum, granulomatous gastritis, foreign body giant-cell granulomatous reactions, and lipogranulomas. Strong expression of OPN mRNA

The effects of bulk versus particulate ultra-high-molecular-weight polyethylene on bone.

Fourteen mature New Zealand white female rabbits underwent implantation of a bulk pellet and of particulate (less than 1,000 micron) ultra-high-molecular-weight polyethylene (UHMWPE), through a drill hole in the proximal right tibia. The left tibia served as a drilled but nonimplanted control. The rabbits were killed after 16 weeks. Histologic examination of the bone-implant interface in the bulk UHMWPE group disclosed a fibrous tissue membrane with infrequent giant cell and histiocytic clusters at surface irregularities. The particulate group demonstrated positively birefringent UHMWPE fragme

Intraosseous hair-induced cyst-like lesion of the maxilla associated with trichotillomania: first reported case and review of the literature.

Heterotopic hair in the oral cavity is a rare condition, with possible etiologies including prior skin graft reconstruction, oral and maxillofacial trauma, or, in some clinical context, no identifiable cause. We report a case of a patient with trichotillomania who presented with a persistent sinus track opening between teeth #9 and #10 and a well-defined periradicular lesion in the area. Microscopically, it revealed a partially lined cyst-like entity containing terminal hairs, associated with a foreign body-type giant cell reaction. The patient admitted to a habit of repeatedly pulling and plu

Biocompatibility of carbon fibre and carbon fibre microparticles.

Carbon fibres are integrated by the organism without causing significant foreign body reaction, but even with normal tissue growth around and encasement of the individual fibres. However, integration of the fibres into the tissue is not as good when the fibres are coated with epoxy-resin. Within the course of several months, progressive rupturing of fibres occurs with implants of pure carbon fibres, but with the mechanical function, however, being taken over by the formation of a connective tissue structure resembling a ligament. Carbon fibre microparticles are absorbed either by macrophages o

Tubercular etiology in a pilonidal sinus of the forehead: truth or myth?

Pilonidal sinuses are a well-described surgical condition first recognised in the 19th century. They have most commonly been reported in the intergluteal cleft; involvement of other sites is limited to isolated case reports. Association of this condition with tuberculosis is rare and how the two conditions affect each other's causation is poorly understood. We report a rare case showing tubercular pathology in a chronic pilonidal sinus on the forehead of a young lady. A careful review of the lesion was required before labelling it as non-tubercular. The histopathological similarity between a f

Cholesterol Granuloma: Unusual Pitfall for Hodgkin Lymphoma Evaluation With 18F-FDG PET/CT.

Cholesterol granuloma (CG) is a foreign body-type granuloma that forms in response to cholesterol crystals. Its etiology and pathogenesis are unclear. 18F-FDG is not a tumor-specific agent. Fibroblasts, macrophages, and multinucleated giant cells also take up 18F-FDG. Like sarcoid granulomas or fibrous dysplasia, CG avidly takes up 18F-FDG and can mimic tumor involvement. We present 2 cases of histologically proven CG, which has been misinterpreted as active residual Hodgkin lymphoma lesion.

FNAC of gouty tophi--a case report.

A 52 yrs old male presented with multiple, painless, firm nodules over extremities, which were mimicking benign neoplastic lesion. Fine needle aspiration was performed from three such nodules which revealed chalky white aspirate. After staining the smears with H&E and Giemsa stains, smears show amorphous pink material, needle shaped crystalline structures, many macrophages and foreign body type giant cells. A diagnosis of gouty tophi was offered which was confirmed by histopathology and serum uric acid level.

[Pulmonary infiltrates in a 19 year old patient with dysuria and hypercalcemia].

A 19 year old patient presented with the typical constellation of sarcoidosis. In the presence of indefinable pulmonary infiltrates, hypercalcemia, raised angiotensin converting enzyme and even evidence of giant and epitheloid cell granulomas, cocaine abuse should be considered. Chronic inhalative cocaine abuse can cause foreign body associated granulomatosis of the lung and other organs. It is important to establish this differential diagnosis by confidential interview and systematic polarisation microscopy to detect foreign material in tissues: unnecessary therapies with potential side effec

Acute Joint Blockage due to Abrasion-Related Dislocation of a Silastic Radial Head Prosthesis: A Histological Examination after 14 Years of Durability.

The implantation of a radial head prosthesis can take place as a therapeutic option after radial head fracture. There are various implants for this purpose. Many studies and case reports about silastic radial head prosthesis implantation describe foreign body reactions with accompanying synovitis and poor functional results. A few studies have investigated the reason for the material failure and the accompanying synovitis. The case report presented shows an unusually long durability of an in situ 14-year silastic radial head prosthesis. 14 years after implantation, a previously full-time worki
